Your patient with multi-system trauma has an ETCO2 = 22 mm Hg and a normal blood pressure. What could be happening?

A newly published study from the Journal of Trauma and Acute
Care Surgery provides great insight into another great use of end-tidal CO2 in trauma patients.

This was a retrospective, observational cohort study evaluating
trauma patients who were intubated and ventilated by Seattle Fire Department paramedics in the prehospital setting as part of their resuscitative care. The study EXCLUDED patients who were:
1. Younger than 18 years,
2. Not intubated with an ETT
3. Did not have continuous ETCO2 waveform data
4. Could not be matched with the in-hospital trauma registry based on the available demographic data.

The primary outcome was hemorrhagic shock, defined as transfusion of at least 1 unit of any blood component in the ED along with either an initial ED systolic BP less than or equal to 90 mm Hg or an initial ED shock index greater than 0.9.

What were the results?
Patients in hemorrhagic shock had lower median ETCO2 values (26.5 mm Hg vs. 32.5 mm Hg; p < 0.001) than those not in hemorrhagic shock. Patients with prehospital ETCO2 less than 25 mm Hg were 3.0 times (adjusted odds ratio = 3.0; 95% confidence interval, 1.1–7.9) more likely to be in hemorrhagic shock upon ED arrival than patients with ETCO2 ≥ 25 mm Hg.

What's the main takeaway?
Incorporating ETCO2 assessment into prehospital care for trauma patients could support decisions regarding prehospital blood transfusion, and triage to higher-level trauma centers, and trauma team activation.

What Causes Enuresis?
Doctors don't always know the exact cause of nocturnal enuresis. They do have some theories, though, on what may contribute to someone developing the condition:

Hormonal problems. A hormone called antidiuretic hormone, or ADH, causes the body to produce less urine at night. But some people's bodies don't make enough ADH, which means their bodies may produce too much urine while they're sleeping.
Bladder problems. In some people with enuresis, too many muscle spasms can prevent the bladder from holding a normal amount of urine. Some teens and adults also have relatively small bladders that can't hold a large volume of urine.
Genetics. Teens with enuresis often have a parent who had the same problem at about the same age. Scientists have identified specific genes that cause enuresis.
Sleep problems. Some teens may sleep so deeply that they don't wake up when they need to p*e.
Caffeine. Using caffeine causes a person to urinate (p*e) more.
Medical conditions. Medical conditions that can trigger secondary enuresis include diabetes, urinary tract abnormalities (problems with the structure of a person's urinary tract), constipation, and urinary tract infections (UTIs). Spinal cord trauma, such as severe stretching of the spinal cord resulting from a fall, sports injury, auto accident, or other event may also play a role in enuresis, although this is rare.
Psychological problems. Some experts believe that stress can be associated with enuresis. It's not uncommon to feel stressed out during the teenage years, and things such as divorce, the death of a friend or family member, a move to a new town and adapting to a new school and social environment, or family tension can feel overwhelming.
Doctors don't know exactly why, but more than twice as many guys as girls have enuresis. It is often seen in combination with ADHD.

Eat Healthy
Make sure you eats lots of fruit and vegtables each and every day. Even try adding a multivitamin to your daily routine. You'll soon see the difference.
Get active
One of the best ways to stay healthy is to exercise. It's recommended to do 30 minutes of cardio exercise 3 times a week. This even includes going for a walk with the dog, or even running around with the children.
Keep stress in check
Getting stressed out is bad for your health. If you are stressed in the work place you are more likely to cause an accident and injure yourself or even fall ill.
Positive outlook
It has been scientifically proven that a person with a positive outlook on life will be healthier than someone who doesn't.When you smile your brain releases small amounts of serotonin that give your body a good feeling. So next time your sad, smile.
Relax
Relaxation has actually become a medical treatment. It's very important to relax in the evenings after working. Try find somewhere that you feel comfortable, and just kick back. It can be anywhere from a chair, to a bubble bath.
Ways to increase your daily excercise
Walk to the shops rather than taking the car. Try taking the stairs whenever possible. If you get two or more buses, can you walk one of the journeys? Or even get off the bus or train a stop earlier.
