⚠️ Anaphylactic Shock – Emergency Overview

1️⃣ Definition

A life-threatening systemic hypersensitivity reaction causing airway, breathing, and circulatory compromise.

βΈ»

2️⃣ Causes

➊ Drugs – antibiotics (penicillin), NSAIDs, anaesthetic agents
βž‹ Foods – nuts, shellfish, eggs
➌ Insect stings
➍ Blood transfusion reactions – severe acute hemolytic transfusion reactions (AHTR) can mimic anaphylaxis:
β€’ Occur rapidly during or after transfusion
β€’ Symptoms: Fever, hypotension, dyspnea, back/chest pain
β€’ Always consider in post-transfusion collapse

βΈ»

3️⃣ Management (ALS Adult Protocol)

➊ Remove trigger if known (e.g. stop transfusion or drug)

βž‹ Call for help / 2222 / resuscitation team

➌ IM Adrenaline (Epinephrine)
➀ 0.5 mg IM (1:1000) in mid-anterolateral thigh
➀ Repeat every 5 minutes if needed

➍ ABC approach:
1. Airway – High-flow Oβ‚‚, consider airway adjuncts if compromised
2. Breathing – Monitor SpOβ‚‚, give oxygen, support ventilation if needed
3. Circulation – Lie patient flat, elevate legs, give IV fluids rapidly (e.g. 500–1000 mL crystalloid bolus)

➎ Additional Medications:
➀ Chlorphenamine 10 mg IV
➀ Hydrocortisone 200 mg IV
➀ Salbutamol neb for bronchospasm

➏ Monitor continuously:
➀ ECG, BP, pulse oximetry, urine output

➐ Post-crisis care:
➀ Observe for biphasic reaction (4–6 hours or longer)
➀ Refer to allergy/immunology
➀ Educate on adrenaline autoinjector use (e.g. EpiPen)
