𝐂𝐚𝐮𝐬𝐞𝐬 𝐨𝐟 𝐍𝐨𝐧-𝐏𝐢𝐭𝐭𝐢𝐧𝐠 𝐏𝐞𝐝𝐚𝐥 𝐄𝐝𝐞𝐦𝐚

1. 𝐋𝐲𝐦𝐩𝐡𝐞𝐝𝐞𝐦𝐚
Obstruction or damage to lymphatic drainage.Edema is firm, non-pitting, asymmetric, often with skin thickening

Causes:
Primary (congenital, e.g., Milroy’s disease).
Secondary (infection like filariasis, surgery, radiation, tumor compression).

2. 𝐌𝐲𝐱𝐞𝐝𝐞𝐦𝐚 (𝐒𝐞𝐯𝐞𝐫𝐞 𝐇𝐲𝐩𝐨𝐭𝐡𝐲𝐫𝐨𝐢𝐝𝐢𝐬𝐦)

Deposition of mucopolysaccharides in interstitial tissues.Edema is doughy, firm, non-pitting.Often associated with other hypothyroid features (dry skin, hair loss, weight gain, cold intolerance).

3. 𝐂𝐡𝐫𝐨𝐧𝐢𝐜 𝐕𝐞𝐧𝐨𝐮𝐬 𝐈𝐧𝐬𝐮𝐟𝐟𝐢𝐜𝐢𝐞𝐧𝐜𝐲 (𝐥𝐚𝐭𝐞 𝐬𝐭𝐚𝐠𝐞)

Longstanding venous stasis can lead to fibrosis and hardening of tissues.Early: pitting edema.Late: becomes non-pitting due to tissue fibrosis.

4. 𝐋𝐢𝐩𝐞𝐝𝐞𝐦𝐚

Abnormal deposition of fat in lower limbs.Usually bilateral, tender, sparing the feet.Can appear like non-pitting edema.

5. 𝐈𝐧𝐟𝐞𝐜𝐭𝐢𝐨𝐧𝐬 & 𝐈𝐧𝐟𝐥𝐚𝐦𝐦𝐚𝐭𝐨𝐫𝐲 𝐂𝐚𝐮𝐬𝐞𝐬

Cellulitis, erysipelas → localized, warm, erythematous non-pitting swelling.Post-inflammatory fibrosis after chronic infection.

6. 𝐎𝐭𝐡𝐞𝐫𝐬 / 𝐑𝐚𝐫𝐞

Pretibial myxedema in hyperthyroidism (Graves’ disease).
Malignant infiltration or scarring causing lymphatic obstruction.
Drugs: e.g., calcium channel blockers sometimes cause firm edema, but usually pitting at first.

Infertility/Subfertility:
Implies inability or reduced chance of conception with regular sexual in*******se for a period exciding 6 months
Causes:
• Female factors: Ovulation problems, tubal issues/blockageas, endometriosis, uterine abnormalities, and advanced age.
• Male factors: Abnormal s***m production, function, or quality of produced semen.
• Combined factors: A combination of both male and female factors.

Diagnosis:
Female fertility
1. Ovulation Tests: track levels of luteinizing hormone (LH) in urine
2. Hormone Tests: Follicle-Stimulating Hormone (FSH), Anti-Müllerian Hormone (AMH), Estradiol, Prolactin etc
3. Transvaginal Ultrasound: use ultrasound to evaluate the ovaries, cysts, fibroids and monitor the development of follicles during the menstrual cycle.
4. Hysterosalpingography (HSG) check if fallopian tubes are open, blockages or abnormalities in the uterus.
5. Ovarian Reserve Tests: tests assess quantity and quality of a woman's eggs
Male fertility
• Semen Analysis
o S***m Count: The number of s***m in the sample.
o Motility: The percentage of s***m that are moving.
o Morphology: The shape and size of the s***m.
o Volume: The amount of semen produced.
o pH Level: The acidity or alkalinity of the semen.
o White Blood Cells: Elevated levels could indicate infection.
o Fructose Level: Low levels could indicate a problem with the seminal vesicles or prostate.
• Hormone Testing- Testosterone etc
• Genetic Testing- Identify genetic factors that may contribute to infertility.
• Ultrasound- check for structural issues in the reproductive organs i.e. testicles, epididymis
• Post-Ejaculation Urine Test
• S***m DNA Fragmentation Test
• Vasography- check for blockages in the vas deferens

Anomaly scan and gender reveal

The anomaly scan (anatomy scan) is conducted between 18 and 22 weeks' gestation more common at 19-20 weeks gestation to evaluate anatomic structures of the fetus, placenta, and maternal pelvic organs.
This scan is an important and common component of routine prenatal care.
The function of the ultrasound is to measure the fetus so that growth abnormalities can be recognized quickly, to assess for congenital malformations and multiple pregnancies progress. In addition to plan method of delivery.
During this scan, additional information like gender can also be assessed depending on the parents preference for gender reveal.
Two-dimensional (2D) is used to evaluate fetal structures, placenta, and amniotic fluid volume.
