A 65-year-old female has been planned for ORIF after fall on ground with locked compression plating LCP via antero-lateral approach.

As a diabetic patient she has a higher risk of complications with non-oprative management of this fracture. Therefore, bone graft along with bridging plate technique were used to achieve rigid fixation and enhance healing. Radial nerve was identified and protected.

Note that :

Radial nerve palsy is quite common and can be seen in about 15% of closed fractures (more common in distal humerus)

Neuropraxia most common injury in closed fractures, while neurotomesis in open fractures

Iatrogenic radial nerve palsy is most common in ORIF (10-20%)

 The good news is that 85-90% of cases improve with observation over 3 months , and spontaneous full recovery found at an average of 6 months.

A 30-year-old male with RTA and tibiofibular supra-syndesmotic fracture (42A3 - 44C1). He has been planned on ORIF with DCP of both the tibia and fibula.

The goal of stable anatomic reduction with the restoration of ankle mortise was achieved and maintained by a syndesmotic screw.

Avoiding further damage to the soft tissue along with anatomical reduction of the ankle joint is the crucial factor for good outcomes.

Note that the use of 2 incisions for the tibia and fibula fixation to avoid synostosis and 7 Cm at least between the two incisions. Suturing with Allgöwer-Donati sutures is useful to improve incisional perfusion.

An 8-year-old female with a long history of painless head rotation and Rt side neck mass. Examination revealed Rt side SCM shorting with ipsilateral neck tilt and contralateral chin tilt.
She has been planned on bipolar release of SCM because of the failure of passive stretching.

Note that facial asymmetry can improve as long as the release is done before 10 years of age.

An 18-year-old female presented with severe pain in the right hand with the presence of multiple big foreign body injuries (thorns).

After proper analgesics with paracetamol and Strong opioids, her wounds were explored under local anaesthesia and digital block in the ER.

Fortunately, Removal of the thorns was achieved, soft tissue was preserved with full ROM of the joints, and proper wound irrigation was performed.

Note that it's important to encourage early movement and follow-up with proper antibiotics according to the local guidelines.

A 22-year-old male with a 4-month history of left wrist trauma and scaphoid fracture has developed avascular necrosis of the proximal pole. He has been planned on ORIF with a Vascularized bone graft.

A vascularized corticocancellous bone graft from the dorsal distal radius (Zaidemberg) was used.

Note that some of the risk factors for non-union this patient had is
● proximal pole fracture
● vertical oblique fracture pattern
● displacement >1mm
● smoking

One of the most common orthopaedic injuries. Distal radius metaphysis fracture with dorsal displacement (Colles fractures).

19-year-old male has been planned for operative fixation of Lf Colles fracture after failure to be stabilized with split.

Proper close reduction was achieved, and fixation with 3 K-wike technique was used for stability. In addition, Dressing and POP- Cast were applied.

Note that impacted fracture shortening of the radial metaphysis results in relative ulnar overlength, painful ulnar impaction may occur.

A 17-year-old male presented after a fall from height with (Pilon fracture) 43-A1 and 4F3-A3 , Ruedi and Allgower Classification type II.

He was planned for 2 incisions approach for ORIF of both tibia and fibula.

Compression plates with proper contouring were used to achieve almost perfect reduction and compression at the
site of the fractures.

A 15-year-old male presented with a Rt femoral fracture after a high-speed motor vehicle accident.
32-A1 fracture was managed by ORIF with the available Compression plate and screws.
Note that a Limited contact dynamic compression plate or LCDCP was useful to reduce the footprint of the bone.

A 4-year-old girl with a history of Congenital Muscular Torticollis has been planned for the bipolar release of SCM after a failure of conservative treatment by passive stretching for more than one year.

We performed two incisions (a single incision to reach the sternal and clavicular heads, and another proximal incision behind the ear). Then proper surgical release of SCM has been achieved.

Note that facial asymmetry can improve as long as the release is done before 10 years of age.
