Initial treatment would include aspirin and in a hospital with Cath Lab facilities urgent restoration of blood flow in the blocked artery using minimally invasive techniques e.g. Balloon Angioplasty and Stent. In hospitals that do not have Cath Lab facilities (this includes most hospitals worldwide) a ‘clot dissolving’ medication may be injected after assessment of bleeding risk.

The primary goal of treatment is to restore blood flow as soon as possible to reduce the risk and extent of heart muscle damage. This also reduces the incidence of complications such as sudden death and heart failure. Presenting yourself immediately for clinical evaluation and an ECG to a Centre with experience in handling such cases is of paramount importance. Time to presentation to a health facility has a major influence on clinical outcome and success of treatment. The ECG is central to the diagnosis of a heart attack and should be obtained rapidly. If local interpretation is not possible, transmission of that ECG to a competent unit should be done immediately.

Chest pain or discomfort is characteristically in the centre of the chest and can radiate to the jaw and to the arms. There is often associated sweating and a sensation of shortness of breath, nausea and vomiting can also occur. Presentation may be less typical and have only shortness of breath, nausea or vomiting. A total occlusion will often lead to persistent chest pain (over 20 minutes). Partial occlusion of the artery will often lead to intermittent chest pain. Both sets of symptoms may represent heart attacks.

A Heart Attack is said to occur when heart muscle is damaged as a result of total or partial occlusion by a clot in a coronary artery. The degree of heart muscle damage varies depending on the extent of muscle supplied by the culprit artery.
