How important is BMI?

Body Mass Index (BMI) does play a role in plastic surgery, especially when it comes to liposuction or a tummy tuck, for example. Simply put, the extra weight might render the improvements made by a body contouring procedure much less visible, if at all. If the probability of that is high, the surgeon will advise against the surgery. Thus, the unsurprising rule of thumb is - the closer you are to your ideal weight, the better.

With that said, it's not worth obsessing about BMI as there is no absolute BMI value or range that you need to be for the surgery. BMI remains a blunt tool when evaluating a patient's eligibility. In fact, when discussing weight in the context of plastic surgery, another crucial point is often left unmentioned - weight stability.

The "ideal" weight for a patient in this context is not a BMI value, but a weight that you want and are able to maintain with as little fluctuation as possible, whether upward or downward.

If you receive a liposuction and subsequently gain weight, you will struggle to see the results of the surgery.
If you receive a tummy tuck and subsequently lose weight, repeat surgery may be necessary to address the newly appeared extra skin on the abdomen.

These are just a couple of examples, but the principle applies to most body contouring and augmentation procedures - significant upward and downward fluctuations in weight after plastic surgery are likely to negatively affect the aesthetic results.

So, before you consider plastic surgery, and body contouring especially, make sure you are 1️) happy with your current weight and do not wish to change it, and 2️) are able to keep your current weight stable.

Then you will be happy with the results of your surgery, regardless of the BMI value.

The patient’s main complaint was the width of her nose tip. “Can I do all of this in one surgery?” is a frequent question from patients who seek to have multiple procedures done in a single trip. The short, unsatisfying answer is “that depends”. On the one hand, having multiple procedures done in a single surgery is favorable both for the patient’s time and wallet, but also their aesthetic goals. If the procedures are aimed at transforming the same general area of the body or face, then addressing all the issues at once is best for achieving a balanced result, as well as reducing the number of recovery periods to one. So we are happy to combine surgeries that can be safely combined.

“Safely” being the key word.

Here at INEO, we have a strict “no unnecessary risks” policy. This policy sounds so painfully obvious, that it should be naturally implied in any plastic surgery practice. But the reality is that many plastic surgery patients are prepared to take unjustifiably high risks with their health for the sake of convenience and reduced cost.

The main factor restricting combined plastic surgeries is general anesthesia. If the scope of the desired combination of procedures implies a 10–12-hour surgery under general anesthesia, we will decline such a request. The safe limit for general anesthesia is 6 hours, so if your desired combination of surgeries is likely to be longer than that, we will offer to divide the operations into two or more surgeries.

Why do we ask for photos before the free online consultation?

Before they receive their free online consultation with one of our surgeons, we always ask our patients to send photos of the area they would like to receive surgery on. Why do we do it? After all, won't the video call with the surgeon provide a better opportunity for visual assessment?

Most patients send us their inquiries with a clear goal in mind, requesting a surgery that they believe will best achieve this goal. However, in some cases, the surgical solutions the patient requests are not well suited for the aesthetic changes desired, which can be immediately evident from a few photos of the relevant area. The photos allow the surgeon to make initial comments before the consultation, meaning that the patient receives the surgeon’s professional opinion early on, saving time for both the patient and the surgeon should the desired surgery be ill-advised.

In most cases, though, the photos are simply a good way for the surgeon to prepare for the consultation and thus use the consultation time to provide maximum value for the patient, regardless of whether they decide to move forward with the surgery or not.

Some discomfort is to be expected after surgery, especially in the first few days. This discomfort is well-managed by medication, compression garments, and bed rest, but for our female patients, there is another potential source of discomfort to consider - the menstrual cycle.

When setting the surgery date, we advise our patients to avoid getting their surgery during their period, if possible. Doing so may produce some side effects, such as:

🔹 Increased menstrual blood discharge
🔹 More prominent bruising around operated areas
🔹 Lengthier healing duration

Your period won’t prevent you from receiving surgery and will not affect its result, but it's worth considering for the sake of your own comfort, particularly if your periods come with heavy flow and a high degree of soreness.

Tummy tuck and liposuction is a popular combination of surgeries seen as the most effective solution for one’s abdominal aesthetic problems. Indeed, this surgical combination is an excellent way to restore abdominal contours after significant weight loss or pregnancy. Nevertheless, when a surgery is perceived as “the obvious choice”, important considerations are often overlooked:

🔹 It won’t replace weight loss. If the patient has a BMI of over 27, the probability that this surgery is suitable for them begins to decrease rapidly. This surgery is aimed at removing excess skin, repairing abdominal muscles if needed, and using liposuction to bring the abdomen into ideal proportions with the rest of the body. If the patient is far from their ideal weight, then the effects of liposuction will be minimal, and there is a risk of some excess abdominal skin appearing later if they lose weight after the surgery.

🔹 You will temporarily lose sensitiveness on your tummy. This surgery usually involves removing a lot of abdominal tissue, including nerve tissue. Some patients report loss of tactile sensitiveness for months after the surgery. It can take up to 6 months for the nerves to regenerate, restoring sensitiveness eventually.

🔹 Scarring will take time to subside. Even after your initial recovery period is over, scarring may remain prominent, both in color and in texture. It will appear red and hardened at first, with many patients thinking it will remain that way for good, but it will, over time, gain a natural skin color and become softer. About a year after the surgery, it will become a barely perceptible line.

Finally, it’s important to remember that results cannot be evaluated fully in just a month or two, more like 6 to 8 months. Tummy tuck and liposuction may be the best solution for you, but it’s not a quick and easy one.

You will be spending more time in the hotel than the clinic, and it is important to choose a hotel best suited for your recovery. While it is tempting to choose the least expensive option, it probably won't come with the amenities you will need. Here are a few things to consider when choosing accommodation:

📍 Location – you want a hotel close to parks or pleasant parts of town, that allows you to go for walks and enjoy the city. However nice the hotel room might be, staying indoors the whole time is a recipe for boredom.

🥗 Food – not all hotels have a full restaurant service, offering only breakfast and making ordering takeout necessary. Having a hotel that offers 3 meals a day, even for an additional fee, may end up being cheaper and more convenient than ordering takeout.

🛋 Comfort – you are likely to experience restricted mobility during your initial recovery period, as well as restrictions in how you will be able to lay while you sleep. Choosing a room with a comfortable double bed, and a spacious bathroom will significantly improve your post-op experience.

👩‍⚕️ Medical staff – while not essential for most patients, large-scale operations, such as a mommy makeover or body contouring can be particularly challenging for patients. In such cases, a select few hotels offer medical staff for added peace of mind and prompt medical assistance should it become necessary.

❓”How long do I need to stay in the country after the surgery?”

This is among the top 5 most frequently asked questions. The short answer – between 10 to 14 days. During that 10-day period, the patient will have multiple check-ups with the surgeon, with sutures usually removed on day 10. Even if the sutures are dissolvable, it is important to keep a close eye on the sutured area, changing wound dressings regularly, making sure it is healing properly to avoid excessive or uneven scarring.

❓“But why can’t I leave early and have the sutures removed by a doctor at home?”

It is possible, but it comes with significant risk. The convenience of leaving 2-3 days early is simply not worth the additional risk of complications. This is especially true if your trip involves air travel. Even a 3-hour direct flight – the most common kind for our patients – is stress on the body due to air pressure changes and the general air travel environment. Lifting anything heavy, raising arms, bending down, or just walking fast is prohibited for most during the initial healing period. This can sometimes be hard to avoid when traveling, and in such cases the patient risks the reopening of the wound, bleeding, infection, excessive fluid build-up, just to name a few. These can all have a negative effect on the result of the surgery.

We are happy to share with you another Before & After, this time of a rhinoplasty performed by Dr. Renaldas Vaičiūnas.

In January 2021 the patient received open rhinoplasty aimed at the removal of a nasal hump. The patient was discharged after 1 night at the clinic, and 10 days after her surgery the sutures were removed. While the aesthetic improvement is already evident, the patient will enjoy full results of her rhinoplasty in 12-14 months after the surgery.

Pre-operative drawings (or pre-operative markings) are closely associated with plastic surgery and are standard practice. And while the reason for the drawings seems intuitive, there is more nuance to them than most patients expect. So what are these markings for?

It’s a map of the procedures to be done, which is both for the surgeon and sometimes the assisting surgeon, if the operation is sufficiently complex. It marks not only the areas to be operated on, but also the incision locations, which have a direct relationship to the quality of the results.

In addition to being a visual “to-do list”, it helps achieve a precise and symmetrical result. More often than not, the surgeon will draw on patients who are standing upright, which is how the patients will experience and evaluate their transformation. But the surgery will be performed with the patient laying horizontally on the operating table, and the drawings are vital for the surgeon to maintain the “upright” perspective of the patient’s body when performing the surgery.

Finally, there is an added psychological component for the patient, where after being marked, they feel more secure going into surgery.

Dog Ears appear when skin folds at the ends of a surgical incision and begins to protrude in a shape that the name suggests. The likelihood of dog ear formation is highest with surgeries where the skin is tightened using an elliptical incision, such as a tummy tuck or a breast reduction. In such cases, skin tension is unevenly distributed along the incision, with the center of the incision being where the most tension is focused. The tension decreases away from the center, with no skin tension where the incision ends. If the difference in tension is significant enough, the skin may fold on itself slightly at the very edges.

With experienced surgeons, dog ears are a rare occurrence. However, even if dog ears do appear, they can be easily addressed with quick and simple corrective surgery that is done under local anesthesia. Flying back home, you will still be in the early stages of your recovery process, which isn't compatible with heavy luggage. The best way to pack as light as possible is taking only the essentials:

✔️ Negative COVID test and or passport/ID card.
✔️ Comfortable, loose clothing that is easy to put on and take off. Sweatpants, hoodies, buttoned dresses or shirts - all good options. Nothing slim-fit!
✔️ Comfortable footwear, such as slippers or shoes (no laces) that are also easy to get in and out of. Foam clogs or similar type shoes are ideal.
✔️ Personal hygiene items such as menstrual pads, earplugs, deodorant, dry shampoo, toothbrush, toothpaste, etc.
✔️ Electronic devices for entertainment (laptop, phone, e-book reader, etc.) and a socket adapter for their respective chargers, since electrical sockets in Lithuania have two round pins.

All these items will already amount to a sizable suitcase, so make sure your luggage has wheels!
