🏸 Lateral tibial plateau fracture
- A break in the upper surface of the tibia where it meets the femur.

⚠️ Causes:
🤾 Injury causing a force greater than the bone can withstand.
🤾 Usually due to a direct blow.
🤾 Indirect stress caused by twisting or bending.

🧎What are the symptoms of a lateral tibial plateau fracture❓
1. Pain when weight is applied
2. Tenderness and swelling in the leg or calf
3. Limited range of motion
4. Severe cases: numbness or "pins and needles" in the foot due to nerve damage

🩺 Medical Management
🔸Imaging: X-rays, CT, or MRI for accurate assessment
🔸Conservative treatment (if non-displaced fracture):
🔸Immobilization using brace or cast
🔸Non-weight bearing with crutches
🔸Surgical intervention (for displaced or complex fractures):
🔸ORIF (Open Reduction and Internal Fixation)
🔸Plates, screws, or rods to stabilize bones

🏋️ Rehabilitation

Physiotherapy starts as soon as it's safe (post-op or after initial healing). Goals are to reduce pain, restore movement, strengthen muscles, and return to function.

1. Postoperative care if undergoing surgery.
2. Early range of motion exercises as tolerated.
3. Strengthening exercises eg. Isometric quad sets, gluteal squeeze, heel slides, straight leg raise etc.
4. Gait training

📌Other than exercises, physiotherapist will help u with some electrical modalities to enhance blood supply to promote bone and cartilage healing as well as for pain management.

🏃🏻‍♂️Osgood-Scaltter Disease🏀

Osgood-Schaltter disease is a painful swelling on the knee which happens when the patellar tendon is irritated. It often affects children who are still growing, especially in those who are hoing through a growth spurt. A growth spurt can make the child's body more sensitive to stress and strain on the muscles, bones, ligaments and tendons when they use their knees a lot (intense sport training). This can irritate the joint and cause pain and swelling.

🏥Syptoms:
1. Knee pain especially below the knee cap area
2. Swelling on the tibial tubercle
3. Tenderness when touched
4. A hard and painful bump just below the kneecap
5. Pain when squatting, stairclimbing, jumping and running.

🏥Causes:
1. Repetitive strain injury: Intense physical activities can put alot of pressure on the knee and cause overuse injury
2. Muscle imbalance: Hamstring thigtness and quadriceps weakness
3. Age: Children between 11 to 14 are more likely to have this pain as they experience the most groth spurts
4. Playing sports that require alot of running and jumping

👩🏻‍⚕️Treatment:
1. Rest: Avoid the sport activity that caused the irritation in their knee.
2. Icing: Apply ice on painful area 15 mins for few times per day to reduce inflammation
3. Physical therapy: Ultrasound,, TENS and stretching quads muscles to relieve tension on the patellar tendon.

🦶🏻Accessory navicular syndrome⚡️

An accesory navicular is a small extra bone or cartilage located near the inner side of the foot, just above the longitudinal arch, where the navicular bone sits. Some people may have this additional piece of bone congenitally. This can cause pain and inflammation in middle of foot.

👉🏻Causes of Accessory Navicular Syndrome:
1. Overuse or Repetitive Stress: High-impact activities such as running, jumping, or intense sports can cause stress to the accessory navicular, leading to inflammation or pain.
2. Foot Deformities: Abnormal foot structure, such as flat feet can further stresses the accessory navicular.
3. Trauma or Injury: Direct injury to the foot or a sudden impact can cause trauma to the accessory navicular
4. Ankle Sprains: Acute injuries like ankle sprains can trigger inflammation of the accessory navicular
5. Tight Footwear: Shoes that don’t provide sufficient room and adequate support can lead to gait abnormality and worsening symptoms.

👉🏻Symptoms of Accessory Navicular Syndrome:
1. Pain in the Inner Arch: Pain along the inside of the foot, near the arch. This pain may worsen with activity or standing for prolonged periods.
2. Swelling and Tenderness: The area around the accessory navicular may become swollen and tender to the touch, especially after physical activity or excessive movement.
3. Difficulty with Movement: Some individuals may experience stiffness or difficulty moving the foot, especially when walking or engaging in high-impact activities.
4. Pain with Footwear: Tight or ill-fitting shoes may exacerbate the pain, especially if the footwear places pressure on the arch.

👉🏻Risk factor of Accessory Navicular Syndrome:
1. Age: The condition tends to affect adolescents or young adults with higher physical activity.
2. Foot Structure: Individuals with flat feet or overpronation are at higher risk.
3. Sports Participation: People who participate in sports that involve running, dancing, or jumping are more likely to experience irritation of the accessory navicular due to repetitive stress on the foot.
4. Family History: A family history of accessory navicular may increase the likelihood of developing the syndrome.

👉🏻Treatment Options:
1. Rest and Activity Modification: Resting the foot and reducing high-impact activities can help to reduce inflammation. Lower-impact sports such as swimming or cycling are good alternatives while recovering.
2. Ice Therapy: Applying ice to the affected area can help reduce swelling and numb the pain.
3. Orthotics and Footwear Adjustments: Custom orthotics or insoles can provide better arch support and reduce stress on the accessory navicular.
4. Advanced Therapies: Shockwave therapy can help to reduce pain and inflammation and accelerate recovery.
5. Physical Therapy: Targeted exercises can strengthen tendons and the arch, improve endurance and promote healing.

🙆‍♀️What is a Rotator Cuff Repair?

The rotator cuff is a group of four muscles and tendons that stabilize the shoulder and help in lifting and rotating the arm. A rotator cuff tear can result from:

🏷️Repetitive overhead activities
🏷️Trauma (e.g., fall on outstretched arm)
🏷️Degeneration with aging

🙆‍♀️Rotator cuff repair surgery involves reattaching the torn tendon(s) to the head of the humerus, often using arthroscopic or mini-open techniques.

💡⛑️Post-Surgical Physiotherapy Management

Rehabilitation is crucial for optimal recovery and typically follows a structured, phase-based approach over 4–6 months (may vary depending on surgeon’s protocol).

⚠️ Precautions Throughout Rehab:

1. Avoid sudden overhead movement early on
2. Respect pain and swelling — do not push through sharp pain
3. Regular communication with surgeon for protocol adjustments

⏱️ Typical Timeline Summary:

0–6 weeks: Passive motion only

6–12 weeks: Active movement starts

3–4 months: Strengthening begins

5–6 months: Functional/full recovery phase

👩‍⚕️📍Pain management after rotator cuff repair is essential to support healing, reduce discomfort, and enable participation in physiotherapy.

1. Ice Therapy / Cryotherapy

Applied over the shoulder (15–20 minutes, several times daily).
Reduces inflammation and numbs pain.

2. Therapeutic Ultrasound

Promotes tissue healing and improves local blood flow.

3. Interferential Therapy (IFT) / TENS

Electrical stimulation that reduces pain signals and promotes muscle relaxation.
Often used in early rehab to relieve discomfort.

4. Gentle Manual Therapy

Passive joint mobilizations (Grade I–II) to reduce stiffness and pain.
Soft tissue release techniques around shoulder, neck, and scapula.

5. Positioning and Sling Education

Proper arm support in sling to reduce tension on repaired tendon.
Education on resting and sleeping positions to avoid strain.

6. Patient Education

Reassurance and guidance help reduce fear-related pain or guarding.
Teaching pacing and avoiding overuse.

‼️Remember: Your physiotherapy program will be personalized according to your specific need.

⛷🎮Skier's thumb (Gamekeeper thumb)👍🏻👍🏻
This is an injury to the ulnar collateral ligament(UCL) of the metacarpo-phalangeal joint. UCL is a strong ligament that supports the thumb when pinching or gripping and if it is damaged may lead onto to a chronic instability of the thumb which causes problems with function.

The injury happens when falling onto the outstretched thumb and is more likely if the thumb is gripping something at the same time. Falling when skiing while holding a ski pole is a common cause hence the name frequently given to this injury.

👉🏻Injury can be happened in several ways:
- partial tear of ligament
- Complete rupture of ligament
- Bone fracture

👉🏻Syptoms:
Pain, swollen and bruises

👉🏻Risk factors:
- falling or landing on an outstretched thumb or trying to grab something
- sports that require handheld equipment or hand/ball contact
- bicycle falls where thumb catches on the handbars
- A chronic version called 'gamekeepers thumb'where repeated motions gradually stretch and tear the ligament.

👉🏻Treatment:
1. If the ligament is partially torn then a splint or cast is usually worn for six weeks and after its removal a programme of exercises is used to get the thumb moving again. It can take several weeks for the movement and the strength to return.

2. If the ligament is completely torn then an operation to repair the ligament will be performed. This is followed by a similar period of splinting and subsequent exercise programme.

3. For some patients it is recommended to use a splint to support the thumb after this type of injury when undertaking sport or heavy activity. Your treating surgeon or therapist can advise on this

31/05/2025

🦶🏻Shin splints/ medial tibial stress syndrome⚡️
Shin splints refer to pain along the inner edge of the shinbone (tibia). It is an inflammation of the muscles, tendons and bone tissues around the tibia. It is common in runners, dancers and military trainees. It often occur in athletes when they start to train harder as the increased activity puts stress on the muscles, tendons and bone tissues.

👩🏻‍🏫Risk Factors:
1. Shin splints often occur after sudden changes in physical activity, such as:
- Changes in frequency such as increasing the number of days exercises
- Changes in duration such as running longer distances
- Changes in intensity such as running on hills after running on only flat surfaces

2. People in military training.
3. People who have flat feet or high arches.

👩🏻‍⚕️Symptoms:
1. Sharp and dull or throbbing pain
2. Occur both during and after exercises
3. Be aggravated by touching the sore spot

🧑🏻‍⚕️To help prevent shin splints:
1. Make sure you're moving right. A sports medicine professional can watch a video with you of you running to help find areas in which you can improve to lower the risk of shin splints. Often, small changes in how you run and exercises to build strength can lower your risk.
2. Don't do too much. Too much running or other high-impact activity done for too long, too hard can overload the shins.
3. Choose the right shoes. Use a shoe that's comfortable, well cushioned and is suited to your foot. If you're a runner, replace your shoes about every 350 to 500 miles (560 to 800 kilometers).
4. Think about arch supports. Arch supports might help prevent the pain of shin splints, especially if you have flat arches.
5. Think about shock-absorbing insoles. They might reduce shin splint symptoms and prevent them from coming back.
6. Lower the impact. Cross-train with a sport that puts less impact on your shins. Examples are swimming, walking or biking. Start new activities slowly. Increase how long and hard you do them little by little: About a 10% increase in the amount of impact activity per week is a good progression.
7. Add strength training to your workout. Exercises to strengthen your legs, ankles, hips and core can help get your legs ready for high-impact sports.

What is Hip Arthritis❓

📍 Hip arthritis is a condition that causes inflammation and wear of the cartilage in the hip joint. This results in pain, stiffness, and reduced movement.
📍 The most common form is osteoarthritis, but others include rheumatoid arthritis and post-traumatic arthritis.

⏺️Causes of Hip Arthritis

1️⃣ Age-related wear and tear (Osteoarthritis)
2️⃣ Autoimmune diseases (e.g. Rheumatoid Arthritis)
3️⃣ Previous hip injury or fracture
4️⃣ Genetics – family history of arthritis
5️⃣ Obesity – increases joint stress
6️⃣ Overuse - repetitive stress on the joint can wear down the cartilage
7️⃣ Developmental conditions – such as hip dysplasia

⏺️Signs and Symptoms

1️⃣ Pain in the hip, groin, thigh, or buttock
2️⃣ Stiffness, especially in the morning or after rest
3️⃣ Reduced range of motion
4️⃣ Limping or difficulty walking
5️⃣ Clicking or grinding sensation in the joint
6️⃣ Pain worsens with activity and improves with rest

⏺️Physiotherapy Treatment and Management

👍Physiotherapy plays a vital role in managing hip arthritis.
👍The goals are to reduce pain, improve mobility, and strengthen muscles around the hip.

🥇Pain Management

🔥 Heat therapy to relax muscles and improve blood flow

☃️ Cold therapy to reduce inflammation and numb pain

⏩ Ultrasound or TENS (Transcutaneous Electrical Nerve Stimulation) if needed

🥈Exercise Therapy

🤸‍♀️ Stretching exercises: to improve flexibility
🏋️‍♀️ Strengthening exercises: to support the joint (e.g., gluteal and core muscles)
🧘‍♀️ Range of motion exercises: to maintain joint mobility
🚶‍♀️ Low-impact aerobic exercises: such as walking🚶‍♀️, swimming🏊‍♀️, or cycling🚴‍♀️

🥉Manual Therapy

Gentle joint mobilization and soft tissue techniques by a physiotherapist

💡Hip Care Tips

👉Avoid high-impact activities (e.g., running, jumping)
👉Use a walking aid (like a cane) if needed to reduce stress on the joint
👉Wear supportive footwear
👉Use chairs with firm cushions and armrests
👉Avoid sitting for long periods; change position frequently
👉Warm-up before activities and cool down afterward
👉Maintain a healthy weight to reduce pressure on the joint

⏺️When to Seek Medical Attention

1. Contact your doctor or physiotherapist if you experience:
2. Sudden worsening of symptoms
3. Swelling, redness, or warmth in the joint
4. Difficulty performing daily activities despite treatment

📍 Hip arthritis is a manageable condition with the right treatment and lifestyle changes.
📍 Physiotherapy plays a key role in improving quality of life and maintaining function. Stay active🚶‍♀️, follow your exercise plan, and take care of your joints.

Remember: Your physiotherapy program will be personalized according to your specific need.

✴️Distal radio- ulna joint fracture✴️

🔖Is a break in the radius bone at its distal end, near the wrist.

🔖 Causes:
1. Falls on an outstretched hand are the most common cause.
2. Sports injuries or playground falls.
3. Elderly - more susceptible from falls due to lower bone density.

🔖 Types (common):
1. Colles' fracture
2. Smith's fracture

🔖 Diagnosis: 1. X-ray
2. CT scan: complex or multiple fractures
3. MRI: may be use to assess ligaments
damage.

🔖 Treatment:
👉 Non- surgical: Immobilization with a splint or cast for stable, minimally displaced fractures.
👉 Surgical: maybe required for displaced, unstable or complex fracture.

🔖 Recovery:
📍 Most distal radius fractures heal within 4-6 weeks.
📍 Physiotherapy is IMPORTANT to regain full wrist function and strength.

🔖 Complications:
1. Nerve injury: can occur, particularly of the median nerve.
2. Tendon issues: tendinitis or rupture are possible complications.
3. Carpal tunnel syndrome: can develop as a complication.

🚨Remember: Your physiotherapy program will be personalized according to your specific need.
