A heartfelt gratitude to YKBS 干细胞移植基金会 for the wonderful opportunity of sharing our experiences with using Traditional Chinese Medicine as adjunct treatment for cancer. As we all know, cancer is not an easy disease to manage and a multi-faceted approach is needed in order to ensure patient's compliance and survival. This sharing session did just that and hopefully it can offer a glimpse on how TCM can help with managing cancer as an adjunct therapy together with modern medicine.

𝗔 𝗖𝗮𝘂𝘁𝗶𝗼𝗻𝗮𝗿𝘆 𝗧𝗮𝗹𝗲 𝗳𝗼𝗿 𝗩𝗼𝗹𝘂𝗻𝘁𝗲𝗲𝗿𝘀: 𝗠𝘆 𝗘𝘅𝗽𝗲𝗿𝗶𝗲𝗻𝗰𝗲 𝘄𝗶𝘁𝗵 𝗮 𝗖𝗵𝗮𝗿𝗶𝘁𝘆 𝗢𝗿𝗴𝗮𝗻𝗶𝘀𝗮𝘁𝗶𝗼𝗻

I recently had an unsettling experience while volunteering as a Traditional Chinese Medicine (TCM) practitioner. The intention was to provide free medical services to the poor and needy, but what unfolded was far from the altruistic mission I signed up for.

Initially, the organisation's owner presented a saintly image, committed to helping the underprivileged. However, behind this façade, I faced continuous harassment and numerous red flags:

𝗠𝗲𝗱𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗟𝗶𝗺𝗶𝘁𝘀 𝗮𝗻𝗱 𝗙𝗶𝗻𝗮𝗻𝗰𝗶𝗮𝗹 𝗠𝗶𝘀𝗺𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁

1. Unrealistic Medication Limits: I was pressured to limit prescriptions to three to five days. The reasoning provided was that recipients wouldn’t take all the medicines or would obtain them from other free clinics (This being untrue from my observation). Despite collecting sufficient donations to cover costs, this restriction seemed more about cost-cutting than care.

2. Questionable Financial Claims: The owner frequently claimed the clinics were losing money. This raised significant concerns, especially when they continued to open more free clinics despite alleged financial struggles. A review of the accounts revealed discrepancies, showing a significantly lower loss than claimed.

𝗛𝗮𝗿𝗮𝘀𝘀𝗺𝗲𝗻𝘁 𝗮𝗻𝗱 𝗨𝗻𝗽𝗿𝗼𝗳𝗲𝘀𝘀𝗶𝗼𝗻𝗮𝗹 𝗕𝗲𝗵𝗮𝘃𝗶𝗼𝘂𝗿

1. Intrusive Communication: Throughout a week, I received persistent messages, including late-night and early-morning texts, pressuring me to adhere to unreasonable demands. This included a racially insensitive message about not prescribing TCM medicines to certain patients, despite these medications not being Halal-certified.

2. Inconsistent Rules: I was admonished for prescribing medications for extended periods and for patients not present at the clinic, without any formal rules or regulations provided. Requests for written guidelines were ignored, reinforcing the arbitrary nature of the demands. Strengthening this, the owner called during the midst of consultation with patients, highlighting the highly unprofessional nature of the organisation.

3. Message Deletion: The owner frequently sent messages and then deleted them, fearing I would screenshot the conversations. This behaviour added to the overall lack of transparency and trust.

4. Financial Harassment: The final straw came when I was sent an exaggerated account statement of losses, which I was pressured by circumstances to cover. Despite knowing these losses were not my responsibility, I paid the amount out of frustration, only to realise the deeper manipulation at play.

𝗥𝗲𝗳𝗹𝗲𝗰𝘁𝗶𝗻𝗴 𝗼𝗻 𝘁𝗵𝗲 𝗘𝘅𝗽𝗲𝗿𝗶𝗲𝗻𝗰𝗲

This experience has been traumatic and disillusioning. It highlighted the dark side of some charitable organisations, where volunteers are exploited to maintain a veneer of benevolence while hiding unethical practices. Such experiences can easily deter dedicated individuals from continuing their volunteer efforts.

𝗠𝗼𝘃𝗶𝗻𝗴 𝗙𝗼𝗿𝘄𝗮𝗿𝗱

1. Research and Caution: Before volunteering, conduct thorough research on the organisation. Look for reviews, speak to current or past volunteers, and trust your instincts if something feels off.

2. Transparency and Integrity: Genuine organisations will operate transparently, with clear guidelines and respectful communication. They value their volunteers and focus on their mission without hidden agendas.

3. Support Networks: Engage with networks or groups of volunteers for support and advice. They can provide insights into reputable organisations and share their experiences.
Despite this negative experience, my commitment to helping others remains unwavering. I share this story to caution others and to advocate for true compassion and integrity in all charitable efforts. Let’s work together to ensure that our volunteer work genuinely benefits those in need, without falling prey to manipulation and exploitation.

What is the buzz surrounding Non-sugar sweeteners (NSS) lately? WHO has released a new guideline for the usages of NSS and this post is to summarise what have been outlined. Here's a quick overview based on the latest WHO guidelines and research:

🍬 Short-Term Benefits: NSS can reduce sugar and calorie intake temporarily, which might help with immediate weight management (Blackburn et al., 1997).

📉 Long-Term Considerations: Long-term use of non-sugar sweeteners may be linked to health risks such as type 2 diabetes and heart diseases (Rios-Leyvraz & Montez, 2022; Sylvetsky & Rother, 2018).

🥗 Healthier Alternatives: Instead of relying solely on NSS, consider natural foods with intrinsic sugars like fruits, which provide essential nutrients and help improve overall diet quality (Louie & Tapsell, 2015; Te Morenga et al., 2012).

In my journey as a Traditional Chinese Medicine (TCM) practitioner, particularly amidst the ongoing challenge of COVID-19, I've been constantly reminded of the delicate dance between the ancient wisdom of herbal treatments and the advancements of modern pharmaceuticals. The integration of TCM into the broader spectrum of COVID-19 management has been both a privilege and a complex responsibility, especially when it comes to combining herbal formulas like Xiao Chai Hu Tang with conventional medications such as prednisolone.

From the onset of the pandemic in 2021, I've embraced the role of Xiao Chai Hu Tang, often in combination with Bai Hu Tang or other formulae as proposed by Huang, H. (2020), to address the multifaceted symptoms of COVID-19 in patients preferring a holistic approach. These formulas, with their rich history and balanced properties, have shown promising results in alleviating "heat" symptoms and restoring equilibrium within the body's systems. However, an area of particular concern and professional vigilance has been the potential interaction between these herbal formulas and prednisolone, a corticosteroid frequently prescribed to manage the severe inflammatory responses seen in COVID-19.

My caution stems from a foundational understanding of pharmacokinetics and the influence of herbal constituents on drug metabolism. A study that has significantly shaped my practice is by Homma et al. (1995), which highlights how ingredients like glycyrrhizin in Glycyrrhiza (found in Xiao Chai Hu Tang) can modify the pharmacokinetics of prednisolone, impacting its effectiveness and safety profile. However, according to the same paper, glycyrrhizin couldn’t explain the interaction alone and other bioactive are to be investigated as well.

A defining moment in my practice emerged with a case where a patient, already prescribed prednisolone by her physician, was undergoing a TCM treatment I had tailored for her, featuring a combination of Xiao Chai Hu Tang and Bai Hu Tang. Upon learning about the prednisolone prescription, I ceased the TCM regimen. This experience highlighted the critical need for a cohesive approach and transparent communication between TCM practitioners and conventional medical doctors. It served as a compelling illustration of the necessity to acknowledge and navigate the complexities inherent in merging diverse therapeutic traditions.

This experience propelled me to delve deeper into the mechanisms of such interactions and reinforced the imperative to conduct thorough assessments and foster dialogue with patients about their comprehensive treatment plans. It also highlighted the need for practitioners on both sides to collaborate closely, ensuring that treatments complement rather than counteract each other.

Reflecting on these challenges and lessons, I'm reminded of the responsibility that comes with the privilege of caring for others. It's a journey of continuous learning, adaptation, and collaboration. As we navigate the complexities of integrating TCM with modern pharmaceuticals, our priority remains the safety and well-being of our patients, guiding them through their recovery with both wisdom and caution.
