Bleeding risk, cancer, and atrial fibrillation, what do we know?

A connection exists between cancer and atrial fibrillation (AF) as cancer is statistically associated with AF within the first 3 months after a cancer diagnosis.

A diagnosis of AF carries a risk of a cancer diagnosis as well.

Patients with cancer also have an increased risk of developing venous thromboembolism (VTE) and major bleeding, especially when they begin anticoagulation therapy.

While specific scores are available to evaluate VTE risk in cancer patients, the standardized risk models available for AF (CHA2DS2-VASC and HAS-BLED score) do not consider the presence of malignancy.

What is the use of direct oral anticoagulants in pediatric cancer patients?

Most children with cancer-associated thrombosis are still treated with low-molecular-weight heparin.

Unlike adults, direct oral anticoagulants (DOACs) have not been approved in the pediatric population until recently, when rivaroxaban was approved for pediatric use by the EMA and the FDA.

The phase III Einstein Jr multicenter randomized controlled study (NCT02234843) showed that rivaroxaban is safe and effective in children with VTE.

The DIVERSITY study (NCT01895777), compared the efficacy and safety of dabigatran etexilate to the standard of care in pediatric patients with VTE. The results indicate that dabigatran was non-inferior to the standard of care in terms of efficacy and might be a suitable alternative to care standards. However, in this study, there was not a represented population for cancer patients.

Therefore, the use of DOACs in children will certainly increase in the future, but it will take time since many drugs are not registered yet for pediatric cancer patients.

What is the thrombosis risk in patients with ovarian cancer undergoing neoadjuvant chemotherapy (NACT)?

Ovarian cancer carries a high risk of venous thromboembolism (VTE) events, and chemotherapy is a well-known significant risk factor for VTE.

In two retrospective cohort studies of patients with ovarian cancer receiving NACT, VTE risk during the primary treatment period was 18โ€“27%.

A total of 8โ€“12% were diagnosed with VTE during NACT, 3โ€“6% during the immediate postoperative period following interval debulking, and 3โ€“10% during subsequent adjuvant chemotherapy.

The risk in women undergoing NACT might be higher also because of the higher cancer and/or comorbidity burden that prompts the decision to defer primary surgery.

What information should be given to patients with cancer, and when should it be given on the cancer care pathway?

Patients with cancer are going through a particularly uncertain period.

Together with the uncertainty about their future, they must deal with uncertainty resulting from the COVID-19 pandemic, the risk of infection, and difficulties in everyday treatments and checkups.

Due to the COVID-19 pandemic in Europe:

- An estimated 1 million cancer cases could be undiagnosed

- An estimated 100 million cancer screening tests were not performed, leading to later stage diagnoses and decreased overall survival

- Up to one in two people with potential cancer symptoms were not urgently referred for diagnosis

- One in every five cancer patients is currently still not receiving the surgical or chemotherapy treatment they need.

In addition, COVID-19 and cancer are both risk factors for VTE.

For this reason, in this pandemic period, it is even more important to raise awareness about the risks of VTE in patients with cancer.

But what information should be given to patients with cancer, and when should it be given on the cancer care pathway?

Currently, the provision of patient information about the risks, signs, and symptoms of CAT is inadequate.

Patients and clinical staffโ€™s poor knowledge about CAT lead to delayed presentation, diagnosis, and treatment of VTE.

Do you know the symptoms of blood clots?

Recognise the signs of a pathology is essential to protect ourselves.

Blood clots can happen in a personโ€™s leg or arm, but also in the lungs. In this last case it is called pulmonary embolism, or PE.

PE can be deadly and sudden death is the first symptom in about 1 over 4 people who have a PE.

One person every three people with blood clots will have a recurrence within 10 years.

This is why it is so important to recognise the symptoms of blood clots.
