Transforaminal & Transpedicular approach Dr. Kim JW
A gentleman in his 50s' with back / left leg pain and weakness. Initially routine transforaminal appraoch & decompression was done. Then mid-pedicle was drilled and cannula was inserted to expose epidural space.

Big facet, big scope(10mmHview) Dr. Kim JW
A gentleman in his 50s' with left buttock and leg pain despite of injections and epidural neurolysis. After epidural neurolysis, walking time had become longer to 5 minutes. Eighteen G needle was inserted epidural space for local anesthesia and guid wire through isthmus route. Then mid-portion of S1 SAP was drilled and foraminal annulus lateral portion was exposed. Using annular cutter, medial bone of S1 SAP was removed and cannula was inserted into foramen without touching DRG. Then remained bone. facet capsule were trimmed debrided...

Cord draping(T-L fx)-PVP, thecal sac trapping(L45 listhesis)-direct decompression 30M F/U
A lady in her late 70s' with back & both buttock pain. Both big toe dorsiflex Gr IV- on sitting. Initially T12,L1 PVP was done. But right side dominant buttock pain was remained. Using 10mmHview, right foraminal decompression(L45), and central decompression was done under local anesthesia. At 30M F/U, L45 listhesis was not progressed and her T-L juction kyphosis was improved.

Transcoporeal foraminotomy without fusion for C56 right 8 Y F/U
A gentleman in his 50s' with right leg pain for several weeks. He had undergone transcoporeal foraminotomy without fusion about 8 YA. U/E motor was symmetrical in both side. At 8 Y F/U, C56 disc space was not collapsed and F/E was maintained.

Huge recurrent HLD with stable fracture after ground fall 10mmHview Dr Kim JW
A lady in her 90s' visited again due to severe pain and both leg weakness. She had undergone endoscopic decompression about a month ago. 2-3 weeks ago, she had experienced ground fall. Using 10mmHview, SAP L5 waas removed and epidural space was exposed. Cannula was not entered disc space and extruded portion was removed.

No back pain during lying down at 3 YF/U after two level decomp. just cook & eat alone...
A lady in her 80s' with back pain during sitting, standing and cooking / eating. She had undergone two level decompresssion under local anesthesia. She cooks as old fashion(preparing all the vegetables by herself...) and eats alone... Anyway, no progression of listhesis was seen.

Less than 180 degrees to full shoulder abduction, 3M after C5 decompression
A gentleman in his 60s' with right shoulder abduction weakness. C45, 56 anterior decompression without fusion was done. At 3MF/U, shoulder abduction to ear and Gr IV+ . Cervical lordosis was improved.

Post. foraminotomy for C7T1 Rt foraminal lesion 7Y after C67 ACDF(from left)
A gentleman in his 60s' with right arm, hand and shoulder pain & weakness. he had undergone ACDFC67(from left) 7YA. Right C7T1 foraminal lesion was problem. Posterior foraminotomy was done and foraminal HCD was removed also. At 1 M F/U, motor power was restored.

Old films,easy way to solve puzzle-severe back pain after PVP(6M) selctive decompression Dr. Choi WC
A lady in her 80s' with severe back pain after PVP 6MA. She had undergone fusion L4 to S1 about 2 YA. L23 level compression was increased, so selective decompression L23 from left was done. Nowadays, expected age of patient with ADLs were... And old films were available frequently...
