Sometimes, the smallest details in surgery make the biggest difference.

When I was working with my colleague Jeroen Stevens, we explored something often overlooked — **electrostatic charge on breast implants**.

These implants naturally carry static electricity, which can attract microscopic airborne particles the moment they are opened… even in a sterile operating room. ([OUP Academic][1])

Over time, this may contribute to **biofilm formation, infection, and capsular contracture** — complications every surgeon strives to prevent. ([OUP Academic][1])

So we asked a simple question:
👉 *Can we reduce this invisible contamination?*

Our work led to a simple intraoperative technique to minimise this risk.

It’s not about doing more.
It’s about doing things **more thoughtfully**.

Because in surgery — precision is not just what you see, but also what you don’t.

Lately for some reason, I have been seeing and managing hand surgery cases which is a subspecialty of Plastic Surgery. Today I met this lovely lady who presented with a birthmark on the back of her hand. She is getting married soon and wants the appearance of her dorsum of her hand improved.

Port wine stain (PWS) on the hand is a flat, pink to purple birthmark from dilated capillaries, common and usually harmless, growing with the person but potentially darkening, thickening, and developing bumps (vascular nodules) over time, requiring early laser treatment to prevent skin texture changes and improve appearance.

In the practice of medicine, one can get blindsided.

You may notice a huge fracture on the X-ray getting so focused on that fracture and inadvertently miss the second fracture.
What is the most common fracture missed in the Emergency Department? Answer -The 2nd Fracture.

In Emergency Surgery you may get so fixated on repairing the defect or correcting the error of your resident or student that you became oblivious that the patient is losing a lot of blood or is becoming hypothermic.

In surgery like in life, if we make immediate conclusions and run a pattern of focusing directly on the "problem at hand", this may result in tunnel vision that gives us blind spots. We always need to momentarily step back, have self reflection and see the big picture again to be better surgeons (and human beings).

This 61 year old lady presented with brow ptosis and dermatochalasis. I performed a brow lift and upper blepharoplasty (upper eyelid surgery) and here she is seen on post-operative Day 5.

One caveat to achieve natural and lasting results in periorbital surgery is to never perform upper eyelid surgery alone when the brow is low (brow ptosis) because this will later cause the eyelids to droop more paradoxically.
