Isolated abnormal laboratory tests necessitate obtaining abdominal CT for pediatric patients with BAT. Our findings highlight significance of clinical findings in the trauma bay, regardless of lab findings, when deciding to order abdominal CT imaging for pediatric BAT patients.

이번주 CHEST Journal에 흥미로운 외상케이스가 실려서 공유합니다.

" 때로는 교과서를 벗어난 접근이 생명을 구합니다."

18세 남성, 40피트(12m) 추락, 광범위한 출혈성 폐손상으로 ECMO를 걸었지만, 단일 회로 최대 flow(6L/min)로도 산소화 실패하였으나 과감하게 두 번째 ECMO 회로를 추가하여 parallel configuration(총 12L/min)으로 전환 → 즉각적인 가스교환 개선.
더 놀라운 건, 심한 기침으로 폐출혈이 악화되자 모든 정맥 진정제가 실패한 상황에서 ECMO 회로에 isoflurane vaporizer를 직접 연결하여 흡입마취제로 진정을 성공시킨 것.
심지어 ECMO 가동 중에 골반과 대퇴골 수술까지 완료.

Take-home messages:
• 젊은 외상 환자 + 광범위 폐손상 = 단일 ECMO 회로로는 부족할 수 있다
• Conventional approach 실패 시 창의적 해결책을 주저하지 말 것
• 불가능해 보이는 것도 다학제 협진으로 가능하다

6개월 후 이 환자는 고등학교를 졸업하고 직장에 복귀했습니다.

Neck의 Zone I 부위 손상은 수술적 접근이 쉽지 않습니다. 본 논문에서는 transmanubrial approach라는 방법을 통해 median sternotomy, thoracotomy 혹은 clavicle resection 보다 덜 침습적인 방법으로 해당 부위의 수술 시야를 얻을 수 있는 방법을 소개하고 있습니다.

The transmanubrial approach mini-sternotomy provides access to the thoracic outlet. It is a less invasive alternative to median sternotomy, thoracotomy, or clavicle resection and can be safely performed in hemodynamically stable patients.

현재 문헌에 따르면, 병원 전 전혈의 사용은 현장에서 병원 도착 시 활력 징후의 빠른 개선, 혈액제제 요구량 감소, 입원 시 혈액 응고 프로파일 개선 등 임상 결과 개선과 연관될 수 있습니다. 외상 환자의 예후에 전혈의 적시 투여가 중요한 것으로 밝혀진 점을 고려할 때, 병원 전 전혈의 사용은 조기 및 후기 사망률 개선과 연관될 가능성이 높습니다. 군인 및 민간인 외상 환자의 병원 전 단계에서 전혈 사용에 대한 문헌이 증가하고 있음에도 불구하고, 조기에 적극적인 소생술이 필요한 출혈성 외상 환자의 사망률에 대한 전혈의 효능을 확인하기 위한 향후 임상 시험이 필요합니다.

65세 이상의 다발성 늑골 골절 환자의 예후 분석을 위해 Jeff Choi를 비롯한 스탠포드 연구진은 13개 기저 상태를 이용한 머신러닝(릿지회귀 분석)을 통해 RFF index 및 이를 계산하는 모바일 앱을 개발해 배포했습니다.

이 논문은 RFF index의 external validation에 대한 것으로 JTACS에 관련 내용으로 벌써 두 번째 실린 논문이네요.

The Rib Fracture Frailty (RFF) Index, was evaluated in a small-scale feasibility study. It may outperform age in predicting outcomes for rib fractures, especially in lower injury severity, offering a simple approach to clinical risk prediction with minimal provider entry.
