باهي كيف نشترك أنا كطالب طب وكطبيب ؟

أكيد موقع بهذه الامكانيات مارح يكون مجاني و فيه اشتراك سنوي لازم يندفع ، ويختلف من بلاد لبلاد ، مثلا اشتراك لمدة سنة في ال US حيكلفك حوالي 519 دولار ، فيه بعض الدولة يديرو في اتفاقيات مع ال UpToDate ويوفرو فيه بشكل مجاني للدكاترة الي في بلدانهم ، ومنها النرويج والسعودية ، أي بمعنى الدكتور سعودي على سبيل المثال متاح ليه ال UpToDate بشكل مجاني ( #وزارتنا ديرولنا لقطة الموضوع ساهل ومايكلف واجد وبحوث علمية معتبرة تشير لتحسن النظام الصحي للدول بعد ماتستخدم في الuptodate)

- في برنامج اسمه Better Evidence UpToDate Donation Program برعاية Harvard university and university of birmingham للدول أو المناطق الي يكون النظام الصحي متهالك وتقدر تقدم وتحصل على full access أنت والمستشفى ولا الكلنك متاعك بشكل مجاني أو بسعر مخفض

تجربتي مع برنامج Better Evidence :

عبيت الأبليكيشن يوم 1-6-2020 وقالولي ممكن تاخد 45 يوم عمل بيش نردو عليك ، والصراحة ماكنتش نتوقع انهم يردو عليا قريب ، بس بعد 12 يوم جاني ايميل انه ال application متاعي تم قبوله وحصلت free access وانه بيانات الدخول توى يبعتوهم في ايميل آخر الأيام الجاية

الأبليكيشن ساهل وبسيط ، عبيت بيانات واضحه وطلب مني اني نتكلم بتفصيل على السبب تقديمي على هدا البرنامج ووين نشتغل وتفاصيل اخرى ، كتبتهم حوالي 500-600 كلمة ووضحت فيها بالتفصيل الي مطلوب

حبيت نشارك معاكم بالك حد يستفيد

الدليل الإرشادي لرصد المصابين بفيروس كورونا المستجد COVID -19

الأعراض :
1 – ارتفاع حرارة الجسم
2 – أعراض تنفسية ( سعال, ضيف في التنفس )

أولا : حالة الاشتباه:
أ - حالة تجاوزت حرارتها 38 درجة مئوية ولديها واحدة من الأعراض التنفسية, شريطة أن :
1 – يكون على اتصال مباشر خلال ( 14 يوما ) بشخص ثبت إصابته بفيروس كورونا
2 – كان بإحدى الدول الموبوءة خلال (14 يوما) من عودته للبلاد ( أي قبل ظهور الأعراض)

ثانيا : الحالة المحتملة
حالة يشتبه إصابتها, وكانت النتائج المختبرية:
1 – لا يوجد نتيجة حاسمة لفيروس كورونا خلال التحليل المختبري.
2 – نتيجة إيجابية شاملة لفيروسات كورونا, مع عدم وجود بيانات مختبرية لمسببات مرضية أخرى للجهاز التنفسي

ثالثا : الحالة المؤكدة :
حالة تم تأكيد إصابتها بفيروس كورونا المستجد مختبريا بغض النظر عن العلامات والأعراض.

الإجراءات الوقائية :
1 - مراقبة القادمين من المناطق التًي ثبت بها انتشار المرض.
2 – انصح بالابتعاد عن مصدر العدوى وأماكن الازدحام
3 – انصح بالتهوية الجيدة وارتداء الكمامات مع غسل اليدين
4 – انصح بالابتعاد عن المريض بمسافة تضمن عدم وصول الرذاذ
5 – انصح بالابتعاد عن أماكن الازدحام وعدم مخالطة المصابين

لغاية هذه اللحظة، ما زال أغلب الناس يعتقدوا أن كورونا فايروس يحمل أعراض قويّة بينما في الواقع هو العكس تمامًا، خاصة لما يكون الإنسان المصاب بيه مايشكيش من أمراض مزمنة.

أعراضه مشابهة لنزلات البرد الموسمية وتتمثل في التعب والإرهاق، ارتفاع درجة حرارة جسمك أكثر من 38 درجة مئوية، كحة جافة أو ممكن يصير مع بعض المصابين حتى حالات من الإسهال أو ترجيع.

بحسب منظمة الصحة العالمية، أغلب الناس ممكن مايطلعش عليها أصلا أعراض؛ لكن 1 من اصل 9 إجمالي عدد الإصابة ممكن تتحول لإصابة خطرة كالصعوبة في التنفس لدى الكبار في السن والمصابين اللي يعانوا من أمراض قلبيّة أو داء السكري أو تحت الأدوية المهبطة للمناعة.

Novel coronavirus in , (January 2020)

أكيد سمعتو بال لل الي صايرة في الصين وبعض دول شرق آسيا وفيه حتى cases وصلو لأمريكا وكندا وأستراليا !
القصة بدت في نهاية السنة الي فاتت تحديداً في مدينة صينية اسمها Wuhan اسجلن بعض الحالات، العامل المشترك بينهم أنهم كان يترددو على مطعم في المدينة يقدم في وجبات من لحم الذئاب والجرذان والخفافيش وغيرها !
حاليا أكثر 250 حالة مصابة بالفايرس ، منهم 50 حالة حرجة ، وبعض المسافرين من المدينة نفسها نقلوا المرض لدول أخرى حول العالم ، بس السلطات الصينية تداركت الموقف ودارت حجر صحي للمدينة .
المصابين كان عندهم حرارة وال CXR وضحت bilateral infiltration ، بالإضافة في بعض الحالات صارلهم kidney failure ، و 9 حالات وفاة اسجلن فيما فات
الWHO نصحت بعدم السفر للأماكن الي ممكن تصير فيها العدوة.

2 years old child admitted due to generalized fits and vomiting early in the morning. It stated with excessive crying, preceded by generalized fits lasted for 3-5 minutes, and drowsiness for few hours after the fits. She is a known case of cyanotic congenital heart disease, on long term medication of propranolol, during the episode, she is more blue than usual. On examination, she is afebrile, dusky lips and toe and failure to thrive, tachypnea, has ejection systolic murmur grade 2/6 at the left parasternal area. CNS examination showed no neurological deficit.

سبيتار جهزتلكم برنامج خاص يسهّل عليكم كل شيء من حجز موعدك مع دكتورك والعلاج والاستشارات والنصائح الطبيّة، وإنت في مكانك وبكبسة زر بس .. كيف ؟

- قبل كل هذا خلينا نشرحوا معنى (التطبيب عن بعد) وشن حتستفيد منه؟

التطبيب عن بعد: هو نظام حديث لمتابعة المريض من قِبل دكتوره، يعني تقدر تتواصل مع دكتورك وتستشيره، وتسأله في أي وقت وفي أي مكان عن طريق تطبيق وموقع خاص تسجل من خلاله وتتحصل على كل الخدمات الطبيّة إللي تحتاجها.

يومكم مبارك !

اليوم بنتكلمو على واحد من أفضل البرامج الي ممكن يستفيد منها طالب العلم بشكل عام وخاصة طالب الطب واللغة ، وهو ال ANKI

فكرته أنه يساعدك على حفظ ومراجعة المعلومات بطريقة ال flash cards ، لكن مايميز ال Anki عن ال Flash cards التقليدية هي تركيزه على المعلومات الصعبه وإهمال السهلة ( بناءً على مستوى تذكر المستخدم للكلمات ) ، رغم أنه الفكرة بسيطه لكن مفيدة هلبا ، لأنها بتخلي عملية الحفظ والمراجعة المعلومات عملية مثمرة ومركزة على المطلوب فقط

الفكرة تتلخص في النقاط التالية :

* أنك اضيف أي معلومات جديدة تمر عليك إلى البرنامج في Decks معينه ( وفي Decks واتية توى نحط رابط ليهم )
* افتح البرنامج كل يوم عشان تراجع المعلومات السابقة
* قيم كل معلومة تراجعها بناءً على قوة تذكرك لها
* والباقي على البرنامج، لكن شن هو الباقي ؟

بعد تقوم بإخبار البرنامج مدى صعوبة هذه المعلومة أو تلك (بواسطة النقر على الأيقونة المناسبة تحت كل معلومة أثناء عملية المراجعة) يقوم البرنامج بجدولة المعلومة بحسب صعوبتها، فكلما كانت المعلومة صعبة كلما قرب موعد عرضها القادم عشان تراجعها وتقيمها من جديد ، وكل ماكانت المعلومة أسهل كلما ابعد البرنامج موعد عرضها القادم.

مثلاً: لو ضفت معلومة جديدة إلى البرنامج في يوم السبت، عندما تفتح البرنامج يوم الأحد حتشوف المعلومة معروضة عليك لمراجعتها، إذا أنت قد نسيت المعلومة أو أنك تتذكرها بصعوبة، فقم بتقييمها بدرجة منخفظة، البرنامج سيعرف أنها صعبة عليك وحيعيد عرضها عليك في اليوم التالي (الإثنين)، إذا رأيت أنك تتذكرها بشكل جيد وممتاز فقم بتقييمها بدرجة مرتفعة، البرنامج سيفهم أنها سهلة بالنسبة إليك وسوف يقوم بعرضها عليك بعد ثلاثة أيام أو أكثر.

بالنسبة لتطبيقات ال ANKI متوفرة على مختلف الأنظمة والأجهزة (وندوز – لينكس – ماك – أندرويد – IOS – بلاك بيري )، بالإضافة إلى تطبيق الويب (Web Application) ، بشكل مجاني ماعدا الIOS فهو مع الاسف غير مجاني ( لكن يستاهل كل دولار تصرفه فيه )

لما المريض ياخد exogenous steroid لاكثر من اسبوعين يصير suppuration for HPA axis ويبدا الجسم معتمد على دواء الكورتزون ..
لو وقفناه على فجاة " حيقعد الجسم من غير cortisone الحالة صارلها " adrenal insufficiency “ ويخش المريض في adrenal crisis " و الحالة هني تعتبر emergence ..

فلما نعطوه العلاج لاكثر من اسبوعين وبعدين الحالة بدت تتحسن " اي كان التشخيص “ لازم انقصوا في الجرعات تدريجياً ونديروا slowly withdrawal .. لتفادي adrenal crisis

Mediterranean diet and cardiovascular disease

The majority of studies showing an association between a Mediterranean diet and health benefits, including reduction in cardiovascular disease (CVD), have included only observational data. The few available randomized trials (including the large PREDIMED trial) on the effectiveness of a Mediterranean diet for primary and secondary prevention of CVD have been evaluated in a new meta-analysis . Among individuals without CVD, a Mediterranean diet reduced the risk of stroke, but did not reduce overall or cardiovascular mortality. Among patients with known CVD, a Mediterranean diet reduced both total and cardiovascular mortality. However, the small number of included trials and low quality of evidence limits the certainty of the effects of a Mediterranean diet on these clinical outcomes. We advise that the choice among diets to maintain good health (eg, low fat, vegetarian, Dietary Approaches to Stop Hypertension [DASH], and the Mediterranean diet), be based on individual risk factors, personal preference, and the ability to adhere to a given diet.

White coat hypertension and risk of cardiovascular events

White coat hypertension is defined as elevated office blood pressure with normal out-of-office blood pressure readings, and white coat effect is defined as elevated office blood pressure in a patient treated for hypertension with normal out-of-office readings. The risks associated with white coat hypertension and white coat effect are unclear. In a meta-analysis of 27 studies comparing cardiovascular outcomes of patients with white coat hypertension or effect versus normotensive patients, white coat hypertension was associated with an increased risk of cardiovascular and all-cause mortality, and a nonsignificantly higher risk of all cardiovascular events. In contrast, white coat effect was not associated with increased risk. While we do not initiate pharmacologic treatment for white coat hypertension, lifestyle modification should be encouraged and out-of-office blood pressure measurements monitored for the development of worsening or sustained hypertension.

use in patients with nonalcoholic fatty liver disease

Nonalcoholic fatty liver disease ( ) begins with simple steatosis, but may progress to nonalcoholic steatohepatitis ( ) and advanced fibrosis. While lifestyle interventions (eg, weight loss) are important, pharmacologic therapies are limited. In a recent cohort study of over 360 patients with NAFLD, daily aspirin use was associated with a lower baseline risk of NASH and fibrosis, and a lower risk of disease progression over time compared with non-regular aspirin use. These results are promising, but we would like to see confirmatory data from a randomized trial before recommending aspirin for hepatoprotective effects.

حبة وحدة قادرة على التخفيف من خطر الاصابة بالجلطات 🌟

فى دراسة اكلينيكية PolyIran فى مجلة Lancet جمع الباحثون حوالى 7000 شخص بالغ ( أعمارهم ما بين 40 الى 75 عام ). بعضهم يعانى و بعضهم لا يعانى من أمراض القلب) و قسموهم مجموعتين: مجموعة غيرولهم ال life style 🌟و مجموعة غيرولهم ال life style بالاضافة الى اعطائهم حبة تحتوى على أربعة أدوية (4-Drug PolyPill) و هم: Aspirin و Hydrochlorothiazide و Enalapril فى الأشخاص اللي عانوا من الكحة كعرض جانبى تم استبداله ب Valsartan) و Atorvastatin وتابعوا المجموعتين لمدة 60 شهر 🌟

وجد الباحثون ان المجموعة اللي تلقت العلاج بالاضافة لتغيير ال life style أظهرت انخفاضا واضحا فى معدل حدوث أزمات القلب MI و الدماغ Stroke و الموت المفاجئ Sudden Death (سواء فى المرضى اللذين يعانون او لا يعانون من أمراض القلب) .
بالرغم من أن النتائج مبشرة و توحي بأننا نقدروا نحموا الأفراد من مضاعفات القلب عن طريق تطبيق صحي عام Public Health Intervention الا أنها أثارت جدلا واسعا، لان الدراسة تعتمد على مبدأ One size fits all أى حبة واحدة بنفس الخليط من الأدوية تناسب الجميع و هذا غير منطقي 🌟. و كذلك لأن الدراسة تم تصميمها فى 2011 قامت بادراج الأسبرين فى الدواء المجمع ... و لكن دراسات عديدة فى 2018 أثبتت أنه لا توجد فائدة للأسبرين فى الوقاية من أمراض و مضاعفات القلب فى الأشخاص الطبيعيين Primary Prevention 🤔.
