Over-pronation has been found to be the a common cause of this Achilles tendonitis. Excessive rolling of the foort inwards cause our lower leg to rotate internally. This puts a shearing force on our calf muscles which are connected to the Achilles tendon. Now this is where the Achilles tendon gets over-stretched, which results in inflammation.

Patellofemoral pain syndrome is characterized by pain around and behind the kneecap, especially when climbing stairs, going from sit to stand and vice versa or when sitting with the knees bent for a prolonged period of time.

Three factors contribute to this syndrome and they are:

- a muscle imbalance in the quadriceps
- weak gluteal muscles
- overpronation in the foot

What is metatarsalgia? This is pain to the metatarsal heads, which are the ends of the metatarsal bones that make up the ‘ball’ of the foot. This pain is usually felt on the bottom of the foot and is aggravated by walking or weight-bearing. This condition may develop in people who spend extended periods of time standing or walking, or in those who participate in high impact sports such as running or basketball.

Certain conditions, such as over pronation, lack of big toe joint function, can lead to overload of the smaller metatarsal heads; since the smaller metatarsals are not designed to bear this much weight, inflammation and pain may result.

A custom foot orthotic is beneficial in that it will reduce pressure and loading to the smaller metatarsals heads, effecting transferring pressure back to the first (and biggest) metatarsal, where it should be. Also, support of the foot arch and full contact of the orthotic to the entire bottom of the foot disperses pressure away from the painful metatarsals, reducing tissue stress and allowing for healing.

Heel Spurs = Plantar Fasciitis gone bad. Heel spurs are a result of the tight fascia pulling on its' origin on the calcaneous. Bone under stress responds with a spur.

There are so many patients with spurs in Jamaica than in Canada. I think it may be because in Canada, patients spend more time in closed, more supportive shoes than patients in Jamaica do.

The treatment for Heel Spurs is exactly the same as it is for Plantar fasciitis. This cludes stretching of the calf muscles and the quadratus plantae, rolling of all the soft tissues of the foot as well as strengthening for the muscles of the foot and ankle.

Arch support and footwear must also be addressed as the soft tissues of the foot, ultimately, need support. In many cases, over the counter orthotics can work. Sometimes, they are not enough and people need to get custom made orthotics.
