A young patient was seen in the clinic for blackish discoloration on dorsal tongue. The lesion was asymptomatic and noted incidentally. The patient's medical history was negative for any significant diseases. Based on the clinical presentation, a diagnosis of physiological pigmentation was made.

Physiological pigmentation is a common phenomenon seen in dark-skinned individuals that results from an excessive expression of melanin in body tissues. The expression can vary in color from brown to black and can be influenced by external factors, such as smoking. Gingiva and the tongue are more frequently involved oral sites. Since the pigmentation is not a pathological process, no treatment is required. The patient may need to be reassured though.
