What does surgeon fear the most?

A bad case of severe gallbladder infection especially in patients who have other medical problems. Sepsis can happen and surgery can cause bleeding. Pretty much a nightmare scenario. Emergency operation for acute bad gallbladder surgery in older patients carries higher risks than doing an elective gastric bypass !

What is NOSES?

NATURAL OR***CE SPECIMEN EXTRACTION SURGERY (NOSES):

one step further during laparoscopic or robotic colo-rectal surgery to reduce postoperative pain and scar.
Retrieval of the "resected part of colon" after all the difficult surgery does require 6cm cut at the abdomen.
Some small tumor and non-cancerous disease may benefit from NOSES that prevent a "cut in the abdomen". Without this 6cm-cut, patients will definitely feel less pain, reduce risk of infection and hernia at this 6cm cut.

GLP-1 ( Ozempic, Wegovy, Mounjaro...)

1. Yes. It works. It reduces weight in majority of patients (10 to 20% total body weight loss)
2. It caused nausea in most patient especially in the beginning of treatment. Nausea slowly goes off.
3. You have no appetite because it slows down gastric emptying. Foods stay in your stomach longer.
4. Most side effects are gastrointestinal related because it is a gut hormone.
5. Yes, it is expensive over long term. And most likely you have to be on it long term.
6. You have to tell your surgeon you are on it before any surgery. Because you need to skip one dose before surgery.
7. Stopping the drug will cause rebound of weight. Drug means long term usage.
8. Younger patients probably better off doing the surgery than taking medication.
9. Surgery definitely "cheaper" in the long run. And surgery provides 30 to 35% total body weight loss.
10. One year of the drug probably can "buy" the cost of the surgery, at least in Malaysia.

IFnot good enough?

Recent Cochrane review showed Intermittent fasting is not better than other diet program to lose weight. Which i agree from personal experience. I regain weight easily when I stop IF while on vacation. But I always go back to it whenever back to work because it is just easier for me to lose those gained weight. But I truly believe in fasting. Fasting rests our intestines. There is no reason for our bowels to keep working every few hours or hourly. IF has other good effects other than weight loss, examples, it increase insulin sensitivity, it helps get rid of dead cells ( autophagy), it reduces inflammation, it also promotes brain and heart health. We should not focus on weight loss only.

Minimally invasive surgery is as good as open surgery in term of cancer clearance. It gives same survival benefits. And it has added advantages such as less pain post operation, earlier return to work, and less bleeding during the surgery. This is true to most intra-abdominal cancer.

Stay curious to our body!

It is important to know what has changed and what is new to our body. We the owner should be more alert to whatever changes to our mind and body, and seek help or reassurance from friend, family, or anyone we trust. No one know more than us about ourself.

Undescended te**is increases cancer transformation on the te**is itself because intra-abdominal temperature is higher than our sc***um. Surgeon will normally advise to fix it by bringing it down back into the proper place inside the sc***um.

When should I get scoped?

I treated 3 young colon cancer patients within 1 month. It blew my mind how many pre-cancerous polyp we miss if we follow the recommended colonscopy guideline. My threshold to do colonoscopy is much lower now for any new symptom.
