👩‍⚕️ 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗣𝗿𝗼𝗳𝗶𝗹𝗲:
72-year-old female
Severe right knee pain, particularly in the medial joint line
Severe varus deformity

🩺 𝗠𝗲𝗱𝗶𝗰𝗮𝗹 𝗛𝗶𝘀𝘁𝗼𝗿𝘆:
Previous distal femoral osteotomy performed over 40 years ago

📸 𝗫-𝗿𝗮𝘆 𝗙𝗶𝗻𝗱𝗶𝗻𝗴𝘀:
Terminal right knee tri-compartmental osteoarthritis
Severe varus deformity of the distal femur
Retained hardware from the previous surgery

🔬 𝗠𝗼𝗱𝗲𝗿𝗻 𝗔𝗽𝗽𝗿𝗼𝗮𝗰𝗵:
The goal was to perform a total knee replacement that restores the anatomy and the axis of the right lower limb in a single stage. The challenges included:

𝗛𝗮𝗿𝗱𝘄𝗮𝗿𝗲 𝗥𝗲𝗺𝗼𝘃𝗮𝗹:
Difficulty due to possible metal cold welding and bone apposition
Finding the correct screwdriver not commonly used after 40 years

𝗥𝗲𝗮𝗹𝗶𝗴𝗻𝗺𝗲𝗻𝘁 𝗢𝘀𝘁𝗲𝗼𝘁𝗼𝗺𝘆:
Concomitant with the use of a stemmed implant to bypass the osteotomy site
Utilizing a modern plating system to support the fragments and promote healing

🏥𝗣𝗼𝘀𝘁-𝗢𝗽 𝗦𝘂𝗰𝗰𝗲𝘀𝘀:
The patient recovered exceptionally well, achieving excellent pain-free range of motion and full weight-bearing at 1 month post-op.

📢 فهم Periacetabular Osteotomy (PAO) 🦴
🔹 ما هو PAO؟
PAO هو إجراء جراحي لتصحيح خلل التنسج الوركي حيث يكون تجويف الورك غير مشكل بشكل صحيح، مما يسبب ألمًا وعدم استقرار.

🌟 𝗥𝗲𝗺𝗮𝗿𝗸𝗮𝗯𝗹𝗲 𝗥𝗲𝗰𝗼𝘃𝗲𝗿𝘆 𝗮𝘁 𝗔𝗴𝗲 𝟵𝟬 🌟

In January 2024, our 90-year-old patient, with no significant co-morbidities, suffered a severe left hip fracture. Initially treated with a proximal femoral nail at another hospital, she faced complications due to full weight bearing, leading to nail protrusion into the acetabulum and significant deformity. She was bedridden.

🔧 𝗦𝘂𝗿𝗴𝗶𝗰𝗮𝗹 𝗜𝗻𝘁𝗲𝗿𝘃𝗲𝗻𝘁𝗶𝗼𝗻 🔧
I removed the nail, grafted the acetabulum, and reconstructed the hip using a primary uncemented cup and a modular femoral stem, secured with a trochanteric hook plate and cerclage wires.

🚶 𝗜𝗻𝗰𝗿𝗲𝗱𝗶𝗯𝗹𝗲 𝗢𝘂𝘁𝗰𝗼𝗺𝗲 🚶
She was able to bear full weight the next day and was discharged on day five. Now, she's recovering well in a rehab center.

🌟 تعافي مذهل في سن التسعين 🌟

في يناير 2024، تعرضت مريضة تبلغ من العمر 90 عامًا، دون أي أمراض مصاحبة، لكسر شديد في الورك الأيسر. تم علاجها في البداية باستخدام مسمار فخذي قريب في مستشفى آخر، لكنها واجهت مضاعفات بسبب السماح لها بتحمل الوزن الكامل، مما أدى إلى بروز المسمار في الحُق وتشوُّه كبير. كانت طريحة الفراش.

قمت بإزالة المسمار، وترقيع الحُق، وإعادة بناء الورك باستخدام كأس غير مثبّت أساسي وساق فخذية وحدوية، مع تثبيت بواسطة لوحة وخطافات وحلقات سلكية.

🚶 نتائج مذهلة 🚶
تمكنت من تحمل الوزن الكامل في اليوم التالي وتم خروجها في اليوم الخامس. الآن، تتعافى بشكل جيد في مركز إعادة التأهيل.

🚨 𝗖𝗮𝘀𝗲 𝗛𝗶𝗴𝗵𝗹𝗶𝗴𝗵𝘁: 𝗔𝗿𝘁𝗵𝗿𝗼𝘀𝗰𝗼𝗽𝗶𝗰 𝗠𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁 𝗼𝗳 𝗦𝘆𝗻𝗼𝘃𝗶𝗮𝗹 𝗖𝗵𝗼𝗻𝗱𝗿𝗼𝗺𝗮𝘁𝗼𝘀𝗶𝘀 🚨
Today, I had the privilege of treating a 47-year-old male patient suffering from pain, effusion, and a limp in his right knee. Imaging studies, including X-rays and MRI, revealed synovial chondromatosis involving both the anterior and posterior compartments of the knee.

💡 𝗢𝘂𝗿 𝗔𝗽𝗽𝗿𝗼𝗮𝗰𝗵: We opted for an arthroscopic management technique to remove all osteochondral fragments. This minimally invasive surgery allowed us to address both the anterior and posterior compartments effectively.
