A New Chapter in Neurosurgery at HSIP
1st July 2025

Today marks another historic milestone for the Neurosurgery Unit at Hospital Sultan Ismail Petra (HSIP). A testament to how far we’ve come and a reflection of our commitment to advancing neurosurgical services in Kelantan.

This morning, we successfully carried out our first suspected case of high-grade glioma tumour debulking using a combination of cutting-edge technology:
✅ Operating Microscope for high-precision visualization
✅ Intraoperative Fluorescence Dye for tumour margin enhancement
✅ Image Guidance System (IGS) for real-time navigational accuracy

This case marks our first brain tumour resection with microscope + fluorescence dye + IGS, and more importantly, it signifies that Neurosurgery HSIP is now stepping into the era of advanced cranial surgery

On the same meaningful day, we also celebrate the official use of Ward 5B as our dedicated Neurosurgery High Dependency Unit (NHDU). This ward will now serve as a specialized post-operative care unit for neurosurgical patients equipped for close monitoring and recovery after major neurosurgical interventions.

Together, these achievements represent a double milestone, not just a clinical success in the operating theatre, but also a systemic improvement in how we care for neurosurgical patients beyond surgery.

We extend our deepest gratitude to the hospital director and all other departments who have supported this journey from day one. This is not just our success but it’s a shared victory for multidisciplinary teamwork and service development.

Today, we successfully performed the first-ever tumor case in HSIP—a historic moment for the Neurosurgery HSIP. This marks another important step forward as we continue to expand our services.

Our aim is to provide the people of Kelantan with timely interventions and early treatment options, closer to home.

We are deeply grateful for the continued support from other departments and hospitals, which has been instrumental in facilitating our progress since the inception of this service.

Perhaps next, we will take on vascular cases in HSIP—who knows what the future holds?

From 6th to 9th February 2025, the Neurosurgery Department of Hospital Sultanah Aminah (HSA) hosted a prestigious conference focused on neurosurgery perioperative care.

Day 1
featured workshops on “High-Speed Drill Techniques” and “Pain Workshop”. Our team participated in the High-Speed Drill workshop, where we learned the fundamentals of handling the instrument and progressed to hands-on sessions, practicing on lamb skulls, spines, and scapulae. We extend our sincere gratitude to the “Medtronic team”for their exceptional guidance and expertise.

Day 2
centered on “Image-Guided Surgery (IGS)”and “Endoscopic Techniques”, presented by “Brainlab”and “B.Braun”. This session delved into advanced technologies currently shaping neurosurgical practices. We had the opportunity to operate IGS machines and endoscopic instruments, gaining firsthand experience of the tools and techniques used by neurosurgeons.

Days 3 and 4
were dedicated to lectures and interactive sessions with sponsors. We attended insightful presentations and visited various exhibition booths to explore the latest innovations and technologies in neurosurgery. These interactions provided a deeper understanding of the advancements driving the field forward.

We would like to express our utmost appreciation to Neurosurgery HSA for organizing this remarkable biennial event. For our team, which is relatively new to the neurosurgery community, this conference was an eye-opening and invaluable experience.

We hope that, in the near future, Neurosurgery HSIP will be equipped with the latest instruments and technologies to enhance the quality of care for the people of Kelantan.

This week, we managed a case of generalized subarachnoid hemorrhage (SAH) with acute hydrocephalus. The patient underwent an emergency right frontal external ventricular drain (EVD) placement and was subsequently transferred to the ICU for intensive care and gradual weaning.

In response to a request from the staff nurse in charge, our neurosurgery team seized the opportunity to conduct a Continuing Medical Education (CME) session focused on EVD care. This session aimed to refresh and enhance the knowledge of healthcare providers involved in the patient’s care.

Today’s CME brought together multidisciplinary teams, including staff nurses and sisters from the General Surgery Ward, ICU, and Operating Theatre, alongside the Head of Department of Anesthesiology at HSIP and several anesthetists. The active participation and collaboration across departments underscored a shared commitment to providing the best possible care for our patients in Kelantan.

This session marks the beginning of a broader initiative to elevate neurosurgical care at HSIP. Moving forward, we plan to organize additional training programs and teaching sessions to continuously improve the expertise and skills of our staff.

Together, we strive for excellence in neurosurgical care in Kelantan.

< CONTINUOUS MEDICAL EDUCATION (CME) – HSIP >

CME sessions at HSIP are typically conducted every Thursday, with each department or unit taking turns to present at the hospital level.

Today, it is our turn to present, giving us the opportunity to raise awareness about hemorrhagic stroke.

The topic of hemorrhagic stroke is particularly relevant for this CME as we recently managed a case earlier this week. Hence, our presentation is titled: “Hemorrhagic Stroke Demystified: From Rupture to Recovery.”

Our objective in this CME is to provide a concise and general overview of the disease, including:
• The nature of hemorrhagic stroke
• Its causes and risk factors
• Recognizable symptoms
• Diagnostic methods
• Immediate responses required
• Treatment and management options

It is important to note that hemorrhagic stroke differs significantly from ischemic stroke and is considered the deadliest form of stroke. By addressing this topic, we hope to raise awareness, particularly among the people of Kelantan.

This concern is heightened due to an alarming trend in the post-COVID era, where the average age of affected individuals has shifted, with more cases observed in younger adults (ages 30–40), alongside the typical prevalence in adults over 40.

Yesterday, we received a referral from Pos Brooke, Gua Musang, a remote area in southern Kelantan located near the border of Cameron Highlands, approximately 200 km from Hospital Sultan Ismail Petra (HSIP).

A young gentleman presented with left-sided hemiparesis, facial asymmetry, and slurred speech. From this history, it was evident that the patient was experiencing a stroke. As we know in neurosurgery, “Time is brain,” and prompt intervention is critical.

The patient was taken to the nearest hospital equipped with CT scan facilities, Hospital Gua Musang, where a diagnosis of spontaneous right temporo-parietal lobar hemorrhage secondary to a hypertensive emergency was made.

Remarkably, despite the significant bleed, the patient remained fully conscious. He was urgently transferred to our center without delay for surgical intervention.

Following his arrival, the patient underwent an emergency right craniotomy and evacuation of the clot. Alhamdulillah, the surgery was successful, and he is now in the ICU, undergoing a gradual recovery process.

A Growing Concern

Hemorrhagic stroke remains a major public health challenge in Malaysia and is one of the leading causes of death within the neurosurgical field. Alarmingly, there has been a rising trend of such cases occurring in individuals as young as their 30s.

At HSIP, we are committed to reducing this burden, particularly in Kelantan, through timely interventions, improved healthcare access, and public awareness.

Yesterday, with the support of our Hospital Director, we successfully organized a meeting involving all Heads of Departments and Units at Hospital Sultan Ismail Petra (HSIP).

The meeting also welcomed our esteemed Neurosurgery colleagues from Hospital Sultanah Nur Zahirah (HSNZ), Kuala Terengganu, who previously assisted in setting up the Neurosurgery service at HSIP.

For those who may not be aware, we initiated Neurosurgery services at HSIP in mid-June 2024. This began with bi-monthly visits from the Neurosurgery team at HSNZ. Although we were unable to provide 24/7 services initially, we managed to perform a few neurosurgical procedures during these visits without the need to transfer patients to HPUSM.

Our first case was a VP Shunt procedure for TB meningitis, performed in June 2024. This milestone was followed by another significant case in October 2024: a bifrontal craniotomy and clot evacuation for bifrontal SDH, marking the conclusion of the HSNZ team’s visiting period.

The primary purpose of the recent meeting was to formally announce that our Neurosurgery service has been fully operational since November 18, 2024. Additionally, we proposed several protocols and guidelines to establish clear directions for the care of Neurosurgery patients.

We are deeply grateful for the unwavering support from all Heads of Departments and Units. This initiative is not for us alone but for the benefit of our patients. Our ultimate goal is to provide the best possible care for the people of Kelantan.
