Near Future: New studies to compare normal Malay, Indian and Chinese school students between the ages of 12-17years of age will be done, in their genotyping, as compared to Keratoconus patients between the same age. It is hoped that some features concerning differences in racial upbringing and habits will also help to define aetiopathogenesis of Keratoconus resulting from comparative studies of this kind. JPD

SHOULD CROSSLINKING BE DONE FOR KERATOCONUS CASES AFTER PENETRATING KERATOPLASTY (PK) ?

CASE 1:(ABIE)EARLY RECURRENCE
Abie , a 19year old Indian boy presented with Acute Hydrops in his right eye in 2008.
His left eye had advanced progressive Keratoconus. Both eyes were treated with Crosslinking, after the Hydrops had been treated and subsided. His left eye could be fitted with a Rigid Gas Permeable Keratoconus lens. His vision was 6/12. Right eye was subjected to PK. It gave him good vision for about one year. He next presented with a recurrence of Keratoconus in September 2011 and early Hydrops again. . He was treated and then croslinked . His vision improved and he is still on followup.

CASE 2:( JOHAN) LATE RECURRENCE
Johan was a middle-aged Malay man with Bilateral Keratoconus. 15 years ago, he had had a PK on his right eye., and since then he was told and assumed that he had an Ambylopia in that eye. His both eye were crosslinked, after it was diagnosed he actually had a recurrence of Keratoconus on his operated eye.
Post CXL, he vision was improved to 6/18 in the right and 6.9p on the left.
These two cases also illustrate the superior quality of vision attained when patients are fitted with Keratoconus RGPs which have special curvatures to fit their highly irregular corneal surfaces.

THE QUESTION NOW IS WHETHER OPHTHALMOLOGISTS SHOULD BE PROACTIVE IN FUTRUE, AND DO CROSSLINKING AFTER PENETRATING KERATOPLASTY

The EYE represents the “Coned” Eye of the Keratoconus Patient which needs to be corrected or treated and visually rehabilitated through medical devices like gas permeable contact lenses, or surgically treated by Cross-Linking, Corneal transplant or other techniques.

The HAND represents the Help that can be given by the Keratoconus Society of Malaysia, through its programs and aid and support from the community and the
public.

The YELLOW RAYS depict how through the Society’s awareness and screening programs many yet undetected Keratoconus patients will be enabled to see
