A Neurological disorder from acute rheumatic fever, causing involuntary movements and weakness in children (5–15 years).

Causes: 👇

Autoimmune response post-group A streptococcal infection; antibodies attack basal ganglia.
👉 Risk factors: untreated strep infections, genetic predisposition.

Symptoms:👇

👉 Involuntary jerky movements (chorea), muscle weakness, poor coordination, unsteady gait.
👉 Emotional lability, behavioral changes, speech difficulties.
👉 Worsens with stress/fatigue; improves with rest.

Physiotherapy Management:👇

✅ Goals: Improve motor control, strength, coordination, function; prevent complications.

✅ Assessment: Evaluate movement, strength, balance, daily activities.

✅ Interventions:
👉 Gentle stretching, coordination tasks (e.g., reaching).
👉 Balance/gait training with assistive devices if needed.
👉 Low-resistance isometric exercises.
👉 Weighted vests, deep breathing for sensory/relaxation.
👉 Task-specific training for daily activities.
👉 Educate caregivers on safe handling, home exercises.

✅ Considerations: Individualize; avoid overexertion; coordinate with medical team (medications primary).

✅ Precautions: Prevent falls; avoid high-intensity activities.

✅ Outcomes: Supports recovery; symptoms typically resolve in 6–12 months.

🦴 Osteoporosis definition

Osteoporosis is a disease characterized by a decrease in bone mass that occurs when the rate of bone resorption exceeds the rate of bone formation, a common occurrence as the body ages.

🔄 Difference from Paget’s disease

In Paget’s disease, new bone is formed in an attempt to keep up with the resorption by the overactive osteoclasts, but that new bone is produced haphazardly.

Osteoporosis does not have the elevated blood levels of alkaline phosphatase found in Paget’s disease.

📍 Commonly affected bones

Proximal ends of the femur, vertebrae, and wrist.

Something as simple as a sneeze can cause a vertebral fracture.

When an elderly person falls and breaks a hip (femur), it is very likely the femur that broke first, resulting in the fall.

🔬 Histological features

Reduction in the thickness of compact bone.

Reduction in the number and size of trabeculae in cancellous bone.

📊 Bone mass loss with age

Females lose bone mass more quickly than males starting at about 50 years of age.

Age 50 is the approximate age of menopause.

Ovaries reduce in size and cease the production of estrogen.

Estrogen promotes osteoblastic activity and production of bone matrix.

Osteoporosis is more common in females, but males can develop it, too.

👨‍👩‍👧 Risk factors & prevention

Family history of osteoporosis increases risk.

Best treatment is prevention.

Childhood diet with adequate calcium and vitamin D.

Lifestyle including weight-bearing exercise.

Promoting proper nutrition and exercise early in life can maximize bone mass before age 30, reducing risk.

⚠️ Complications of hip fracture in elderly

Fracture itself may not be serious, but immobility during healing can lead to:

Blood clots → respiratory failure.

Pneumonia due to poor air exchange.

Pressure sores allowing pathogens to cause infections.

Urinary tract infections from catheterization.

The disc is normal, well hydrated, and very bright on T2.

🔹 Grade II
Mild early changes, still bright on T2, nucleus clearly visible.

🔹 Grade III
Gray signal, moderate loss of hydration, unclear nucleus-annulus border.

🔹 Grade IV
Dark signal, significant dehydration, and reduced disc height.

🔹 Grade V
Collapsed disc, very dark signal, and severe degeneration
