No me**es, very young 29 years old, AMH 156pmol/L, abnormally low estrogen, FSH and LH levels = the diagnosis is hypogonadotrophic hypogonadism.

1st IVF, 1st frozen embryo transfer, 1st mosaic embryo, 1st pregnancy.

Her IVF cycle had to be tweaked to suit her diagnosis, was not the easiet to manage due to her hormonal anomalies and risk of OHSS. We eventually had 12 eggs, 12 ICSI, 11 fertilized, 7 blastocysts for PGTA, 5 usable embryos, all are mosaic.

It's extremely satisfying seeing how happy this young couple is, with their 1st pregnancy. Hope everything goes smoothly for them. Due date is in December 2026.

We could not do an embryo transfer for this patient due to her thin endometrial lining, endometrial adhesions and fluid in her endometrial cavity.

We had to do a hysterosalpingogram to assess her fallopian tubes and look for the source of the fluid in her uterus. This came back normal.
She also needed a hysteroscopy to release some intrauterine adhesions.

However, her uterine lining was still thin after her adhesiolysis, and despite high dose estrogens.

So, we performed 2 rounds of endometrial platelet-rich-plasma (PRP) infusion. Her lining grew from 4.7mm to 5.7mm. We performed her frozen embryo transfer on this thickness and she conceived successfully. Her due date is in September 2026.

PRP infusion enhances the uterine lining by releasing concentrated growth factors that stimulate cellular regeneration, increases blood flow and reduces inflammation.

It can be used in patients with thin endometrial lining and recurrent implantation failure, with some success.

11 IVF cycles. The last 3 IVF cycles were with me at Sunfert@Bangsar South.

In total, they failed 7 day-3 embryos and 5 blastocysts from 9 IVF cycles, all non PGT tested embryos.

Her last 2 IVF cycles were PGTA cycles, we made 1 mosaic 3BC male embryo and 1 euploid 5CC female embryo.
Mum and Dad brought their son for a visit, so obviously, the mosaic male embryo implanted 😃. The euploid embryo didn't make it.

They've been married since 2016.
They started their IVF journey in 2020, when Mum was 37.
They saw me at Sunfert@Bangsar South when Mum was 40. We did 3 IVF cycles.
We transferred Ethan when Mum was 42.

What we did differently:
▪︎ Specialised fertilization media used in the lab
▪︎ Used microfluidics for the s***m selection
▪︎ PGTA
▪︎ Endometrial receptivity window analysis
▪︎ Regulated her menstrual cycle
▪︎ Added immune modifiers

Day trips from Johor to Sunfert, KL and back to Johor again, to get this adorable drool machine 🤣. A 5AA mosaic embryo has turned into this gorgeous baby boy.

She had failed a euploid and another mosaic embryo before coming to see me at Sunfert@Bangsar South. She was 40 then.

We checked her endometrial receptivity window, gave her mild immune modifiers, regulated her menstrual cycle and transferred a mosaic embryo.

This 33 year old got pregnant on her very 1st try with tablets and timed sexual in*******se.
She's 8 weeks pregnant now, baby is due in November 2026. So, yes, I don't have embryo pics for this post 😄, since this wasn't an IVF cycle.

Married 2011. Total 8 IVF cycles. Pregnant after 14 years of marriage.

Problems:
1. Recurrent implantation failure
2. Diminished ovarian reserve, AMH 2 pmol/L
3. Clotting disorder
4. Raised immune antibodies
5. Adenomyosis
5. Low vitamin D
6. Low iron levels
7. Mild male factors
We found solutions for whichever issues we could.

Cycles 1 - 5 (2015 - 2023; age 28 - 36) were done in their previous IVF centres: couple had 8 day-3 embryos which failed to implant.

Cycles 6 - 8 (2023- 2024, age 36 - 37) were with me at Sunfert@Bangsar South, we made 2 blastocysts, non-PGTA tested.

1st frozen embryo transfer (FET) at Sunfert (2024), we used a 3CB blastocyst. Mum was 37 years old then. Mum said this was her 1st ever positive pregnancy test after 14 years of marriage!

Not giving up and being relatively young were key factors in their success.

Cute baby Adam, Mum, Dad, Atuk & Nenek came for a visit yesterday, 31.3.26. Was a very happy day for me.

Pregnant at 43. But, the catch is that her embryo was actually made when she was 41.

She started 8 IVF PGTA cycles but completed only 6 cycles. She was hoping to accumulate embryos to be able to complete her family.

What you can see is that, it becomes more difficult for an older woman to make normal embryos, as time goes by. Even a difference of a few short months can change outcomes tremendously.

This is why it's futile for women past their mid-40s to do IVF as success rates are terribly low.

Her history:
1st IVF, age 41 - 3 blastocysts - 1 mosaic embryo, 2 aneuploid embryos
2nd IVF, age 42 - 1 blastocyst - aneuploid
3rd IVF, age 42 - 2 blastocysts - both aneuploid
4th IVF - abandoned
5th IVF - abandoned
6th IVF, age 42 - no blastocyst
7th IVF, age 42 - 1 blastocyst - aneuploid
8th IVF, age 42 - 2 blastocysts - both aneuploid

We transferred her one and only blastocyst, a 5AA mosaic embryo, created in her very 1st IVF cycle. All her subsequent IVF cycles did not yield any usable embryos. Thankfully, she's currently 10 weeks pregnant. Due date is in October 2026.

Starting early usually translates into needing fewer IVF cycles, higher pregnancy rates and a bigger family size.

Starting fertility treatment early, more or less helps you have a larger embryo pool, better quality embryos frozen, higher success rates with each embryo transfer and an overall larger family size.

This Mum is currently 32 years old. She did an IVF cycle at age 28 due to tubal issues. She had 6 blastocysts frozen.
# Her 1st frozen embryo transfer (FET) was in 2022, their daughter Elsa was born by year's end.
# She just did her 2nd FET in February 2026 and is currently pregnant at 6 weeks now. Due date is in November 2026.
We still have 4 more embryos frozen.

Moral of the story, don't wait, start ASAP.

Don't fret & despair if all you have is a single lower quality embryo, which also happens to be a mosaic embryo. These embryos can still turn into healthy babies.

Mum is 38 years old, married since 2017. The couple did an IVF cycle at Sunfert@Bangsar South in November 2025 and collected 10 eggs, 8 ICSI, 5 fertilized and 4 turned into blastocysts. PGTA showed that a 5CA mosaic embryo was their only option for transfer.

Frozen embryo transfer was done in February 2026 and the couple is 8 weeks pregnant now. Baby is expected in October 2026.

Vaginismus can make starting a family difficult. We can try vaginal insemination (VI) with husband’s semen, IUI or IVF.

The difference from other couples, was that her embryo transfer was done under sedation/ mild anaesthesia. Everything else is the same as in other couples undergoing IVF. Her 1st frozen embryo transfer (FET) was successful and they're now 12 weeks pregnant. Expecting baby in September 2026.

Endometriosis and adenomyosis are bad news during fertility treatment. But, it's still possible to have a successful pregnancy. We just need to adjust your egg collection and embryo transfer protocols to suit a woman with chronic inflammation and altered endometrial receptivity. Rest assured, what's more important in this equation is still, your age!

Here's a 38 year old, married since 2019 with history of previous ovarian endometrioma surgery in 2023. Unfortunately, her ovarian cysts are present again, she's developed adenomyosis of her uterus, and her ca125 marker is high.

We workerd through all that (no surgeries involved) and she's currently 12 weeks pregnant. Due date in September 2026.
