13. Full capability to perform many invasive & non invasive cardiac procedure as:
A. Trans thoracic echocardiography. Trans esophegial echocardiography. Implantation of a permanent
Pacemakers (single & dual chamber ). Implantation of ICDs ( intra cardiac defibrillator ). E. Implantation of CRT devices (with or without ICD). SKILLS & ABILITIES:
During my work as consultant interventional cardiologist at the International Medical Center, I was able to expand the service by opening a new Cath lab & introducing the most recent techniques in interventional cardiology as follows:
-The IMC become the first center in the Middle East using the radial approach as routine and doing more than 90% of the cases including the complex PCI. The rate of crossover is 5-10% & the complication rate is 0.002.
-All the cases are included in the 2 famous registries: coronary angioplasty registry & evaluation in Saudi Arabia (CARES) & Saudi cardiovascular data registry (S-CDR). CARES will be published in the Canadian Journal of Cardiology by year 2010.
-IMC is considered as an obesity management center so a considerable number of the patients are morbidly obese & I was able to develop an appropriate technique to cath this difficult group of patients.
-I introduced the new service of using IVUS & Pressure Wire in the cath lab, making the IMC the first center to use them in the privet sector in the western province in Saudi Arabia.
- Doing the complex cases as unprotected LM, CTO & bifurcation lesions, Kissing & crushing techniques, multi-vessel PCI, LIMA & SVG PCI via the radial approach. During my work as consultant interventional cardiologist in PSCCQ since 1st/ Jan/2008, there were 34 to 45 patients per month admitted under my care. I did cardiac catheterization for 40 to 60 cases per month,(28 to 38 interventions/month, 14 cases as primary PCI/month) . I do the PCI cases via different approaches: femoral, Rt. & Lt. Radial with best utilization of IVUS assessment pre &/or post stenting, use of pressure wire, IABP. The cases that were done included: unprotected LM, CTO & bifurcation lesions, Kissing & crushing techniques, multi-vessel PCI, LIMA & SVG PCI. I have done my training program in adult cardiology in PSCC. PSCC is the cardiac center of the military hospital in Riyadh; it is a tertiary hospital, which receives cases that need cardiac intervention from all around the kingdom. PSCC has 168 inpatient beds, CCU with 24 beds and 5 cath labs. Utilizing these facilities, I was able to do the following:
1. More than 1600 cases in the cath lab: as first operator about 600 cases diagnostic and hemodynamic study, as second operator more than 500 cases interventional therapy, and as first operator about 400 cases interventional therapy of which 150 cases primary PCI and many cases needed further evaluation with IVUS. The interventional cases included direct stenting, dilatation and stenting, SVG and LIMA stenting, kissing, crushing and modified crushing techniques.
2. About 1500 echocardiogram (ECHO) report, more than 300 cases were complete study of hands on and report.
3. About 300 cases ETT and nuclear studies.
4. About 200 cases EPS, PPM and ICD implantation as a second operator.
5. All CCU and ICU procedures as intubations, arterial lines, central lines, intra aortic balloon pump and all kinds of TPMs, plural and pericardial tap.
6. Taking care of patients in hospital in the CCU and the adult cardiology ward.
7. The out patient clinics: as general cardiology, heart failure and cardiac transplant, and pacemaker clinic.
8. Rotation in referral and cardiac consultation in all other departments in the military hospital (which includes about 1080 beds) doing the pre-surgical cardiac assessment for non-cardiac surgical procedures.
9. Rotation in A & E which include 42 beds
10. I had the chance to join Professor Hetzer in Berlin heart center to have an overview on their management of heart failure by using INCORE and EXCORE assistant devices and cardiac transplant for 2 months. RESEARCHES:
