A 68-year-old man comes to the emergency department with a 3-week history of progressive dyspnea, orthopnea, and lower extremity edema. His past medical history is significant for hypertension, type 2 diabetes mellitus, myocardial infarction 8 years ago, and congestive heart failure. Current medications include metoprolol, digoxin, enalapril, furosemide, spironolactone, and aspirin. His blood pressure is 135/80 mm Hg, and pulse is 75/min and regular. Symmetric 2+ pitting edema of the lower extremities is present. Point of maximal impulse is displaced to the left and a soft holosystolic murmur is heard at the apex. السوفتوير UpToDate يعتمد عليه أكثر من 1.7 مليون مركز صحي في حوالي 190 بلد حول العالم ، وهو متاح أنلاين و أفلاين وكتطبيق على الهواتف ، ويقدم المعلومات والبروتوكولات الأحدث في جميع المجالات الطبية ، و يقدم خدمة وصول لـ Lexi-comp ومعلومات عن drug-to-drug, drug-to-herb and herb-to-herb وفيه مجوعة من المنظومات الحسابية الجاهزة الي تفيد الطبيب في patient management

باهي كيف نشترك أنا كطالب طب وكطبيب ؟

أكيد موقع بهذه الامكانيات مارح يكون مجاني و فيه اشتراك سنوي لازم يندفع ، ويختلف من بلاد لبلاد ، مثلا اشتراك لمدة سنة في ال US حيكلفك حوالي 519 دولار ، فيه بعض الدولة يديرو في اتفاقيات مع ال UpToDate ويوفرو فيه بشكل مجاني للدكاترة الي في بلدانهم ، ومنها النرويج والسعودية ، أي بمعنى الدكتور سعودي على سبيل المثال متاح ليه ال UpToDate بشكل مجاني ( #وزارتنا ديرولنا لقطة الموضوع ساهل ومايكلف واجد وبحوث علمية معتبرة تشير لتحسن النظام الصحي للدول بعد ماتستخدم في الuptodate)

- في برنامج اسمه Better Evidence UpToDate Donation Program برعاية Harvard university and university of birmingham للدول أو المناطق الي يكون النظام الصحي متهالك وتقدر تقدم وتحصل على full access أنت والمستشفى ولا الكلنك متاعك بشكل مجاني أو بسعر مخفض

تجربتي مع برنامج Better Evidence :

عبيت الأبليكيشن يوم 1-6-2020 وقالولي ممكن تاخد 45 يوم عمل بيش نردو عليك ، والصراحة ماكنتش نتوقع انهم يردو عليا قريب ، بس بعد 12 يوم جاني ايميل انه ال application متاعي تم قبوله وحصلت free access وانه بيانات الدخول توى يبعتوهم في ايميل آخر الأيام الجاية

الأبليكيشن ساهل وبسيط ، عبيت بيانات واضحه وطلب مني اني نتكلم بتفصيل على السبب تقديمي على هدا البرنامج ووين نشتغل وتفاصيل اخرى ، كتبتهم حوالي 500-600 كلمة ووضحت فيها بالتفصيل الي مطلوب

حبيت نشارك معاكم بالك حد يستفيد

Disease: COVID-19

- Mild to severe / fatal
- Fever, cough, shortness of breath is the usual presenting symptoms
- Pneumonia, Severe Acute Respiratory Infection
- Some reported case of encephalitis

▪️For health care workers, understand that:

- Case fatality rate:
Mers Cov 34.4%
Sars 9.6%
Covid19 3.4%

- Symptoms:
80% mild symptoms
77 - 98% fever
46 - 82% cough
11 - 52% Myalgia
3 - 31% SOB
Infection in children, less common

- Hospitalised population:

الدليل الإرشادي لرصد المصابين بفيروس كورونا المستجد COVID -19

الأعراض :
1 – ارتفاع حرارة الجسم
2 – أعراض تنفسية ( سعال, ضيف في التنفس )

أولا : حالة الاشتباه:
أ - حالة تجاوزت حرارتها 38 درجة مئوية ولديها واحدة من الأعراض التنفسية, شريطة أن :
1 – يكون على اتصال مباشر خلال ( 14 يوما ) بشخص ثبت إصابته بفيروس كورونا
2 – كان بإحدى الدول الموبوءة خلال (14 يوما) من عودته للبلاد ( أي قبل ظهور الأعراض)

ثانيا : الحالة المحتملة
حالة يشتبه إصابتها, وكانت النتائج المختبرية:
1 – لا يوجد نتيجة حاسمة لفيروس كورونا خلال التحليل المختبري.
2 – نتيجة إيجابية شاملة لفيروسات كورونا, مع عدم وجود بيانات مختبرية لمسببات مرضية أخرى للجهاز التنفسي

ثالثا : الحالة المؤكدة :
حالة تم تأكيد إصابتها بفيروس كورونا المستجد مختبريا بغض النظر عن العلامات والأعراض.

الإجراءات الوقائية :
1 - مراقبة القادمين من المناطق التًي ثبت بها انتشار المرض.
2 – انصح بالابتعاد عن مصدر العدوى وأماكن الازدحام
3 – انصح بالتهوية الجيدة وارتداء الكمامات مع غسل اليدين
4 – انصح بالابتعاد عن المريض بمسافة تضمن عدم وصول الرذاذ
5 – انصح بالابتعاد عن أماكن الازدحام وعدم مخالطة المصابين

لغاية هذه اللحظة، ما زال أغلب الناس يعتقدوا أن كورونا فايروس يحمل أعراض قويّة بينما في الواقع هو العكس تمامًا، خاصة لما يكون الإنسان المصاب بيه مايشكيش من أمراض مزمنة.

أعراضه مشابهة لنزلات البرد الموسمية وتتمثل في التعب والإرهاق، ارتفاع درجة حرارة جسمك أكثر من 38 درجة مئوية، كحة جافة أو ممكن يصير مع بعض المصابين حتى حالات من الإسهال أو ترجيع.

بحسب منظمة الصحة العالمية، أغلب الناس ممكن مايطلعش عليها أصلا أعراض؛ لكن 1 من اصل 9 إجمالي عدد الإصابة ممكن تتحول لإصابة خطرة كالصعوبة في التنفس لدى الكبار في السن والمصابين اللي يعانوا من أمراض قلبيّة أو داء السكري أو تحت الأدوية المهبطة للمناعة.
