Dr. Nabil Fakih is recognized as an international expert in vascular occlusion complication after filler in acute phase. Dr. Nabil Fakih’s research and leadership of a team of colleagues in developing the 2025 guideline resulted in the following key findings. I recommend reading the full article, as it is a master research. Additionally including the FOEM2 new grading.

Prompt and effective management of vascular occlusion is critical to prevent irreversible tissue damage. A systematic review of 96 PubMed articles was conducted to create a modern action guide, excluding ocular cases, and focusing on current angiosome-based concepts and advanced interventional radiology strategies.

🔹 Early recognition is essential – Pain and skin color changes remain the most prevalent early warning signs.
🔹 Time-dependent outcome – Intervention within 4–6 hours can reverse ischemia in up to 80% of cases.
🔹 High-dose Hyaluronidase (1500 IU every 15–20 min) remains the gold standard; ultrasound guidance reduces the dose required up to tenfold.
🔹 Adjunct therapies – Antiplatelets reduce platelet aggregation, corticosteroids mitigate inflammation, vasodilators aid perfusion, and hyperbaric oxygen therapy enhances tissue oxygenation.
🔹 Structured protocols – The FOEM2 score and ischemic staging provide a framework for precise, evidence-based treatment decisions.

✅ Conclusion: Timely diagnosis, rapid intervention, and adherence to structured, evidence-based protocols are vital to improve patient outcomes. Establishing standardized guidelines will further elevate patient safety and treatment efficacy in aesthetic medicine.

Hyaluronic acid fillers are widely used in aesthetic medicine, yet vascular occlusion remains the most severe complication that can happen. In this case series, we present the novel “THIS and FAT” protocol integrating botulinum toxin type A, high-dose hyaluronidase, injectable and serum PRF, aspirin and antibiotics, nanofat, debridement/dermabrasion, and fat membrane application. Among 25 treated patients, 92% achieved complete healing without scarring, demonstrating highly promising outcomes for ischemia management. Thanks for all authors for their great contributions to this work. Behind every investigation there is a lot of all types of effort.

While sharing knowledge reflects generosity, investigation is the true engine of progress. Research is not only about caring, it is about questioning, discovering, and transforming. Every study expands the boundaries of what we know and opens doors to what we can achieve. Science is not just sharing, it is beyond.

👄 DIRECT LIP LIFT + SILICONE REDUCTION + LIP REDUCTION AFTER DIRECT LIP LIFT ➡️ I’d like to share a strategic case I managed. The patient presented with small lips containing silicone and a long upper lip. The main challenge in planning was determining the best steps to achieve a natural and aesthetically pleasing result. If I had simply resected the silicone, she would have been left with almost no lip volume. So, I offered her a staged approach: first, I performed a direct lip lift using the sashimi technique, which allowed me to reduce the length of the upper lip while creating a fuller, bigger & more lifted red lip. In the second stage, I removed the silicone and refine the lip shape to better match the lower lip. I’m very happy to share her results and truly grateful that she allowed me to present her case. I’m also thankful to her husband, a surgeon himself, who trusted me to provide his wife with a natural and beautiful outcome. I’m proud of the result but even more fulfilled by the fact that I helped my patient achieve the look she dreamed of. Thank you to everyone who puts their trust in me, and especially to those who are patient during the healing process. Achieving excellent results often requires time, care, and a step-by-step approach. Good surgery starts with good planning. When you add precision in suturing and attention to detail, the outcome goes beyond expectations. As I told my patient: go fly like a bird and enjoy what you’ve dreamed of. 👁️ Lower eyelid surgery is a highly delicate and technically demanding procedure. While some cases may be straightforward, others require a more comprehensive approach involving volume restoration, structural support, and adjunctive therapies. The surgical plan must be tailored to the anatomical characteristics, aging patterns, and individual aesthetic needs of each patient, particularly in complex or revision cases, or in the presence of festoons and midface ptosis.

In this case, we performed a lower blepharoplasty using a 3-vector suspension technique, incorporating canthopexy, fat transposition, and midcheek support to restore structural integrity and aesthetic contour. Additionally, we performed a targeted suspension of the festoon region to reduce skin redundancy and elevate the sagging tissue. Festoons are often challenging to fully correct, especially when they present with significant bulging, laxity or water retention, and typically require a multimodal approach to achieve meaningful improvement.

In this patient, additional volume correction was required postoperatively to harmonize the transitions between lifted and non-lifted areas, particularly where anatomical bone projection was insufficient. Autologous fat grafting or dermal fillers could be used judiciously to achieve a seamless integration of facial planes.

Some cases with asymmetric or big festoons, a laser-assisted skin/subcutaneous contraction techniques could be used to into the postoperative plan to enhance tightening and optimize results.

The key to success in advanced lower blepharoplasty lies in experience-driven judgment: understanding the limitations of surgery, anticipating areas where complementary treatments may be required, and building a customized plan that prioritizes both structure and contour.

👄 Direct Lip Lift (Sashimi Technique)

I’m glad to share a new and interesting case of Direct Lip Lift using the Sashimi Technique. In this patient, we achieved a more refined and natural version of her lips, maintaining facial harmony while introducing a new shape with an elegant touch.

This technique allows for personalized design, whether creating a more defined central “V” or less and a subtly rotated lateral lift or less—based on each patient’s facial anatomy & aesthetic goals. During the consultation, we work closely with patients to reach a shared vision, balancing their desires with professional guidance.

In the end, the artistry lies not only in the surgery but also in the planning process, a dialogue where science, aesthetics, and patient identity come together.

Scientific debate is essential for the advancement of medical knowledge, particularly when diverse perspectives are addressed with evidence and respect. Constructive critique, especially within peer-reviewed forums, fosters collective progress and should never be perceived as personal. Science is a space for inquiry, growth, and evolution—not for confrontation.

In this spirit, a commentary was recently published in response to a previously published article by a friend comparing Harmonyca, Radiesse, and Hydroxfill. The commentary was based on scientific evidence and aimed to promote a transparent and constructive dialogue within the aesthetic medicine community. The goal was to contribute to a debate grounded in data, not in commercial interests.

Additionally, after reading the article, I messaged him, and he was fully aligned with engaging in a scientific debate, as he consistently upholds scientific values.

While marketing strategies may influence some narratives, scientific conclusions must remain evidence-based. Scientific truth has no owner, and ethical discourse is defined not only by what is said, but by how it is expressed. When commercial rivalries overshadow objectivity, the essence of scientific progress is compromised. The focus must remain on rigorous analysis, respectful exchange, and the shared mission of improving clinical outcomes.

This is science—grounded in evidence, guided by data. In the end, time validates the truth, since science is in constant discoveries and changes.

The Lebanon chapter comes to an end with my 141st and last fellow, Dr. Blanca , an otolaryngologist from Spain. This journey has been a testament to global learning, medical fellowship, and the meaningful exchange of knowledge across borders.
