“Ever heard of a thyroglossal duct cyst?

This patient presented with a swelling in front of the neck which was increasing in size.

🥇It’s an uncommon congenital anomaly where a fluid-filled sac forms in the neck, often near the hyoid bone.

🥈While usually painless, they can grow over time, causing discomfort or difficulty swallowing.

🥉Treatment typically involves surgical removal including part of the hyoid bone to prevent recurrence.

The UPM ORL team had the opportunity yesterday to be introduced to 3D endoscopic ear surgery and 3D Vitom Exoscope ear and mastoid surgery by the esteemed Professor Prepageran of University Malaya ORL-HNS. It was indeed an exhilarating experience and has broadened our horizons as to the applications of such technology in Ear Nose Throat surgery, giving Depth of field perception in endoscopic work as well as providing an alternative to the use of microscope. 3 cases were done in UPM Teaching Hospital yesterday under the guidance of Prof Prepageran - Cochlear implant in unilateral profound hearing loss in adult, 3D endoscopic ear surgery for attic cholesteatoma, Combined 3D endoscopic ear and 3D Vitom Exoscope surgery (Storz) for middle ear and mastoid exploration for suspected cholesterol granuloma and bilateral Eustachian tube dilatation (Balloon from SYS company)

A lovely picture of suppurative (pus forming) otitis media (infection of middle ear) which was caused by mycobacterium tuberculosis, commonly known as TB. TB is most commonly known to infect the lungs, causing long term cough and blood stained phlegm but it can also infect any parts of the body causing problems at the region as well. For the ENT surgeon, one of the area which we commonly encounter TB would be in the lymph nodes, causing a chronically enlarging lymph node which may or may not be painful, with pus formation within, commonly presenting as a cold abscess. Other area of the upper airway such as larynx, adenoids, tonsils and even the middle ear can also be infected by TB, albeit much less commonly seen. The as the bacteria is inhaled and may lodge at those areas. There’s a long way to go before we can even hope to eliminate TB.

A reddish lesion behind the ear drum suggestive of a vascular mass - glomus tympanicum causing the patient to experience pulsatile tinnitus which affects her daily living. A minor surgery was done to resolve her symptoms. 耳膜后长了一个红色的血管肿瘤导致病人一直被耳鸣所困扰,做了小手术之后就把问题彻底解决了。

My ORL Journey

This is a journey which I've embarked on since 2009. After obtaining my undergraduate degree in medicine from USM (Universiti Sains Malaysia) and completing my internship training, I began working in the ear nose and throat (Otorhinolaryngology ORL) department of Hospital Kuala Lumpur serving as a Medical officer. In due course, I've obtained my MRCSed (ENT) qualification from the Royal College of Surgeons (Edinburgh) and completed my specialist training with a Masters Degree in Otorhinolaryngology, Head and Neck Surgery from University of Malaya. I have also served as an ORL specialist under the Ministry of Health in various hospitals in Johor prior to joining University Putra Malaysia. I believe pursuing the ‘Perfect Surgery’ and passing on knowledge via research and medical education is the essence of being a Surgeon/Doctor, and that is where my heart lies. I hope to make this a platform where I can share pieces of this journey and my insights into the field of Otorhinolaryngology and Head and Neck Surgery.
