PATIENT TESTIMONIAL: MR WONG WHO UNDERWENT A SUCCESSFUL HIGH RISK & TECHNICALLY CHALLENGING CAROTID ENDARTERECTOMY (CEA) UNDER DR YOW KUAN HENG, CONSULTANT VASCULAR SURGEON.
Mr Wong had developed episodic blindness in his right eye. This was a classical sign of a mini-stroke/Transient Ischaemic Attack/TIA caused by clot coming from his narrowed right carotid artery in his neck. This is called critical carotid stenosis.
The clot had travelled to his eye arteries causing episodic blindness. It would be a matter of time before carotid clots were pushed under pressure to the brain, blocking off his cerebral arteries to cause a major stroke/death.
The treatment was an operation to remove the unstable, ruptured carotid plaque. This is a carotid endarterectomy (CEA).
Level 1 research evidence indicates that carotid surgery is 1st line treatment over stents.
Mr Wong's CEA was high risk & technically difficult because he had previous radiotherapy for throat cancer:
1. He had a double/tandem stenosis in his right carotid system.
2. The other 3 backup/collateral arteries to his brain (2 vertebral & left carotid arteries) were also critically narrowed from radiation.
3. He had scarring in the operative area due to radiotherapy.
However, Dr Yow with Dr Sham Kumar Sathinand, Consultant NeuroVascular Anaesthetist & the rest of the Complex Vascular Surgery team operated with skill, precision & careful monitoring to achieve an excellent outcome.
Mr Wong went home well in 3 days. Due to his CEA, a major stroke was prevented.
Which patients benefit from carotid surgery(CEA)?
1. Recent TIAs
2. Minor stroke with good spontaneous recovery
3. Major stroke with good recovery from IV thrombolysis
Such patients should have carotid surgery as early as possible after their TIA/stroke to prevent a further big stroke/death, ideally within 2 weeks.
