🇮🇹Alcuni estratti del trattamento osteopatico di un ciclista in preparazione alla gara di MTB Granfondo 🚵‍♂️

RAZIONALE TRATTAMENTO
1) Riequilibrio funzionalità scapolo-toracica per un miglior assetto sulla bicicletta in termini di ergonomicità. Come? 🤔 Trattamento disfunzioni sul rachide dorsale e ripristino mobilità articolare.
2) Riduzione carico compressivo in cervico-dorsale per favorire la risposta alle continue sollecitazioni. Come? 🤔 Trattamento a monte e a valle delle disfunzioni cervicali e dorsali attraverso tecniche dirette.
3) Riequilibrio catene muscolari > arto inferiore destro. Come?🤔 Rilascio dei tessuti muscolari, manipolazione articolazione tibio-tarsica della caviglia, manipolazione tratto lombare

"Moby Dick" di Herman Melville è in assoluto il mio romanzo preferito. Vedere così da vicino i giganti del mare è stato per me un sogno che si realizza.
Di seguito una citazione del libro che descrive alla perfezione il mio bisogno di avvicinarmi al mare ogniqualvolta il mio spirito me lo chieda..
"Chiamatemi Ismaele. Qualche anno fa – non importa quando esattamente – avendo poco o nulla in tasca, e niente in particolare che riuscisse a interessarmi a terra, pensai di andarmene un po’ per mare, e vedere la parte equorea del mondo. EBM and TESTS in CLINICAL PRACTICE 👨‍⚕️

☝️We often find ourselves witnessing a struggle between the market and appropriate responses to health problems.

➡️ Every therapist must have adequate tools to be able to critically interpret the results of scientific research, translate his knowledge into appropriate approach, and measure the impact of his activity.

🔝EBM, from this point of view, should provide a framework capable of getting used to confronting explicitly the limits of knowledge.

✔️I consider osteopathy an art, but "ars" and "evidences" still need integration.
***
✋We are all well aware of the difficulties that EBM has encountered in medicine and even more in manual therapy.
But in a reality where the goal is a close collaboration with the scientific community and other therapists, we cannot forget that osteopathy is an art, but an art based on solid foundations: anatomy, physiology, pathology, biomechanics. ..

⚠️The relation between abnormalities in the lumbar spine and low back pain is controversial.

📖Already in in in 1994 Jensen M. Et al. performed MRI examinations on 98 asymptomatic people. Given the high prevalence of disc degeneration they came to the conclusion that the discovery by MRI of bulges or protrusions in people with low back pain may frequently be coincidental.

⬆️According to Brinjikji W, et al. the prevalence of disk degeneration in asymptomatic individuals increased from 37% of 20-year-old individuals to 96% of 80-year-old individuals.

👉According to Chou R, et al. Lumbar imaging for low-back pain without indications of serious underlying conditions does not improve clinical outcomes. Therefore, clinicians should refrain from routine, immediate lumbar imaging in patients with acute or subacute low-back pain and without features suggesting a serious underlying condition.

👍CONCLUSION: These data suggest that therapists require to refrain from using imaging as a routine exam in patients with low back pain, unless there are elements that suggest a serious underlying condition. In fact, pain is an experience associated with actual tissue potential damage but includes emotional, sensory and social components. Imaging is only a piece of the puzzle, positive outcomes must be contextualized in the patient's anamnesis and symptomatology without forgetting that they are not always related to pain.
