Pharmacist - Nurse

Nurse: Good morning, Mr pharmacist; I need help on how to reconstitute and administer Vancomycin for a patient.

Pharmacist: Hey, good morning, sure; what dosage has the patient been prescribed?

Nurse: 1 g BID, IV, 14/7

Pharmacist: awesome! Vancomycin needs to be reconstituted to a final concentration of 5mg/ml for administration.
Here, we have a 500mg vial of vancomycin, which means your patient will need two per dose.

Step 1:

Add 10 mL of sterile water for injection (SWI) to a 500-mg vial to yield 50 mg/mL solution; further dilution is required. Repeat the same process with another vial.

Step 2: FURTHER DILUTION

Since your patient is receiving 1 gram, you will need to further dilute your two reconstituted Vails to obtain a final concentration of 5mg/ml.

To find the total volume needed to achieve 5mg/ mL
You can use this formula
Total volume = Dose/Final concentration
= 1g(1000)/5mg/mL
= 200ml
Since we already have 20 mLs containing the drug(10 mL by 2 vials we earlier reconstituted), we shall subtract this from 200mLs; then we shall need 180 mLs of NS.

Step 3. ADMINISTRATION

You will now get your 20mLs containing Vancomycin you earlier reconstituted and add to 180 mLs of normal saline in a burette or whatever giving set you have and run this for at least 2 hours.

IMPORTANT
1. Never give Vancomycin as an IV bolus;
2. Don't rapidly infuse Vancomycin, risk of red man syndrome and a greater propensity for AKIs.
3. Infusion rate depends on the dose for example, if the patient dose is 500 mg, this can be infused over an hour and for those who need 1g over 2 hours.

Doctor: Hi, I have a patient with sepsis; which antibiotics would you recommend and are available?

Pharmacist: hello Doc, what type of sepsis, what's the foci?

Doctor: Abdominal sepsis,

Pharmacist: Any perforation?

Doctor: Yes, this is a patient we were managing as a case of intestinal obstruction who has now developed sepsis with evidence of perforation.

Pharmacist: Oh okay! In this case, we have nosocomial sepsis and considering our hospital I recommend the following options

1. Piperacillin-tazobactam 4.5 g every 6 hourly
2. Imipenem/cilastatin 500 mg every 6 hourly
3. Meropenem 1- 2 g every 8 hourly

To either of the above add Vancomycin 1g of every 12 hourly

OR

We could use Cefepime plus Metronidazole plus Vancomycin.

Doctor: Oh, okay, thank you, why do you recommend the addition of vancomycin?

Pharmacist: In secondary abdominal sepsis ( with perforation), there is a need to cover for enterococcus and considering our antibiogram, we have beta-lactam resistance enterococcus; therefore, vancomycin will take care of that.

Doctor: Thank you so much.
