Neurodynamic Mobilisation: A Case Report Sept '17

CONCLUSION: 'In a female collegiate athlete with persistent neuropathic pain, initial improvements were achieved with traditional rehabilitation, but her pain continued. Considerable additional improvements were achieved following the addition of self-administered, supervised nerve mobilization. Neurodynamic testing should be performed on patients with possible peripheral nerve involvement and treatment commenced if positive.'(1)

Questions: Do you use nerve tension/sliders to treat? Which do you choose and why? Are you trying to de-sensitise or improve its (suggested) mobility? How would you clinically reason its use?

This systematic review suggests benefits are not clear for conditions other than in the back and neck (2).

"The purpose of the current paper is to present 2 cases of saphenous nerve entrapment at the adductor canal and to highlight a nerve tension technique to aid in the clinical diagnosis of this condition."

HIGHLIGHTS:

- Saphenous nerve entrapments are a very rare condition within today’s body of literature, and the diagnosis remains controversial.

- The saphenous nerve is a purely sensory nerve, and is the longest terminal branch of the posterior division of the femoral nerve, arising from the L3 and L4 nerve roots.

- The distribution of pain in saphenous nerve entrapment patients has been reported as at the knee (90%), thigh (7%) and calf (3%)

- Those undergoing ACL repairs are at risk of damage to the saphenous nerve at a rate of 30% – 59% depending on the technique utilized...

- Currently the main treatment options include surgical debridement and corticosteroid injections, however there is a body of evidence supporting the use of many manual therapy techniques for the treatment of various nerve entrapments throughout the body.

- Nerve tension testing may prove to be an aid in the diagnosis of saphenous nerve entrapments within a clinical setting in order to decrease time to diagnosis and proper treatment.
