- Prior to a first appointment, please email
EislekPsych@gmail.com and request a
questionnaire. This is a requirement for the
first appointment in order to better
understand the needs of the patient/family
prior to the first meeting. It must be
returned by email in advance of the first
appointment, otherwise the appointment
will be cancelled.
