Bone Turnover Markers
The field of bone turnover markers has developed considerably in the past decade. Biochemical monitoring of bone metabolism depends upon measurement of enzymes and proteins released during bone formation and of degradation products produced during bone resorption.

A summary list of bone resorption markers is as follows:
Urinary hydroxyproline
Urinary total pyridinoline (PYD)
Urinary free deoxypyridinoline (DPD)
Urinary collagen type 1 cross-linked N-telopeptide (NTX)
Urinary or serum collagen type 1 cross-linked C-telopeptide (CTX)
Bone sialoprotein (BSP)
Tartrate-resistant acid phosphatase 5b

C or N telopeptide (NTC/CTX):
Elevated levels can be used in the confirmation of increased bone turnover.
Elevated levels can identify persons with osteoporosis who have elevated bone turnover and who, as a result, are at increased risk for rapid disease progression.
The patient's response to antiresorptive osteoporosis treatment can be monitored through this test.
This test can be used to monitor and assess how effective antiresorptive therapy has been in patients treated for disorders such as osteopenia, osteoporosis, and Paget disease.
This test can also serve as an adjunct means of monitoring patient response to other treatments for diseases with increased bone turnover, such as rickets, osteomalacia, and hyperthyroidism.
Using pretreatment beta-CTX levels as a baseline, adequate therapeutic response is indicated by a decrease of 25% or more 3-6 months after the initiation of therapy.

The most commonly performed cervical spine procedure is discectomy, either with or without fusion (for the rest of this review, these will be referred to as fusion and simple discectomy, respectively). Posterior laminoforaminotomy is less common and falls outside the scope of this review. Fusion and simple discectomy have good clinical results. For both types of surgery, a high rate of fusion, up to 80% in simple discectomy and more than 95% in fusion, has been reported.

Low back pain (LBP) is the world's most debilitating condition. Disk degeneration has been regarded as a strong determinant associated with LBP.
Overweight/obesity continues to rise in many populations, this could theoretically also increase the prevalence of disk degeneration and LBP. Although evidence is mounting to suggest that lumbar disk degeneration is also part of the spectrum of “obesity-related diseases” as is diabetes and cardiovascular disease, understanding the interaction between genetics, metabolism, environmental factors, and adipokines will broaden the pathophysiologic understanding of disk degeneration.
Obesity can increase the technical difficulty and complication rates of surgery due to exposure problems and associated cardiopulmonary disorders. This directly leads to an increase in health care cost with increased length of stay, consumption of resources, and in-hospital morbidity and mortality.[105] Also, obesity can further affect postoperative subjective outcome status.
