Pay attention to the details, they are important.

Spider veins. Do they have significance for us? Significant. Pay attention to them, their location, shape, size, expressiveness, color.
They do not just appear, but have a reason. They are most often found in the area of ​​the stomach, liver or C-th transition. Why did they appear? The reason is chronically increased tension of the tissues and muscles in the area under the veins. Small vessels are compressed and the blood circulation "looks" for another path. If they form a certain shape, always look at what it is, whether it copies an organ or muscle. In the picture you can see the veins on the left side of the rib cage, which extend from the xiphoid process to the lateral side of the rib cage. Their course copies the attachment of the diaphragm and gives us information about chronically increased tension over a long period of time (months to years). But why is the tension of the diaphragm increased in a given area? Something is chronically irritating it. Some organ. On the left side, it is most often the stomach and flexura colli sinistra, the left bend of the colon. But it can also be the pancreas or spleen.
The larger and more pronounced the veins, the more serious the problem and the longer it lasts.
Remember. If you see spider veins that have a shape, copying an organ or muscle, it is definitely not a coincidence. Palpate the area under and see if you find a change in tissue tension and soreness.

Are you going to a private clinic and are you planning to equip the clinic? Do not invest in unnecessary things that will pay off financially in a long time, or you have to change them regularly, and they do not have the same efficiency as your hands.
Working in a private clinic is a business. And in business, economics is important. In economics, mathematics. But fortunately, in our case, simple ones are enough.
What do you need to be able to perform your practice at a high level and not cost you a large amount per month? You definitely need: an electric height-adjustable table. A neurological hammer.
It is advisable to add: One fitball, overball, expander, sensory mat. This is completely enough for you in terms of material equipment. How much does it all cost together? The most expensive item is the table, but it is worth investing in it. The other items are really cheap.
If you want to exercise with patients, buy a TRX or similar, and a Flowin. And you can exercise the patient very effectively. All in all, an investment of a few tens of euros. Return? Very fast.
Investing in machines, cupping, creams, etc. is expensive and is not nearly as effective as your hands and knowledge. Especially machines. They are very expensive, the effectiveness is not even close to correlating with the price. And the really effective ones, such as a high-power laser (tens of watts) cost tens of thousands of euros. But how many patients really need it? Low percentages. These machines are enough in hospitals or large clinics.

Rather invest in quality education and literature, it will pay off very quickly.

Pay attention to the chest and ribs. Their shape is not a coincidence

Yesterday we presented an interesting case study. Today we continue and follow up. Did you notice anything else besides the wrinkle? Look at the photos. What is circled is called lateral impressions. They are such depressions and “indentations” of the lateral wall of the chest. Sometimes they are larger, sometimes smaller. Do you know what it is? No, it is not a developmental defect or rib anatomy. It is a chronically contracted diaphragm in a given place. Chronically contracted diaphragm fibers. In a given place, the diaphragm “does not breathe”. Its respiratory, but also stabilizing ability is significantly limited in given places. But why? For what reason? Something is pulling inward. Some attachment has changed tension. Nociceptive increased. What organ? What is attached there? For example, on the left side, the stomach and the left flexura colli sinistra, the colonic loop, which is under the stomach, attach to the diaphragm in the given places. If the tension of the stomach changes for any reason, the tension of its superficial fascia and its attachment to the diaphragm will also change. Will the tension of the colonic loop also change? It can, it is right under the stomach and also has an attachment to the diaphragm.

Remember, every lateral impression is important for us. The patient may not always have symptoms of an organ disorder, e.g. GERD, dyspepsia, or constipation. He may come with a headache, shoulder, neck, interscapular pain, ... But the body is already letting us know where the problem is. Something is pulling inward and decompensating the diaphragm. We have to palpate the abdomen and find the source of the pathology.
Because it is true: the brain primarily protects its organ.

We had a very interesting case in the clinic. And we have similar cases very often.

A lady who has been suffering from pain and dizziness for more than a year. She took a lot of medications that did not help at all. She went to physiotherapy, where they only exercised. And massages.
The interscapular muscles, lower fixators of the scapula, but also the muscles of the neck and shoulders were mainly exercised. The assumption was that the headaches were due to weakened muscles and needed to be strengthened. Rehabilitation lasted more than a year and without any result. Massage sometimes had a negative effect and only worsened the condition.
You can see 2 important things in the picture. The first is a pathological scar after appendectomy. The second is a wrinkle in the epigastrium, stomach area, pylorus, gallbladder. Both are important for us. In addition to chronic pain and dizziness, the history showed that the lady often has a bloated abdomen, digestive problems, and does not tolerate fatty foods well.
What does this tell us? The scar after appendectomy is not only on the skin, subcutaneous tissue, muscles, but also on the organ, in this case the caecum - the beginning of the large intestine. We must palpate each scar, find out if it has any signs of pathology - pain, pathological barrier, sensitivity, etc. And this one had them all. Another important one - the neurophysiological relationship caecum-stomach-pylorus - gastrocecal reflex. A problem in the caecum area will change the function and evacuation capabilities of the pylorus. It does not open sufficiently and the patient has dyspepsia. That is neurology.
Epigastric fold - every fold on the abdomen means something to us. A fold is a chronically contracted tissue. As can be seen in the picture, the fold is pulled inward. The stretched tissue protects something, in this case the stomach and duodenum, which had altered mobility and motility, which was manifested by dyspepsia. The change in mobility and motility of the pylorus and duodenum was manifested by intolerance to fatty foods and often a bloated abdomen.
How did we solve the case? In this case, the only therapy is visceral manipulation. We have to work with the scar. Release the caecum, pylorus, stomach, omentum minus and other organs, organ loops and soft tissues in the abdomen. No other approach will work.
After the first therapy, the headaches disappeared after more than a year, after the second therapy, the dizziness.
It is not true that the muscle that hurts is weak. But it is true that the muscle is hypertonic for some reason and we need to find that reason. Is the muscle hypertonic? Why? Where is the primary cause?
Remember: does the patient have a headache? ALWAYS LOOK INTO THE ABDOMEN. ALWAYS. And when the problem is in the abdomen, you have to solve it because: THE BRAIN IS THE PRIMARY ORGAN THAT PROTECTS ITSELF. Differential diagnosis is the alpha and omega. 6 days of pediatric course with great instructor Jane and NDTA - Neuro-Developmental Treatment Approach.
It was a very intensive course, but we learned a lot. Jane did not only teach techniques, proper grip, manipulation, handling, or approach to the patient, but she taught the most important thing. The musculoskeletal system reacts to any changes in our body. Whatever happens, the musculoskeletal system reacts with so-called functional changes. These are manifested by a change in muscle tension (increase) in the muscles innervated by a specific segment, a change in joint mobility (joint blockade) in specific segments, a change in skin and subcutaneous tissue tension, a change in the stretchability of the skin and subcutaneous tissue, a change in the blood supply to the skin and muscles innervated by a specific segment, a change in sweating in a specific segment.
A segment is understood as the innervation area of ​​a nerve that comes from a specific area of ​​the spine. Functional or reflex changes are based on nociception.
As physiotherapists, we are able to diagnose all these changes and therefore find the primary cause. That is why our hands and knowledge of differential diagnosis are essential. No device can do this yet. None. And that's why it's essential to understand what a trigger point in a muscle means. If the problem is, for example, the stomach, it will manifest itself in specific segments with specific changes. Will we exercise a muscle that has been changed and is sore like this? Definitely not. Remember. first remove the source of nociception, then exercise. It doesn't work the other way around.

Your economy matters. To perform our work at a high level, with quality, precision and efficiency and treat patients, a physiotherapist only needs 4 basic things: hands, knowledge, a neurological hammer and a table. That is the basis, which is sufficient in most cases. We can add a stimulation mat, quality literature and a fitball as a supplement. We don't need more. Just calculate how much all the tools, or devices, cost. And they need to be changed over time. And efficiency is not at all the same as your hands. We must master differential diagnostics qualitatively and honestly, understand what is happening under our hands and know the techniques with which we treat a given problem. It is not true that the pain comes from a weak muscle and we need to strengthen it. It is exactly the opposite and the muscle that is hypertonic hurts much more often. Yesterday we wrote about the case study of a patient with pain between the shoulder blades. What can cause pain between the shoulder blades? The most common causes include trigger points from the muscles: rhomboidei, infraspinatus, scalenii, serratus posterior superior, multifidi, latissimus dorsi, trapezius, levator scapulae. Pain between the shoulder blades can also be caused by joint blockades of the C-Th and the entire thoracic spine or vertebrocostal joints. However, we must not forget about visceral pain. Pain from the stomach or duodenum typically occurs between the shoulder blades.
When we think about it, what would we strengthen? The most important thing is to find out the primary cause of the pain. That is why differential diagnosis is the alpha and omega for us. That is why we must think comprehensively and beware of a very simplified view of the type - it hurts because it is weak and we must strengthen it. If the pain comes from a joint blockage or an organ, exercising will only worsen the condition.
Therefore, remember - first solve the source of nociception, then exercise!

No, it is not true that the muscle that hurts is weak and we need to strengthen it.

We hear this very often in the ambulance from patients and also from colleagues - if the muscle hurts, it is because it is weak, we will strengthen it with exercise and the pain will disappear. But it is not like that at all. Thinking this way is not only overly simplified, but also very wrong. On the contrary, much more often the muscle hurts when it is overloaded and hypertonic. And strengthening a hypertonic muscle will only support the pathology.
The last time we had a lady in the clinic who went to physiotherapy with pain between the shoulder blades. The pain was supposedly because the interscapular muscles are weak and need to be strengthened. But the lady felt more pain after each exercise. However, the therapy continued and the lady eventually completely decompensated and the pain was permanent. When she came to us, we found out that the pain was transferred from the stomach, presented in the Head zone of the stomach. Therefore, differential diagnosis is very important and it is not true that the muscle that hurts is weak.
