This young gentleman presented with recent heart attack. Coronary angiogram showed severe bifurcation stenosis (>95% stenosis) at the junction between proximal & mid LAD involving ostial D1 (Medina 1,1,1). This severe bifurcation stenosis was carefully prepared & successfully treated with DCB. It was not an easy job! But again, no stent implanted!

Another Chronic Total Occlusion, CTO (100% stenosis) at mLAD was successfully treated with Drug Coated Balloon. No stent implanted. The patient is recovering well.

Aspiration thrombectomy is indicated during percutaneous coronary intervention in Acute Coronary Syndrome patients with high thrombus burden.

A young patient presented with anterior myocardial infarction. Urgent coronary angiogram showed 100% stenosis at the proximal segment in one of his major coronary arteries - Left Anterior Descending artery, LAD.

Aspiration thrombectomy was performed & showed mixture of fresh & old thrombus (clot). It was then followed by careful lesion preparation. Finally, DCB was deployed with no stent needed.

Sometimes, I may need certain tools (e.g. rotablation, orbital atherectomy, intravascular lithotripsy or IVL etc) for lesion preparation during coronary angioplasty especially in a severely calcified stenosis.

Today, I performed the first intravascular lithotripsy (IVL) at Timberland Medical Centre. It was a severe & heavily calcified stenosis at a dominant Right Coronary Artery (RCA) & was only decided after routine balloon angioplasty failed to achieve optimal result. It was then followed by a DES implantation.

CASE OF THE WEEK

39 year-old young man. Sudden onset of chest pain & collapsed after dinner. Urgent coronary angiogram showed 100% blockage at the Left Main Stem (Very severe in term of location & percentage of the blockage!). Immediate angioplasty was performed with aspiration thrombectomy, followed by drug-eluting stent & drug-coated balloon. Thanks to the excellent team work! The patient recovered well following the catastrophic myocardial infarction (heart attack).
Myocardial infarction is no longer a disease of elderly!

39岁，男性。晚餐后突发胸闷。抵达医院急诊室后已经呈现半昏迷状态。由于心源性休克和肺积水，病人必须依赖紧急气管插管和呼吸机辅助呼吸。冠状动脉血管造影检查发现冠状动脉左主干100%阻塞（阻塞部位和程度都非常严重！）。若不及时将血管开通，病人随时有生命危险。我快速得利用血栓抽吸方法将血栓（血块）从血管里抽吸出来，然后再应用药物球囊和药物支架将100%阻塞的血管开通。感谢一群很棒的医疗团队，我们硬是把病人从鬼门关拉了回来！
心肌埂塞已经不再是老年人的疾病了！
