Close Shaves, Lasting Scars:
Let’s Talk About Acne Keloidalis Nuchae (AKN)

AKN is a chronic skin condition that mostly affects skin of color — and it’s more common than you think.

What is it?
AKN causes small bumps at the back of the scalp or neck that can turn into thick, itchy scars over time. It’s not regular shaving bumps.
It can lead to long-term severe scarring, hair loss, and a drop in the quality of life in a person.

Why are some individuals affected while others are not?

● Coarse, curly hair is more likely to curl back into the skin, leading to inflammation.

● Close shaving — especially with razors or clippers — can trigger or worsen the condition

● Lack of awareness often means late diagnosis and more severe scarring

Prevention & Care Tips
● Avoid close shaves — consider longer fades or protective styles.

● Minimize friction from collars, hats, or helmets.

● Avoid self medication or non clinical diagnosis, especially from friends

● See a dermatologist as soon as possible

Don’t ignore the signs — they usually present as itchy bumps often misdiagnosed as fungal infections, especially when one opts to seek medical assistance from non qualified sources.

When we look at our feet we often neglect the few things we observe and brush them of as they are not painful, itchy or bothersome. When we google symptoms, many skin of colour diagnoses are under represented and often information on the internet may not be enough.

Acral lentiginous melanoma (ALM) is a rare subtype of skin cancer specifically a melanoma arising on the palms, soles, or under the nails. It is also known as acral melanoma and is the most common subtype of melanoma in darker skinned populations, accounting for 40–60% of melanoma diagnoses in Asian and African-American ethnicities

ALM In early stages often appears as a dark brown or black macule with variegated colour, and can be nodular and ulcerated in advanced cases. ALM is slow-growing, and can take months to years to become invasive and spread to organs in the body

The diagnosis is often made on clinical history and presentation, supported by dermoscopy. A skin biopsy is always confirmatory. The outcome may be complete clearance is brought to attention early or in later stages death.

Of note in history this is what lead to the early demise of Bob Marley whose music echos till date.

Pityriasis versicolor or Tinea Versicolor is a common yeast infection of the skin, in which flaky discoloured patches appear on the chest neck and back.

It comes under the blanket of fungal skin infections and is more common in hot, humid climates than in cool, dry climates. It often affects people who perspire heavily. It may clear in the winter months and recur each summer. It is also seen in people who have dry skin and who don't moisturize enough. In our darker skin types we have also noticed a common link with stress and hence mostly starts occuring around school going years and persists in young adulthood.

Pityriasis versicolor affects the trunk, neck, arms, and commonly the back. The patches may be brown to whitish than surrounding skin. Sometimes the patches are associated with a white scaly and may be mildly itchy.

It is often mistreated by over the counter medications hence it recurrs. Or often misdiagnosed and treated as Vitiligo

The treatment is usually a combination of oral and topical medications decided by a dermatologist based on severity and extent of involvement.

Skin tags are commmon benign tumors of the skin often seen around the neck and armpits in especially patients who have friction in these areas

They commonly occur in creases or folds of the skin.

They are not dangerous, but they can be removed for aesthetic and cosmetic reasons. They may at times also appear in patients who have uncontrolled diabetes.

Methods of skin tag removal include over the electrocautery excision, and cryotherapy depending on the number and areas involved

The line between aesthetic procedures and beauty treatments is often blurred.

Aesthetic treatments like botox, fillers and thread lifts are MEDICAL treatments and should only be performed by appropriately trained medical clinicians such as doctors and dentists 🩺

But due to the dire lack of regulation in aesthetic medicine in the UK, you don't need to be medically trained to offer aesthetic treatment. That means anyone can legally buy filler or Botox and inject others.

So, if you're considering having an aesthetic procedure like Botox or fillers, you need to do your due diligence.

All medical treatments come with risks, but in untrained hands, those risks are significantly higher.

You only get one face. Keep it safe by seeing a well-trained medical practitioner who is thoroughly educated in facial anatomy and techniques for injecting different types of fillers.

And similarly, there is currently no set training standard for using laser or IPL machines, which can be very risky in the wrong hands.

These treatments should be obtained only from doctors, nurses or therapists trained to use them in an appropriate healthcare environment rather than a beauty salon.
