A 68-year-old woman with a 20-day history of exertional dyspnoea, acrocyanosis, and critical ischaemia—dry gangrene—of her fingers, presented to a rheumatology clinic.

She also reported the recent onset of pain in the small joints of her hands and wrists, Raynaud’s phenomenon in her fingers and toes, and a feeling of general fatigue. She had a medical history of well-controlled hypertension treated with nebivolol and had never smoked ci******es.
