The sciatic nerve is the longest nerve in the body.

It forms from several nerve roots in the lower spine โ€” typically the L4 and L5 lumbar nerves and the S1 and S2 sacral nerves. These nerve roots join together to create the sciatic nerve that travels through the buttock and down the back of the leg.

Because the nerve begins in the lower spine, irritation affecting these nerve roots can create symptoms that travel into the leg.

For this reason, symptoms often attributed only to the buttock or the piriformis muscle may actually relate to changes affecting the nerve roots in the lumbar spine.

Understanding where the sciatic nerve originates helps explain why symptoms can begin in the lower back and extend down the leg.

When low back pain begins to travel into the buttock, thigh, calf, foot, or toes, the cause is often not in the leg โ€” it starts in the spine. Sciatica develops when a nerve in the lower back is compressed, most commonly by a spinal disc problem.

In the majority of cases, leg pain, numbness, tingling, or weakness that begins in the back is linked to a disc bulge, protrusion, prolapse, herniation, or rupture. These are commonly called โ€œslipped discs,โ€ but each type behaves differently โ€” and that difference determines management.

Even a small bulge can crowd a nerve. When that space is reduced, symptoms travel down the leg. That is why proper evaluation is essential. If leg symptoms are present, a disc issue should be ruled in or ruled out early โ€” not assumed.

X-rays help assess alignment and conditions such as spondylolisthesis (slippage of a spinal bone). When symptoms extend into the leg, MRI is often more informative because it shows the disc and nerve directly. In some cases, both are appropriate.

What does not help when a disc is involved:
โ€ข Twisting the lower back
โ€ข Rotational cracking
โ€ข Deep forward bending
โ€ข Forcing stretches into leg symptoms

These may feel relieving briefly but can increase disc stress.

What may help when guided appropriately:
โ€ข Identifying the exact spinal level involved
โ€ข Determining severity
โ€ข Reducing mechanical pressure on the nerve
โ€ข Controlled, non-rotatory spinal care
โ€ข Spine-specific rehabilitation
โ€ข Spinal decompression when indicated
