How to apply for an aeromedical certificate - certification (7/7)

Once all aspects of your medical examination are completed and are confirmed to be in line with EASA medical requirements, the Aeromedical Examiner will proceed to issue the applicant with his/her medical certificate. The certificate includes information relating to expiration dates and planned future tests that will need to be completed before the next renewal examination. Any limitation imposed by the AME will be included on the medical certificate. A revalidation exam may be undertaken up to 45 days prior to expiration date.

Furthermore, the certificate includes important information concerning "decrease in medical fitness" in which case pilots/cabin crew are informed that they should cease exercising the privileges of their licence and to inform their AME as soon as possible

Once the certificate has been completed, signed and stamped by the AME (using his/her specific AME number), the medical certificate is given to the pilot. All documents relating to the medical examination, including a copy of the applicant's medical certificate and identification document, are finally submitted to the state of license of issue. A copy is also kept by the AME for record purposes.

How to apply for an aeromedical certificate - specialist reviews (6/7)

As part of your initial assessment, detailed psychological and psychiatric evaluations as well as toxicological analysis, will comprise your medical exam.

These reviews help explore any underlying mental health issues (active and past) that could interfere with the applicant's ability to undertake safely a career in aviation. Personality testing may also form part of this important evaluation.

Toxicology tests likewise explore the possibility of associated drug and alcohol abuse that apart from interfering directly with an applicant's ability to operate an aircraft may result in long term medical consequences.

Further specialist assessments or tests may be recommended by the AME based on any abnormalities identified on these reviews.

For further information regarding these tests or any other aspects of your aeromedical exam feel free to contact us.

How to apply for an aeromedical certificate - specialist reviews (5/7)

As part of your initial assessment, certain specialist assessments will need to be conducted with the completion of the appropriate and approved forms.

These specialist reviews include:

1. Assessment by an Ear, Nose and Throat specialist which includes formal hearing tests as well as assessments of the nasal cavities, ear canals and sinuses.

2. Assessment by an ophthalmologist (eye specialist) in order to identify any visual defects and recommend correction where applicable and to correlate the severity of any identified defects to the medical standards.

Further specialist assessments or tests may be recommended by the AME based on any abnormalities identified on examination or in the blood tests.

How to apply for an aeromedical certificate - blood testing and spirometry (4/7)

As part of your assessment, the AME will request blood tests to assess your general health and identify any potential sub clinical medical issues. The complete blood count allows the AME to exclude anaemia whilst that a fasting lipid test is required above age 40 as part of the pilot's screening for heart disease. Other tests may be requested by the AME according to the pilot's history and findings on examination.

A spirometry test or breath test is a test performed during your initial exam. This test allows the AME to screen for underlying lung disease affecting the pilot's airways, including asthma or restrictive lung disease. During your renewal exam, the spirometry is not generally requested but instead a peak flow test is performed by the AME as part of the general examination.

How to apply for an aeromedical certificate - the electrocardiogram (3/7)

The ECG is a test which measures the electrical activity of the heart. It forms part of all initial examinations. Thereafter, the frequency of this ECG will depend on the class of medical applied for - however, the aeromedical examiner may refer the pilot for an ECG test earlier whenever this is indicated. Many times, abnormalities identified on the ECG by the reporting cardiologist (a doctor who specialises in the function of the heart), may recommend further tests, including stress testing, holter exams as well as an echocardiogram test.

Rhythm abnormalities such as incomplete bundle branch blocks will be investigated if other abnormalities are present on cardiovascular examination. All other rhythm abnormalities or abnormal ECG pattern (such as complete bundle branch blocks, atrial fibrillation broad/narrow complex tachycardia, pacemakers, AV conductionn, Q waves, ST/T wave abnormalities) will be reviewed by a cardiologist and further tests may be required. The cardiologist, after reviewing the pilot and the results of the tests, will then advice the AME on the pilot's fitness to fly.

I would like to thank Dr Elton Pllaha (cardiologist at AeroMedics Malta - EASA & FAA AeroMedical Examination Centre) for the above contribution.

How to apply for an aeromedical certificate (2/7)

A general medical examination will be performed by the AME once you complete your application form. This will take the form of a complete medical examination that includes your height, weight, blood pressure, heart, lungs, stomach, limbs, skin, eyes and vision (including colour vision), ears and hearing and finally the nervous system (including balance).

At the end of your medical a urine test will be performed to look for the presence of blood or glucose in the urine (which may be indicative of an underlying medical issue). A peak flow meter test will also be performed to assess your breathing capacity.

The above thorough examination is aimed at identifying any issues which may interfere with the aircrews career or issues which may lead to in-flight disability. Once identified, they will be rectified by the AME together with his team of specialists.

Feel free to enquire further on any questions that you may have relating to your aeromedical.
