♦️Physiotherapy Management in Chikungunya

Chikungunya is a viral infection transmitted by mosquitoes, often leading to polyarthritis and musculoskeletal pain especially in the small joints. Although acute symptoms subside within days, many patients develop chronic joint stiffness and pain lasting for weeks or months, similar to rheumatoid arthritis.

Common Symptoms

•Persistent joint pain and stiffness (especially wrists, ankles, knees)
• Myalgia and fatigue
• Reduced range of motion (ROM)
• Difficulty with functional mobility
• Post-viral fatigue Syndrome

🟥 Acute Phase physio management(0–2 weeks):

• Rest and joint protection
• Cold therapy (cryotherapy) for inflamed joints
• Gentle pain-free ROM exercises
• Isometric strengthening (e.g., quadriceps, gluteal sets)

🟨 Subacute Phase physio management(2–6 weeks)

• Thermotherapy (hot packs) to relieve stiffness
• Progressive active ROM exercises
• Stretching of tight muscles
• Strength training with resistance bands
• Postural correction and body mechanics education

🟩 Chronic Phase physio management(>6 weeks)

• Aerobic conditioning (e.g., walking, cycling, water therapy)
• Functional training (ADL practice, balance work) • Proprioceptive exercises
•Home exercise programs
• Hydrotherapy for joint unloading (if available)

♦️What is Sciatica?

Sciatica refers to pain that radiates along the path of the sciatic nerve, which branches from the lower back through the hips and buttocks and down each leg. It is usually caused by compression or irritation of the nerve, often due to a herniated disc, spinal stenosis, or piriformis syndrome.

♦️ Signs and Symptoms

• Radiating pain: Sharp, burning, or shooting pain down the leg, usually one-sided.

• Low back pain: May or may not accompany leg pain.

• Numbness or tingling: Especially in the back of the thigh, calf, or foot.

• Muscle weakness: In the affected leg or foot.

• Pain worsens: With prolonged sitting, standing, coughing, or sneezing.

As a physiotherapist offen get to see patients who gets upper back and neck pain , sometimes caused by wearing wronged size bra. Here I shared a simple method you can do it home to find your actual bra size !

How to Measure Your Bra Size at Home

Step 1: Measure Your Underbust (Band Size)

➡️ Wrap a measuring tape snugly around your ribcage, just under your bust.

➡️ Round this number to the nearest whole number.

• If the number is even, add 4
• If the number is odd, add 5

✅ This total is your Band Size

Example:
Underbust = 31” → 31 + 5 = 36 (Band size)
Underbust = 32” → 32 + 4 = 36 (Band size)

Step 2: Measure Your Bust

➡️ Measure around the fullest part of your bust (not too tight).

Step 3: Find Your Cup Size

Subtract your Band Size from your Bust Measurement
The difference determines your Cup Size
If the differences one , cup A , if two Cup B, if three cup C …

Example:
Bust = 38”
Band Size = 36”
Difference = 2” → B Cup
Your bra size = 36B

♦️ Common Causes:

1. Carpal Tunnel Syndrome (CTS) – Most frequent cause; compression of the median nerve at the wrist.

2. Pronator Teres Syndrome – Compression at the forearm near the elbow.

3. Cervical Radiculopathy (C6-C7) – Compression at the cervical spine, especially C6.

Why MRI Isn’t Always the Full Picture
While an MRI spine shows detailed structural images, it doesn’t reveal how your body actually moves or functions.

Here’s why movement analysis can be more accurate for certain cases:

♦️ Pain doesn’t always match the scan , Many people have disc bulges or degeneration with no symptoms at all!

♦️ MRI is static , It captures you lying still. But your pain often shows up only during movement.

♦️ It misses muscle imbalances, poor posture, or coordination issues , all of which are key to treatment.

Movement tells the real story.
That’s why physios combine imaging with functional assessments to treat the cause, not just the image!

Do You Really Need an MRI? 🤨

Not always. Physiotherapists often recommend an MRI only if you show red flag symptoms & signs that may point to a more serious condition.

Red flag symptoms include

• Severe or unrelenting night pain
• Unexplained weight loss
• History of cancer
• Numbness or tingling in the groin area (saddle anesthesia)
• Loss of bladder or bowel control
• Sudden weakness in arms or legs
• Fever with back pain

These may indicate serious issues like a tumor, infection, or nerve compression that need urgent care.

What is Osteoarthritis?

♦️Osteoarthritis is a degenerative joint disease where the cartilage (the smooth tissue that cushions your bones) gradually wears away, leading to pain, stiffness, swelling, and reduced movement.

♦️Common Causes

•Age-related cartilage degeneration

• Previous knee injuries

• Overweight or obesity

• Muscle weakness around the knee

• Poor joint alignment (e.g., bow legs or knock knees)

♦️Physiotherapy Management

• Strengthening exercises

• Stretching and range of motion exercises

• Weight management advice if necessary

• Supportive devices (like knee braces)

A simple meme posted a few days ago seemed to have disturbed some Medical Professionals .

And while discussing, it alarmed me how little *The Physiotherapists* are known for their assessment, decision-making, and clinical reasoning skills.

We complete a 4-year medical-based degree, enter with a high Z-score , train in hospitals and are SLMC registered.

We assess thoroughly, identify red flags 🚩, and refer to concerned specialists when needed.

Every profession is valued, and we respect the hierarchy.
But let’s be clear , we don’t just “follow instructions.”

Mechanical Back Pain isn’t a medical emergency.

The meme simply explains that over 90% of back pain is mechanical and medications or surgery aren’t the first line of treatment. These cases respond better to movement, education, and exercise-based rehab areas physiotherapists specialize.

We also emphasize the importance of lifestyle changes, ergonomic advice, posture correction, and active recovery things medication alone can’t fix.

What is Sciatica?

Sciatica is a type of nerve pain that radiates along the path of the sciatic nerve the largest nerve in the body which runs from the lower back through the hips, buttocks, and down each leg.

Signs & Symptoms

🔸 Sharp, burning, or shooting pain.

🔸 Pain radiating to the leg

🔸 Tingling, numbness, or weakness in the affected leg

🔸 Pain worsens with sitting, bending, or coughing

Common Causes
