A biomimetic composite restoration is a dental restoration technique that aims to replicate the natural properties of the tooth both in structure and function by closely imitating the biomechanics, aesthetics, and layering of natural enamel and dentin.
Instead of simply “filling” a cavity, the biomimetic approach focuses on preserving maximum tooth structure and restoring the tooth in a way that mimics its original mechanical behavior. Root canal treatment completed under rubber dam isolation:

Locating the middle mesial canal in maxillary molars is always a challenge especially when calcified. It truly puts your endodontic skills to the test. But with the right irrigant, precise irrigation protocols, and a steady hand, even the most complex canal systems can be successfully negotiated.

Following RCT, the tooth was restored biomimetically using an all-ceramic E-max overlay for optimal function and aesthetics.
📌 Note: All-ceramic restorations may appear radiolucent on radiographs.

This case highlights the importance of individualised case selection in implant dentistry. Our patient, aged 81 with a significant history of cigarette smoking, presented with no absolute contraindications to implant therapy.

A Straumann implant was selected and placed following comprehensive risk assessment and treatment planning. While smoking and advanced age are considered relative contraindications, they do not preclude successful outcomes when managed appropriately.

Negotiating curved root canals is an art and requires careful technique to avoid procedural errors like ledging, transportation, or perforation. A small, flexible hand file (my personal favourites being a #8 or #10 K-file) is used with a watch-winding or balanced force motion to establish a glide path. Copious irrigation with sodium hypochlorite and the use of lubricants like EDTA gel help reduce friction and prevent instrument binding. Precurving files can aid in following the natural canal anatomy. Gentle, incremental advancement with frequent recapitulation ensures smooth negotiation while maintaining patency.
