La stragrande maggioranza dei pazienti ha paura della vicinanza emotiva. Il chiederci aiuto è una forma di vicinanza emotiva e per questo attiva in loro emozioni connesse a traumi relazionali, il che mobilita l'ansia e le strategie di evitamento della stessa (ovvero la resistenza). Come psicoterapeuti ci può capitare, per questo, di accogliere persone restie a dichiarare la loro sofferenza interiore: clienti che affermano di NON avere alcun problema, che il problema ce l'ha il loro partner, clienti che sono vaghi, che tergiversano o che chiedono a noi quale sia il motivo per il quale LORO ci hanno cercato. Alternativamente possiamo incontrare clienti che non possono fare alleanza con noi perché sono talmente confusi dall'ansia da non riuscire a prestare attenzione al mondo esterno, o perché sono terrorizzate da noi. Attraverso la visione di filmati esploreremo modi di gestire alcune tra le principali resistenze alla costruzione dell'alleanza.

How to clean up the countertransference in ISTDP: dealing with overwhelming feelings when you practice ISTDP

Desensitize to the mixed feelings technique:
1) Select a patient with whom you have a hard time.
2)Select a brief clip that trigger affection in you
3) Select a brief clip that trigger frustration/anxiety/self-attack
4) Watch the first clip in loop until you can feel the affection in your body
5) Watch the second clip in loop just paying attention to your own triangle of conflict until you have a rise of anger
6) Pay attention to your anger experience bodily
7) Recall the affection feeling and pay attention to both the affection and the anger in your body

Dear colleagues, Johannes Kieding just published a video in which we role-play a therapy session. The purpose is to demonstrate principles and techniques used to deal with a provocative and defiant patient. Johannes plays the role of therapist, while I play the role of patient. In the final part of the video, we discuss subjective experiences, theoretical principles, interpersonal skills and inner skills involved in the process.
The learning goals are:
1) how to deal with persistent defiance by recognizing UTA even when it is mixed with toxic defenses;
2) how to indirectly disconfirm projections;
3) how to prevent an interpersonal conflict that would keep the patient from experiencing his inner conflict
