Patellar tendon-bearing AFOs
1. A PTB AFO uses the patellar tendon and the tibial condyles to partially relief weight bearing stress on skeletal structures distally, with more weight bearing distributed along the medial tibial condyle
2. It is often prescribed for diabetic ulcerations of the foot, total fractures, relief of the weight bearing surface in painful heel conditions such as calcaneal fractures, postoperative ankle fusions and avascular necrosis of the foot or the ankle

Casting in preparation of making a temporary prosthesis. Initial stage of casting is identifying prominent Bonny areas and build up for relief. After which a wrap cast is done, marking the weight bearing areas.

Assisting a client to stand on a walking pylon for the first time after amputation. She appreciated our efforts since she had not stood up for the last 11 months.
This is the first step in prosthetic rehabilitation. We believe in a short while she'll walk on the pylon and eventually on a prosthesis

Heel spur
A heel spur (calcaneal spur) is a calcium deposit causing a bony protrusion on the underside of the heel bone (calcaneus). On an x-ray a heel spur is seen as an extended protrusion beneath the heel bone pointing forward about half an inch. Without visible X-ray evidence, the condition is known as "heel spur syndrome ". In itself the heel spur is not painful but may cause pain around the heel which is frequently associated with plantar fasciitis, a painful inflammation of the plantar fascia.

Causes of heel spur.
Heel spurs are often caused by strains on the foot muscles and ligaments or stretching of plantar fascia or repeated tearing of the membrane covering the heel bone (calcaneus)

Treatment.
Treatment of calcaneal spur includes but not limited to:
1. Exercises
2. Custom-made or over the counter prefabricated orthotics
3. Anti-inflammatory medication
4. Cortisone injections

If these conservative treatments fail, surgical intervention maybe indicated.
Neema orthopaedic has these services.

Prosthetic Rehabilitation
Amputation is not the end of life but rather the beginning of another life. Before amputation, one walks with two legs. After amputation, one is bed ridden for the first two or so days after which ambulation starts.
One has to start by learning to stand and support oneself with one leg. To make it easy and imprint in one's mind to stand in two legs, it's common practice to fit the person with a temporary pylon or a temporary prosthesis (artificial leg) the earliest possible. It's good if possible to let the amputee interact with a prosthetist/orthopaedic technologist at earliest time possible. He/she will create an image of using a prosthesis/artificial leg the earliest possible and advise on the appropriate time to start prosthetic Rehabilitation. If a clear image of walking on a prosthesis is drawn in the mind of the amputee, prosthetic Rehabilitation process sails through smoothly for the benefit of the amputee. What is Knee Replacement Surgery?
The knee joint is one of the most crucial joints which helps in performing our daily activities such as walking, running, sitting and exercising. A knee joint bears the entire weight of the body. Infection, injuries, trauma, age, genetics or arthritis are some of the conditions which can damage the knee joints. Initially, the patients are recommended with conservative treatments in case if these treatments fail to provide relief to the patient knee replacement surgery is recommended.

Knee replacement surgery is a complex procedure which makes it important to undergo this surgery from a skilled and experienced surgeon. There are different types of knee replacement surgery - the type of approach used vary from patient to patient. It may take around months or year for the patient to recover completely.

Conditions that Damage Knee Joints
Following are some of the conditions that can lead to damaging of knees. Initially, the surgeon prescribes patients with anti-inflammatory medicines, pain killers, exercises and physiotherapy. In case if all these treatments fail to provide the patient with relief from the severe pain knee replacement surgery is recommended:

Obesity
Wear and tear of joints
Certain medications
Trauma
Hormonal changes
Genetics
Age factors
Lack of nutrition
Instability in joints
Rigorous sports activity
Injury while hitting some object
Osteoarthritis
Osgood-Schlatter Disease
Patellar tendonitis
Bursitis
Patellofemoral pain syndrome
Dislocated kneecap
Psoriatic arthritis
Rheumatoid arthritis
Iliotibial band syndrome
What is an Artificial Joint?
The artificial joint is popularly known as a prosthesis. This artificial joint is made of metal and hard plastic material. The joint is designed in such a way that it replicates the function of the natural joint. The shin bone portion is replaced with an artificial part which is made from plastic or metal material whereas strong plastic model (patella) is used to replace the kneecap. The artificial knee joint can last for many years but can wear out or loosen with time for which revision surgery is required.

Do Many People Undergo Joints Replacement Surgery?
Joints Replacement is one of the most common orthopaedic surgery. Many people undergo joint replacement surgery due to age factor, severe injury or genetic factor. Knee replacement surgery is performed on a patient who are over 50 years of age or has a severely damaged knee joint. Post-surgery the patients are able to move or walk around more freely.

Do I Need to Undergo Knee Replacement Surgery?
Prior to recommending the surgery, the surgeons perform a series of tests such as X-ray and arthroscopy to know the exact condition of your joints. The surgeons may initially prescribe you with medicines and exercises to manage your pain and mobility. In case if you still have a stiff and painful knee while performing simple activities then knee replacement surgery is recommended.

Types of Knee Replacement Surgery
There are 4 Major types of Knee Replacement Surgery. The type of knee replacement surgery recommended varies from patient to patient:-

Total Knee Replacement Surgery: - This surgery is performed by replacing the joint surface of the shin bone and of thigh bone end part.
Unicompartmental (Partial) Knee Replacement: - This surgery is recommended in case if only one side of the knee is affected with arthritis.
Patellofemoral Arthroplasty (Kneecap Replacement): - This surgery involves replacing of kneecaps underneath or below the trochlea.
Complex or Revision Knee Replacement: - In this surgery, the longer stem is used to support and stabilize the component to fix the bone cavity.
What Happens During Knee Replacement Surgery?
This surgery is performed under general anaesthesia. The surgeon makes an 8 to 12-inch long incision in the front portion of the knee. With the help of surgical tools, the surgeon removes the damaged portion of the knee joint. Knee bone, kneecap, shin and cap are resurfaced and fixed with cement to hold the metal or plastic artificial joint. The new joint relies on the muscles and ligaments for essential support and normal function. With minimally invasive procedures, the surgeon can perform this surgery successfully without making any large incisions.

What Happens After Surgery?
Post-surgery your stay in the hospital is for around 4 to 5 days for initial supervision. The day after the surgery you may be told to stand and move with the help of parallel bars, this is done to adjust your body with the newly fitted joint. With physiotherapy and proper medications you can experience a dramatic improvement in the overall movement of your joint. After a month or two, you can walk comfortably without any assistance and can enjoy most of the activities except rigorous jumping and running.

What is the Success Rate of Knee Replacement Surgery?
The knee replacement surgery success rate can expand up to 92% to 97%. The following points might help you expand the success rate of the surgery:-

Taking prescribed medications properly.
Following regular physiotherapy.
Be careful that you do not twist or turn the joint for at least 6-8 weeks.
Avoided doing and physical activity which can put pressure on your knees.
Keep away from extreme activities.
