Payment: You are not expected to provide payment until after each session. I am able to accept cash, check, card (this has a small additional charge), venmo, or zelle. I can also provide an itemized receipt for your payment.

Hours: My current hours for booking are from 10am-2pm Monday-Friday. I am willing to be flexible with these hours as needed. Feel free to reach out to me via Facebook messenger or email to schedule a time outside these hours if needed.

Pricing: All of my pricing is based on national averages for what the billing codes in physical therapy are typically reimbursed. A billing code is a specific code in physical therapy that is used to determine what services were provided and is billed based on a specific amount of time spent on that service. I believe in keeping all my pricing fair so the initial evaluation is a fixed price for all and the prices for each follow up session is based on fixed pricing of each billing code. An example of a billing code includes manual therapy, which can include message type techniques, in one session I may perform at least 8 minutes of this technique leading to billing one unit of manual therapy that is reimbursed at $30. To continue this example if you do a one hour session then I will charge 4 units total based on the time. This could mean 1 unit of manual therapy (1 unit equal to $15) and 3 unit of therapeutic exercise (1 unit equal to $17) which would equal a total of $66 for the session. Each follow up treatment session varies in price due to the various billing codes that are appropriate to provide the necessary care during the session. The lowest a follow up session could cost is $60 and the highest a follow up session could cost is $100. Below is listed all fixed costs for the initial evaluation and all billing codes.

Tricare information: Please be aware that I am considered as an out of network provider by Tricare overseas. A referral from your primary care physician is not necessary to seek care from Taking Flight Physical Therapy. When seeking care without a referral you are using the point of service option from Tricare. The point of service option means that you are paying the provider directly for their services instead of paying a co-pay with insurance covering any additional fees.

Hey everyone!

My name Is Roxann Lubiarz and I am licensed physical therapist! I am excited to be able to start providing care to the Misawa base community. Here are a few things to get to know me a little more!

I am married to an F-16 pilot who is stationed here as part of the 13th fighter squadron (CP!) and this is our first overseas station. We have a two year old Weimerainer named Nova who is a ball of energy and smart as can be! My husband and I are both from Las Vegas and were stationed In Texas before arriving here.

I graduated in May 2022 from University of Southern California with my Doctor of Physical Therapy and I am licensed through the state of Nevada! Even though I am a new graduate I have had experience treating patients with general orthopedic conditions, vestibular conditions, pelvic floor dysfunctions, neurologic conditions, and treating kiddos of all ages from birth to school age! I truly have a passion for helping people get back to life after an injury, working with kiddos to reach their milestones, and helping with pain management.

My services through Taking Flight Physical Therapy is out of network from Tricare so everything is cash based at this time and does not require a referral from your primary care provider. That being said, treatment will start with an initial evaluation where we will work together to create an individualized plan of care tailored to your needs!
