A patient presented to me as an emergency case with severe naso-orbito-maxillary trauma associated with degloving nasal fractures and naso-orbito-ethmoidal fracture following facial injury caused by an elevator accident. Clinical and radiographic examination revealed comminuted nasal bone fractures with sagittal maxillary split fracture.
Open reduction and internal fixation (ORIF) were performed under general anesthesia. Nasal bone reduction was achieved using a traction technique involving a wire looped around a screw temporarily fixed to the nasal bone for controlled repositioning. Stabilization was completed using miniplates and screws, internal and external nasal splints were applied.
For management of the sagittal maxillary fracture, one 6-hole miniplate was placed transversely along the palatal aspect posterior to the incisive foramen, while an additional plate was fixed buccally approximately 5 mm above the incisor apices to enhance stability and maintain alignment.
Postoperative recovery was satisfactory, with favorable functional and aesthetic outcomes as seen at the later picture.
