1) Assist in choosing the best treatment option and appropriate care provider and setting
2) Assist to doctors' appointments and coordinate care among all providers
3) Advocate before, during and after hospitalizations, doctors' visits, ER visits, etc.
4) Reduce unnecessary hospitalizations and ER visits
5) Provide family/caregiver education and support
6) Develop Custom Care Plan to be implemented by ECN professionals and/or the family caregivers
7) Manage Medications
8) Manage Chronic Diseases such as Alzheimer/dementia, CHF, COPD, Diabetes, Cancer, arthritis, among others
9) Translation of medical terms/reports to local languages
