Patellar tendon-bearing AFOs
1. A PTB AFO uses the patellar tendon and the tibial condyles to partially relief weight bearing stress on skeletal structures distally, with more weight bearing distributed along the medial tibial condyle
2. It is often prescribed for diabetic ulcerations of the foot, total fractures, relief of the weight bearing surface in painful heel conditions such as calcaneal fractures, postoperative ankle fusions and avascular necrosis of the foot or the ankle

Assisting a client to stand on a walking pylon for the first time after amputation. She appreciated our efforts since she had not stood up for the last 11 months.
This is the first step in prosthetic rehabilitation. We believe in a short while she'll walk on the pylon and eventually on a prosthesis

Heel spur
A heel spur (calcaneal spur) is a calcium deposit causing a bony protrusion on the underside of the heel bone (calcaneus). On an x-ray a heel spur is seen as an extended protrusion beneath the heel bone pointing forward about half an inch. Without visible X-ray evidence, the condition is known as "heel spur syndrome ". In itself the heel spur is not painful but may cause pain around the heel which is frequently associated with plantar fasciitis, a painful inflammation of the plantar fascia.

Causes of heel spur.
Heel spurs are often caused by strains on the foot muscles and ligaments or stretching of plantar fascia or repeated tearing of the membrane covering the heel bone (calcaneus)

Treatment.
Treatment of calcaneal spur includes but not limited to:
1. Exercises
2. Custom-made or over the counter prefabricated orthotics
3. Anti-inflammatory medication
4. Cortisone injections

If these conservative treatments fail, surgical intervention maybe indicated.
Neema orthopaedic has these services.

Prosthetic Rehabilitation
Amputation is not the end of life but rather the beginning of another life. Before amputation, one walks with two legs. After amputation, one is bed ridden for the first two or so days after which ambulation starts.
One has to start by learning to stand and support oneself with one leg. To make it easy and imprint in one's mind to stand in two legs, it's common practice to fit the person with a temporary pylon or a temporary prosthesis (artificial leg) the earliest possible. It's good if possible to let the amputee interact with a prosthetist/orthopaedic technologist at earliest time possible. He/she will create an image of using a prosthesis/artificial leg the earliest possible and advise on the appropriate time to start prosthetic Rehabilitation. If a clear image of walking on a prosthesis is drawn in the mind of the amputee, prosthetic Rehabilitation process sails through smoothly for the benefit of the amputee.
