甲状腺癌

Thyroid Cancer: (Signs & Symptoms)

Often presents as a thyroid nodule and may be asymptomatic early → red flags suggest malignancy or local invasion

🔹 Common Presentations
→ Painless thyroid nodule (solitary mass)
→ Anterior neck swelling / visible thyroid enlargement
→ Neck discomfort/pressure (fullness/tightness)

🔹 Concerning Features (“Red Flags”)
→ Rapidly enlarging neck mass (more concerning for aggressive tumors, e.g., anaplastic)
→ Fixed hard nodule (irregular, reduced mobility)
→ Cervical lymphadenopathy (central/lateral nodes)
→ Hoarseness (recurrent laryngeal nerve involvement)
→ Dysphagia (esophageal compression)
→ Dyspnea/stridor (tracheal compression; advanced local disease)

🔹 Key Point
→ Many thyroid cancers are found incidentally on imaging or routine exam

🔹 Evaluation (High-yield)
→ TSH first
→ Neck ultrasound (nodule + lymph nodes)
→ FNA biopsy for suspicious nodules/abnormal nodes
→ CT/MRI if invasive features; laryngoscopy if hoarseness

⭐ Exam Tip
→ Hoarseness + fixed nodule + cervical nodes = high suspicion for malignancy; stridor/dyspnea suggests airway compromise → urgent assessment.
