This happened the minute I landed back from IcureStroke 2023 in Istanbul.
For a neurovascular surgeon with a capacity in both techniques Endo and micro, coiling ruptured MCA aneurysms may sometimes feel inadequate, but sometimes it is precisely the thing to try.
for this 45 Yo woman who suffered a subarachnoid hemorrhage WFNS II and who came to our service at day 10 after "med-wandering" from a city to another .., I decided to first try an endovascular coiling as the neck was right at the M1 bifurcation with all the three branches arising next to the neck without overlapping. I was ready to convert to a microsurgical round. The coiling procedure was somehow artistic trying to achieve the best framing possible using the best loop diameter. the coiling was eventually fine with total occlusion and patency of all branches was verified. uneventful course and a few days stay at ICU.
these life saving endovascular procedures are unfortunately usually not considered as major surgeries in our background as compared to when I do craniotomies for clipping. Only my coronary arteries know about it.
I am happy this patient is safe. she will be back to her family soon while I hope I can really upgrade the workflow to a decent one and be able to spend some days off with my family. I Know Aneurysms calls are duty calls ..
