Ladies and gentleman, esteemed colleagues, partners and cherished guests,

Good evening and welcome to the 10th anniversary celebration of our company. Tonight is a momentous occasion—a time to reflect on our journey, celebrate our achievements, and look forward to an even brighter future.

Over the past decade, we have grown from strength to strength. This company has always been committed to continuous improvement and expansion. We don’t rest on our laurels; instead, we evolve, innovate, and strive to be better each day for the people we serve. Growth is not just a measure of success; it’s a reflection of our shared dedication and passion. Of course , your contributions cannot be overlooked.

Some of you might believe the company’s growth is driven solely by ambition; or your may presume it to be greed. But let me share a deeper perspective: my greatest aspiration is to see every one of you grow alongside it. Let me tell you, when I heard our annual theme was “colors of the world”. I wondered what does that mean? Traditional costume? Instantly, I had an idea, my family would wear outfits representing Malaysia’s 3 major ethnic groups. I initially planned to wear baju melayu, but my tummy is too buldging and the kain pelikat make it even more buldging (....challenging). So, I opted for Tang suit, Dr. Tan choose a Baju Kurung and My daughter dazzled in a lehenga – Symbolising Malaysia’s vibrant diversity.

It’s a great experience to meet with a world-class endoscopic spine surgeon. I would like to thank the World MISS 2024 organizing committee for giving me this great opportunity to share a topic: “Why should spine surgeons embrace the transformative potential of endoscopic spine surgery”.

This endoscopic spine surgery is a game-changing evolution of spine surgery. With the world's increasing aging population and young people with electronic gadgets using habits (new disease called tech neck), spinal problems are not only affecting the elderly. Many times, a degenerative spinal problem is not a disease per se but giving only symptoms, therefore removing the symptoms will solve the problem and doing more may create a disease. For young patients with spinal issues, fusion is never the best option if stability is not a problem. Fusion at a young age will give rise to adjacent level disease. Therefore, for both elderly and younger people, endoscopic spinal surgery is the best option if there is no instability issue for fusion. Having said that fusion is still indicated in certain spinal conditions. Therefore, discuss with a doctor and not a computer for the best option for you.

Just finished the IESU2024, rushing to Germany for 13th international symposium on nTMS for neurosurgery and Neuromodulation. This symposium is about the technology using navigation (more precise) and magnetic stimulation for mapping prior to brain surgery and also Neuromodulation to treat difficult neuropathic pain condition (such as central stroke pain, phantom limb pain, complex regional pain syndrome and other neuropathic pain conditions) and major depressive disorder.

As a doctor, our responsibility is not only to teach but also to learn, so patient will benefit the most.

It is a great honour and privilege to welcome you all to this significant event for the Malaysian Society of Endoscopic Spine Surgery.

In my personal opinion, both specialization and subspecialization are essential for enhancing the quality of healthcare. However, paradoxically, this trend can lead to the fragmentation of care. I believe there is a significant issue when a single area of specialty is further divided into multiple factions, as this can impede overall progress in the field. Therefore, our society was established to foster collaboration between the neurosurgery and orthopedic spine communities, emphasizing the importance of working together to advance patient care. I believe that in healthcare, different specialties should not be in competition, but should work together to complement each other. This collaboration ensures holistic care for our patients and fuels the remarkable progress we are seeing in our field.

I am proud to share that last year, we successfully signed Memorandums of Understanding (MOUs) with our colleagues from Korea, Taiwan, and Thailand. This year, we are further expanding our international collaboration by forming new MOUs with Indonesia, Singapore, and Vietnam. These partnerships represent a major step forward in advancing endoscopic spine surgery, as we work together to enhance the quality of care and drive innovation in the field across borders.

Endoscopic spine surgery is a revolutionary advancement in our field, providing a minimally invasive alternative that addresses many of the challenges posed by conventional open spine surgery. Traditional open surgery, while effective, often requires large incisions, which can lead to significant tissue damage, longer recovery periods, and higher risks of complications such as infection and blood loss. This, in turn, impacts patients’ quality of life and delays their return to normal activities.

In contrast, endoscopic spine surgery offers a solution that not only minimizes these risks but also delivers faster recovery, reduced postoperative pain, and shorter hospital stays. By using smaller incisions and precise techniques, we are able to achieve excellent outcomes in many spinal conditions, including herniated discs, spinal stenosis, and degenerative disorders. The minimally invasive nature of this technique has clear advantages, and the growing evidence of its effectiveness shows that it has the potential to redefine spine surgery for a large subset of patients.However, for this technique to truly reach its full potential, it is essential that we actively work to accelerate its development and expand its applications. While endoscopic surgery has proven highly successful in many scenarios, there are still complex pathologies where traditional open surgery remains the best option—for now. This does not mean that endoscopic surgery is limited; rather, it shows the importance of advancing our skills, refining the technology, and pushing the boundaries of what can be achieved with this technique.

In order for endoscopic spine surgery to reach its full potential, we must actively work to accelerate its development and broaden its applications. While open surgery remains essential for some complex cases, this should inspire us to further refine endoscopic techniques and push their boundaries. By investing in research, training, and innovation, we can adapt endoscopic approaches to handle more challenging cases and increase their use in a wider range of pathologies. This progress will benefit both patients and surgeons, ensuring that minimally invasive solutions become a central part of spine care moving forward.

When I founded this society, my vision was to popularize this technique so that more patients could benefit from it. At the same time, it is essential that we protect the interests of the surgeons who dedicate their skills and expertise to this evolving field. Together, we can ensure that endoscopic spine surgery continues to grow and that both patients and surgeons thrive within this exciting evolution.

Thank you all for your continued support and dedication to this mission. Let's keep working hand-in-hand to push the boundaries of what is possible in endoscopic spine surgery.

I would like to thank the organiser of Neurosurgery Association Malaysia and Sarawak Neurosurgery Update 2024 for giving me this great opportunity to deliver a talk "Hyperactive Cranial Nerve Dysfunction Syndrome (HCNDS)".
HCNDS include few conditions including Trigeminal neuralgia, Hemifacial Spasm, Glossopharyngeal Neuralgia, Geniculate Neuralgia, Paroxysmal vertigo+/-tinnitus

The above conditions are mainly diagnosed clinically, therefore the medical history given by patients are extremely important. I realize, nowadays, many patients believe with advancement technology and A.I., all disease can be diagnosed by machine, personally it's not applicable to all diseases, especially the above mentioned disease. I always spend a lot of time listening to patient because i strongly believe clinical skill still holds good even with whatever advancement of technology or A.I. Having said that, I still agree that technology and A.I. still can assist us in managing patient in many occasions.

All the above conditions is possible cured with accurate/precise diagnosis.

I would like to expressed my heartfelt gratitude to Prof Pornpavit Sriphirom and Dr. Chaiyaporn for giving me this opportunity to present a topic "Cervicogenic headache resolved after bilateral C3/4 posterior endoscopic Cervical foraminotomy (PECF)" in the 9th ASEAN Minimally Invasive Spine Surgical Technique (ASEAN MISST) congress.

Here, I have met many world renown endoscopic spine surgeons and exchanged many ideas.

Headache can be because of neck pathologies; pain generators can be from upper cervical nerve root compression, myofascial pain, upper cervical facet joint pain syndrome, cervical discogenic pain and Chiari malformation. Diagnosis can be difficult. However, detail history (the story by patient) is the most crucial because there is no one gadget to identify the site of pain and imaging can not confirm the pain generator in most of the cases. Effective treatment can be delivered only after precise identification of pain generator/generators.
