Boarding vs Non-Boarding Medicine Chest Inspection

Dalam industri maritime hari ini, kadang-kadang kita mendengar tentang dua pendekatan untuk pemeriksaan medicine chest:

Boarding inspection dan non-boarding inspection.

Non-boarding inspection biasanya ditawarkan kerana kosnya lebih rendah dan prosesnya lebih cepat. Ada yang hanya melibatkan semakan inventory list, gambar ubat, atau dokumen yang dihantar dari kapal.

Namun, satu perkara penting yang perlu difahami oleh ship operators ialah:

Marine Department sebenarnya tidak membenarkan inspection dijalankan secara non-boarding untuk tujuan certification.

Ini kerana medicine chest compliance bukan sekadar memastikan senarai ubat mencukupi.

Pemeriksaan yang sebenar juga melibatkan perkara seperti:

• cara ubat disimpan
• label ubat yang jelas dan betul
• susunan ubat untuk kegunaan semasa kecemasan
• keadaan sebenar medical equipment
• pemantauan tarikh luput ubat

Semua aspek ini hanya boleh dinilai dengan pemeriksaan fizikal di atas kapal.

Atas sebab itu, boarding inspection masih merupakan kaedah yang sah dan lebih menyeluruh untuk memastikan tahap compliance sebenar.

Apabila inspection dilakukan secara on board, inspector dapat melihat keadaan sebenar medicine chest dan memastikan semua ubat serta peralatan disimpan dengan selamat, teratur dan mudah digunakan ketika kecemasan.

Selain itu, boarding inspection juga memberi nilai tambahan melalui interaksi secara terus dengan crew kapal.

Inspector boleh:

• memberi penerangan ringkas tentang penggunaan ubat tertentu
• berkongsi basic medical handling semasa kecemasan
• membuat sedikit assessment terhadap kefahaman crew

Interaksi ini membantu meningkatkan pengetahuan dan keyakinan crew untuk bertindak apabila berlaku kecemasan di laut.

Beyond Compliance: When Medical Readiness Matters

Two weeks ago, our Medicine Chest Inspector conducted a comprehensive medicine chest inspection, onboard assessment, and crew training on board a vessel.
As with every assignment, the focus was not merely regulatory compliance, but operational readiness.

One week after the inspection, a medical emergency occurred onboard.
A crew member experienced syncope followed by acute shortness of breath while on duty.

Immediate first response was carried out effectively due to several critical factors:
• The oxygen set was in good condition and recently serviced
• All medical equipment was complete and fully functional
• Crew members were confident in the indication, setup, and proper use of oxygen therapy — not just aware of its presence

The crew member’s condition was successfully stabilised prior to further medical guidance.

Incidents like this highlight a key reality of maritime operations:
medical emergencies at sea rarely offer second chances.

In many vessels, medicine chests appear compliant on paper,
yet crew members may lack the confidence to act decisively under pressure.

This is where standard inspection differs from inspection combined with scenario-based onboard training.

The true objective is not simply to pass an audit,
but to ensure that when an incident occurs:
• Crew members know exactly what to do
• Equipment performs as expected
• Early decisions do not worsen the situation

Shipowners often ask about the return on investment for onboard medical training.
The answer is straightforward:
a well-managed medical emergency can prevent escalation, off-hire, deviation, and potential loss of life.

A properly maintained medicine chest,
combined with trained, calm, and competent crew,
forms an essential part of an effective risk management strategy — not an optional extra.

Field experience consistently proves one thing:
preparedness is not theoretical. It is tested on the worst day, not the best.

Kes letupan di kapal ICON AMARA pada Ogos 2024 mengingatkan kita tentang satu realiti pahit dalam dunia maritim — kecederaan serius boleh berlaku bila-bila masa, tanpa amaran.

Dalam kejadian tersebut, dua kru maut, dan seorang lagi mengalami kecederaan parah pada muka dan tangan. Pada saat begitu genting, tindakan awal bukan sekadar penting, tetapi boleh menentukan peluang hidup.

Di tengah laut, bantuan perubatan bukan “10 minit dari hospital.” Bila berlaku letupan, kebakaran kecil, atau kecederaan akibat suhu dan tekanan tinggi di bilik enjin, kru hanya bergantung kepada apa yang tersedia di atas kapal — dan di sinilah peranan medicine chest menjadi sangat kritikal.

Sebuah medicine chest yang lengkap dan diselenggara baik membolehkan kru:

– Mengawal pendarahan dan melindungi luka melecur

– Menstabilkan mangsa sebelum proses pemindahan

Tetapi semua ini hanya boleh berlaku jika medicine chest berada dalam keadaan terbaik — lengkap, teratur, dan mengikut tarikh luput.

Kes ini menjadi peringatan kepada semua bahawa penyelenggaraan medicine chest bukan sekadar rutin pemeriksaan.

Ia adalah sebahagian daripada sistem keselamatan kapal. Ia adalah persediaan menghadapi situasi tidak dijangka. Dan yang paling penting, ia adalah bentuk tanggungjawab terhadap nyawa setiap kru di atas kapal.

Accidents will always be unpredictable. Preparation shouldn’t be.

The Importance of Maintaining a Proper Medicine Chest Onboard

At sea, time becomes the quietest enemy. When an injury happens, when someone suddenly feels dizzy, or when early signs of infection appear, there is rarely an option to “check tomorrow” or “drop by a clinic.” That is why the medicine chest is not just a box of supplies — it is the crew’s first line of defence.

Life onboard is different from life ashore. Long voyages, unpredictable weather, heavy workloads, humidity, fatigue, and confined spaces all increase the risk of injuries and illnesses that can escalate quickly if not treated immediately. Without a well-maintained medicine chest, the crew is exposed to preventable dangers.

A properly maintained medicine chest should meet several essential requirements:
– Medications that are in-date and stored correctly
– Complete emergency treatment equipment
– Clear and accurate labelling
– Updated inventory records
– Easy and immediate access during emergencies
