Sleep-Related Hypoxemia: The Silent Nighttime Threat

Many patients experience significant oxygen desaturation during sleep—often unnoticed and undiagnosed. This condition, known as sleep-related hypoxemia, is more than just a number on an oximeter. It’s a clinical red flag tied to serious consequences like pulmonary hypertension, arrhythmias, cognitive decline, and reduced quality of life.

What is Sleep-Related Hypoxemia?
- Defined as sustained drops in SpO₂ < 90% for ≥5 minutes during sleep
- Not always caused by obstructive apneas
- Often overlooked in patients with COPD, ILD, OHS, and neuromuscular disease

At-Risk Populations:

* Patients with COPD (even with normal daytime oxygen)
* Those with overlap syndrome (COPD + OSA)
* People with ILD, neuromuscular disorders, or obesity hypoventilation

Diagnostic Tools:
✅ Overnight oximetry
✅ Polysomnography
✅ Capnography / ABG (in select cases)
