๐Ÿ“… Recently, we encountered a patient with persistent calf pain and swelling. Initial clinical suspicion pointed towards a soft tissue sarcoma due to the aggressive nature of the swelling. This case reminds us of the critical importance of treating such symptoms seriously and promptly.

๐Ÿ” After a thorough evaluation and biopsy, the final histopathological diagnosis revealed that the patient had chronic fibrinous bursitis, not a soft tissue tumor. Chronic fibrinous bursitis is a condition characterized by the accumulation of fibrin in the bursae, leading to significant swelling and pain.

As surgeons, we sometimes encounter unexpected twists in what seem like straightforward cases. Recently, I was referred a patient with an initial diagnosis of lipoma, a benign fatty tumor. However, the routine excision revealed a surprising and alarming finding: liposarcoma, a rare malignant tumor.

This scenario, often termed "whoops surgery," underscores the vital importance of thorough pre-surgical assessments. What can we learn from this?

1. MRI Scans: These provide detailed imaging, helping us to understand the true nature of the tumor.
2. Biopsy: This crucial step differentiates between benign and malignant growths, guiding the correct treatment path.

๐Ÿ” Spotlight on Two Unique Hand Surgeries: Plexiform Schwannoma vs. Giant Cell Tumour of Tendon Sheath ๐Ÿ”

Today, I had the fascinating experience of treating two patients with similar hand swellings, but with completely different diagnoses and treatment journeys. Let's dive into these intriguing cases!

๐Ÿฆ‹ Case 1: The Mystery of Plexiform Schwannoma ๐Ÿฆ‹
A patient came in with a firm, sensitive lump on their finger. It seemed straightforward at first glance, but the histopathological diagnosis revealed it to be a Plexiform Schwannoma. This rare, benign tumor arises from the nerve sheath and demands precision during removal to ensure complete excision and to minimize the risk of recurrence.

๐Ÿ”ฌ Case 2: Unveiling the Giant Cell Tumour of Tendon Sheath ๐Ÿ”ฌ
Our second patient presented with a similar swelling, but with added discomfort. Post-surgery, the histopathology results pointed to a Giant Cell Tumour of the Tendon Sheath (GCTTS). Unlike the schwannoma, GCTTS can be more aggressive locally and has a higher risk of recurrence if not entirely removed. This condition often affects the fingers, making the surgical approach crucial.

๐ŸŒŸ What We Learned:
While these two cases appeared clinically identical, their distinct histopathological diagnoses underscored the diversity of hand tumors. Each required a personalized surgical strategy to ensure the best outcome. This experience reinforces the importance of precise diagnosis and tailored treatment in orthopaedic oncology.

I had the honor of delivering a talk on the "Overview of Bone and Soft Tissue Sarcomas" at the recent MMA Johor Monthly Joint Clinical Meeting organised by KPJ Bandar Dato' Onn. It was a privilege to share insights and advancements in the diagnosis and management of these complex conditions with esteemed colleagues.

During the session, I also presented a challenging case of a patient with a very large soft tissue sarcoma of the thigh. The tumor was successfully resected, demonstrating the importance of a multi-disciplinary approach and meticulous surgical technique in managing such cases.

Grateful for the opportunity to contribute to the ongoing education and collaboration within our medical community. Together, we continue to advance the field of orthopaedic oncology and improve patient outcomes.
