🫀 Common Conditions Leading to CABG

Coronary Artery Bypass Grafting (CABG) is often needed when blood flow to the heart is severely reduced. Here are the most common conditions that may lead to this surgery:

1. Coronary Artery Disease (CAD)
💓The most common reason for CABG.
💓Plaque builds up in the coronary arteries, causing narrowing or blockage → reduced blood flow to the heart muscle.

2. Severe Angina

Persistent chest pain that:
💓Occurs with minimal activity or even at rest
💓Does not improve with medication

3. Previous or High Risk of Heart Attack

💓When blood supply is critically reduced, parts of the heart muscle can become damaged.
💓CABG helps restore circulation and prevent further damage.

4. Multiple Blocked Arteries

💓When more than one coronary artery is affected (multi-vessel disease), CABG is often preferred over stents for better long-term outcomes.

5. Failed Angioplasty or Stents

Coronary Artery Bypass Grafting (CABG) Awareness Month!

What is CABG?

CABG is a type of heart surgery used to improve blood flow to the heart. It’s commonly done when the coronary arteries are narrowed or blocked due to plaque buildup.

🔍 Why is CABG needed?
💓When arteries supplying the heart become blocked, it can lead to:
💓Chest pain (angina)
💓Shortness of breath
💓Heart attacks

CABG creates a “bypass” around the blockage using a healthy blood vessel from another part of the body.

A 56-year-old patient presented with sudden episodes of vertigo when rolling in bed and looking upward. The symptoms were accompanied by nausea and brief loss of balance.

During assessment, the Dix-Hallpike test was positive, indicating posterior canal Benign Paroxysmal Positional Vertigo (BPPV).

The patient was treated with the Epley canalith repositioning maneuver during the session.

✅ Outcome:

Vertigo symptoms significantly reduced immediately after treatment.
By the follow-up appointment one week later, the patient reported complete resolution of dizziness and was able to return to normal daily activities without symptoms.

💡 Take-home message:
BPPV can often be treated quickly and effectively with targeted physiotherapy interventions, particularly repositioning maneuvers that restore normal inner ear function.

Research shows that repositioning maneuvers such as the Epley and Semont-plus are highly effective in treating BPPV, wit...

Research shows that repositioning maneuvers such as the Epley and Semont-plus are highly effective in treating BPPV, with most patients experiencing symptom relief within 4 weeks.

Physiotherapy exercises for vertigo:

1. Gaze stabilization:
Purpose: Improves coordination between the eyes and inner ear.

How to do it:
Sit or stand and hold a target (e.g., a letter on paper) at eye level.
Keep your eyes focused on the target.
Slowly move your head side to side while keeping the target clear.
Continue for 30 seconds.

2. Balance training:
Purpose: Improves balance and reduces fall risk.

Examples:
Feet together stance – hold 30 seconds
Tandem stance (heel-to-toe) – hold 30 seconds
Single-leg standing – hold 20–30 seconds

🧠 What Causes Vertigo?

Vertigo most commonly comes from the inner ear (vestibular system), which helps control balance.

Common causes include:

BPPV (Benign Paroxysmal Positional Vertigo) – tiny crystals in the inner ear move out of place
Inner ear infections or inflammation
Head or neck injuries
Vestibular migraines
Less commonly: neurological conditions

🚨 Common Symptoms
Spinning sensation
Nausea or vomiting
Loss of balance or unsteadiness
Symptoms triggered by head movements
Headaches or neck stiffness

Physiotherapy plays a key role in treating vertigo by:

Identifying the exact cause of your symptoms
Performing specific repositioning maneuvers (especially for BPPV)
Prescribing vestibular rehabilitation exercises
Improving balance, gaze stability, and confidence with movement
Reducing risk of falls
